4 Introduction 1. Introduction The Medicare Shared Savings Program (MSSP) allows Accountable Care Organizations (ACO) to share in the cost savings they achieve for the Medicare program. The Centers for Medicare & Medicaid Services (CMS) is committed to achieving: Better care for individuals Better health for populations, and Reduced expenditures for beneficiaries. One mechanism for achieving this goal is for CMS to partner with ACOs, which accept responsibility for the cost and quality of care outcomes for their patient populations. The ACO MSSP Communications and Dissemination Portal (CDP) provides helpful program-related information, events, FAQs, links to resources, training materials, and reports on cost-saving efforts to participating ACOs. The CDP Home Page offers four major features: Announcements, Events, Resources and Reports. Each feature will be described in detail in subsequent sections. ACOs use the CDP to receive information about their performance and to view and download reports and data sets related to their Shared Savings Program activities and performance. Each ACO can view only data and reports for their own ACO. Some ACOs employ consultants or delegates who use the CDP on behalf of the ACO. These delegates have the same access as the ACO users from the associated ACO. Moreover, some delegates represent more than one ACO; they can access data and run reports on behalf of any one of these ACOs. However, they cannot run reports or view analytics based on combined data from more than a single ACO. 2. Overview The purpose of the CDP is to provide: Information about the MSSP program to participating ACOs. Program resources such as FAQs and training materials to ACOs. Program reports to ACOs concerning their own performance. The CDP is a portal accessed through the CMS Enterprise Portal, which is hosted at the Baltimore Data Center. While the basic functions of the CDP are simple to understand and navigate, this user guide provides a walk-through of the major functions and screens. 5 Overview 2.1 Conventions The User Guide provides screen captures and corresponding narrative to describe how to use the CDP. The term portal is also used to refer to the CDP. Every effort has been made to keep the screen captures and formatting conventions used in this document up to date. In some instances, there may be minor differences between on-screen text and figures. These differences should not affect the user s ability to request access or perform activities. The following conventions are used in this user guide: Screen names: Bold black text; for example, My Home Page Hyperlinks: Blue underlined text Buttons: Bold italic text. Example: Next button Steps: Numbered steps identify the actions required to complete a task. System messages: Bold red text. Example: Error Message or Warning! 2.2 Cautions & Warnings Users must be aware of warnings regarding unauthorized access to information systems, computer usage and monitoring, and local system requirements. The user must read and agree to such notices before accessing the CDP via the CMS Portal. Users accessing the system will have their computer usage monitored and must acknowledge warnings regarding unauthorized access to the system. The user must read and agree to the Terms and Conditions for the CMS Portal, including the Privacy Act statement and the Rules of Behavior. Figure 1. Terms and conditions. 6 Getting Started Caution and Warning messages are presented in a variety of formats: as a text warning message at the top of the active screen, as information text on the screen where an issue has been identified, and as a caution message that requires the user s action. 3. Getting Started 3.1 Set-up and User Access Considerations To ensure access to the system and ensure proper function of the application, use Internet Explorer (version 9 or higher). The CDP may not display or function correctly with other browsers. Please disable pop-up blockers prior to attempting access to the CMS system. To disable pop-up blockers in Internet Explorer 9 or 10: 1. Click the Tools button, and then click Internet options. 2. On the Privacy tab, select or clear the Turn on Pop-up Blocker check box, and then click OK. Each ACO user is able to see only data and reports for their own ACO; consultants or delegates who are employed to use the CDP on behalf of an ACO cannot run reports or view analytics based on combined data from more than a single ACO. 3.2 Accessing the System The CDP is accessed through the CMS Portal. It is a web-based application that requires the user to authenticate access by providing his or her CMS User ID and password. CMS uses Enterprise User Administration (EUA) to manage User IDs and passwords. It allows for centralized administration of User IDs on the entire CMS enterprise portal. For new ACOs, requests for a CMS User ID and related access must be received via mail with original signatures. Form CMS must be completed and mailed to Centers for Medicare & Medicare Services, 7500 Security Blvd., Mail Stop: C , Baltimore, MD CMS Password Policy You must change your login password at least every 60 days. Your new password must: Be exactly eight characters in length. Include a mixture of letters and numbers. Not