1 Signing up for the Donate Life Run/Walk 5K, 10K and Kids Dash Go to the Event Registration Site To sign up for the event, visit and click the green Register Here button. NOTE: Registration is required for the event and registration fees will apply. All registered participants will receive an event t-shirt, finisher medal, breakfast, music and more!! Click I Agree to accept the waiver and proceed to the registration page.

2 Select your preferred registration option. Choose your preferred registration option: Start a Team To start a new team of which you will be the team captain, select the Start a Team button & enter the following information: Team Name: Enter a team name. This will be displayed on your team page. Team names may not be derogatory or defamatory. Team Fundraising Goal: Enter the total fundraising goal for your team. All contributions made to you, your team members, and the general team fund will count towards this goal. Recruiting Goal: Enter the total number of team members you hope to recruit. Join a Team To join a team, select the Join a Team button and select the desired team from the dropdown menu Join as an Individual To join as an Individual - NOT a member of a team - select the Join as an Individual button.

3 Register Multiple People To register multiple people as part of a single transaction, select the Register Multiple People button. Please note that you still have to complete the full registration form for each individual person. Full payment for each registrant will be collected at the end. Important: This form assumes you have not yet registered and are the 1 st registrant. If you have already registered for this event, please register each person individually, rather than using the Register Multiple People option. Next, select one of the following options: Start a Team if you want all registrants to be added to a new team, with you as Team Captain. Create a team name, then complete the registration form for each person. Join a Team if you want to add all registrants, including yourself, to an existing team. Select your team, then complete the registration form for each person. Add Individuals if you want all registrants to be independently registered. In this case, there will be no association between these records; simply complete the registration form for each person. Create Username and Password: After you select your registration type, you will be prompted to create a username and password. Create a username and password by filling in the form and selecting a security question and answer. NOTE: If you have participated in the Donate Life Run/Walk in the past, you can use the same login and password. Select click here at the top to use this login information. You may also use your login information from Google, Yahoo, Facebook, or Twitter to register for the 5K. However, Donor Network West will not be able to assist in password reset or account troubleshooting if you register using one of these options.

4 Personal Information: In the Personal Information section, enter the following *required information: First Name Last Name Address *We strongly suggest using each person s individual address. If more than one person is registered using the same , it will be harder for Donor Network West to troubleshoot accounts, and the address in question will receive multiple copies of each event . Address Phone Number Gender Personal Page URL *you can edit your custom URL here but once entered here it cannot be changed or edited again. Registration Fees: In the Registration Fees section, you must select your registration fee, which is based on your age. You must also: Choose your t-shirt size Enter a discount code if you have one Make a personal contribution toward your overall fundraising goal (optional) Set your personal fundraising goal Click Submit to move to the billing information page. *If you are registering multiple people, follow the steps on the next page.

5 Registering Multiple People Only: After you submit each registrant, you will see the following options: Cancel: Click here to start the entire process over. Add Team Member: Click here to complete the registration form for the next person on your list. Continue: Click here when you have added all registrants and would like to finish and submit payment. Enter your billing information. The total of all registration fees and additional contributions will be displayed. After you click Continue, your registrations will be submitted and you will see a confirmation page. You will also receive a confirmation for all registrations to the address you indicated on the form when registering yourself. Billing Information: Enter your billing information. Your registration fee and any additional contribution amount will be displayed under Amount Due. After you click Continue, your registration will be submitted and you will see a confirmation page and will also receive a confirmation to the address you indicated on the form.