be changed more than once per day. Not contain a user name. Not begin with a number. Not contain four consecutive characters from the previous password. Be different from the previous six passwords. Not contain a reserved word. User IDs not active for a period of 100 days are revoked. When an additional 100 days of non-use has passed, the User ID is deleted from the system. After deletion, a new Application for Access to CMS Computer Systems form must be completed and approved to reinstate the User ID. 7 Getting Started Login When logging in to the CMS Portal landing page for the first time, the user has access to the following features as seen in Figure 2. Login to CMS Secure Portal A user with an established account may select the Login to CMS Secure Portal link; new ACO users should follow the instructions in Section 3.2 Accessing the System to acquire a CMS User ID. Forgot User ID? A user with an established account may select the Forgot User ID hyperlink to retrieve their CMS Enterprise Portal User ID. Forgot Password? A user with an established account may select the Forgot Password hyperlink to retrieve their CMS Enterprise Password. Figure 2. Use the CMS Secure Portal link to access the CDP. Users are not allowed to start concurrent sessions. If a user attempts to access the portal using an ID that is currently associated with another portal session, the portal will present an error message and deny access. Follow these steps to log in: 1. Navigate to the CMS Enterprise Portal at 2. Select the Login to CMS Secure Portal button; the CMS Terms and Conditions screen will appear. 3. Read the information on this screen. As a user of a U.S. government computer system, you need to be aware of and acknowledge certain terms and conditions regarding unauthorized access to those systems, computer usage and monitoring, and local system requirements. 8 Getting Started 4. Select the I Accept button if you accept the terms and conditions. (If you do not accept the terms and conditions click the I Decline button or exit from your browser, and your login will be canceled.) 5. Once the user has accepted the CMS terms and conditions, the system will display the Login screen as seen in the figure below. Figure 3. The Login screen for the CMS Enterprise Portal 6. Enter your 4-character CMS User ID in the User ID field. Click Next. 7. Enter your password in the Password field. Your Password is your 8-character password. Passwords are case-sensitive. Figure 4. On the CMS Enterprise Portal use the ACO/SSP drop-down menu (as seen below in Figure 5) to reveal the link to the CDP (ACO MSSP Reports), and select the link. Users representing more than one ACO will be prompted to select a specific ACO immediately after clicking the link to the portal. Users with only one ACO do not see this prompt. 3.3 System Navigation On-page navigation is standard. To view subsequent entries resulting from a search of the Events, Announcements or Resources when there are multiple pages, use hyperlinks (First, Previous, Next, Last) or the page number to navigate. (See Figure 6). Note: Do not use the back or forward buttons in the browser. 3.4 Exiting the System Users may exit the system at any time. Logout options are available to the user at any time once a user has logged into CMS. To log out, click the Logout option on the CMS Enterprise Portal toolbar near the upper right corner of the browser window to return to the CMS Enterprise Portal home page. The system will end the user's session after 30 minutes of inactivity, and the user will be required to log in again to the CMS Portal home page. 4. After a successful login using CMS credentials, select ACO MSSP Reports from the ACO SSP dropdown menu to go to the CDP Home Page. PLEASE NOTE: The user must access the CDP Home Page prior to accessing the Cognos reports. Going directly to the Cognos reports will result in an error when the user attempts to run a dynamic report. 10 Figure 6. The Home Page is the hub for CDP users. The CDP Home Page is the main user activity screen for the system. The initial view displays the announcements pane as the open tab. The home page has an accordion-style set of tabbed sections. Click on the section header to close the section that is currently open, and click on another section header to open that section. Clicking on the header of the open section will close it without opening another one. From the CDP Home Page, the user has access to the following features, which are briefly described below. Each feature is explained in greater detail in subsequent sections. Announcements The initial view provides a title, date and category. Select the title of the announcement to read more information. The resulting pop-up window includes the title, category, description (if available) and date for an announcement as well as keywords, attachments and links if they are relevant. Events The initial view provides a list of events showing the start date and time, end date and time, and title of the event. Select the title of the event to read more information. Resources Links to useful resources such as FAQs, training materials and websites, including both CMS websites and websites outside of CMS. 11 Reports Program Reports provide performance and trending information to each ACO indicating how that ACO is performing. Program reports are available in the static, historical data format and also as dynamic reports that provide simple trending information through the use of Cognos. 4.1 Using the Announcement Section Announcements are used to keep ACOs informed about changes to the Shared Savings Program and aware of activities and events related to the Shared Savings Program, including training opportunities. The initial view for announcements on the CDP Home Page is a list of all announcements with high priority announcements shown first and most recent announcements appearing before older announcements (see Figure 6). A maximum of 10 announcements is displayed; announcements with a high priority appear at the top of the list with the title in bold and an exclamation mark ( sort alphabetically or sort by date or category. ).Use the arrows at the top of the columns to Use hyperlinks (First, Previous, Next or Last) or page numbers at the bottom of the list to navigate and view additional pages. The list of announcements may be sorted by title (alphabetical or reverse-alphabetical order), by date (ascending or descending) or by category (alphabetical or reverse-alphabetical order). Click the arrow at the top of the column to sort the list based on that column; click again to reverse the sort order based on that column. Figure 7. 12 To view the details of an announcement: 1. Select the title. The resulting pop-up window provides a detailed description as seen in Figure 7, including the following fields. (Note: If the announcement is a high priority announcement, the phrase!!! High Priority!!! appears at the top of the announcement in red italics.) Date and Time: The date and time the announcement was posted. Category: Type of announcement (e.g., webinar or training). Title: The title of the announcement. Description: Details about the announcement. (This is not a required field, so the announcement may not include a description, especially if the title is sufficiently descriptive.) Keywords: Words or phrases used as keywords. Links: Web links (e.g., the URL for a webinar). Attachments: Attachments related to the announcement. 2. Relevant website URLs appear under the links section. Please note that you may be leaving the CMS site by following a link to a web page. 3. If desired, download materials such as Portable Document Format (PDF) files, spreadsheets or documents that are pertinent to an announcement in the attachment section by clicking on the Attachments links. 4. When finished, select the Close button to return to the CDP Home Page Search for an Announcement Announcements may be searched in several different ways: To filter for a specific category, use the drop-down menu under category to make a selection and click Search Announcements. To search for a specific word or phrase in the title or description of an announcement or to search for a keyword assigned to an announcement, type the word or phrase in the Keyword search box and select Search Announcements. To search for announcements during a specific time period, set the date fields and click Search Announcements. Click the title in the resulting list to view the details for a particular announcement. Note: If the announcement is a high priority announcement, the announcement appears at the top of the list of search results with an exclamation mark ( ) in the priority column; in addition, the phrase!!! High Priority!!! appears on the announcement detail page in red italics. To navigate to additional pages resulting from a search, use the hyperlinks (First, Previous, Next or Last) or page numbers at the bottom of the list. Select a date range, category and/or specific word, phrase or keyword to search announcements. 4.2 Using the Events Section The events section is used to keep ACOs informed about upcoming events, such as webinars and Open Door Forums, and provides a source for other information designed to help ACOs understand and perform well in the Shared Savings Program. Events are displayed for the day and time for which the event is active. All event times are displayed in Eastern Time. High priority events in the list appear with the title of the event in bold. As seen in the figure below, the initial display of the Events section includes a graphical calendar showing a six-month period. 14 Figure 9. The events section. Each event in the list of upcoming events has a title, a start date and time and an end date and time. The list shows events that occur today and up to 30 days in the future in reverse chronological order by the start date and time. Selecting the title of an event in the list will show the detailed information for that event in a pop-up window as seen in the figure below. Links: Web links (e.g., the URL for a webinar). Use the Search Events button to search for events. 4.3 Using the Resources Section The resources section of the CDP Home Page provides a collection of resources for ACOs including: Documents such as FAQs and guidance and training documents related to the Shared Savings Program, and Links to other useful CMS and non-cms websites. Use the Search Resources button to locate a resource. 4.4 Using the Reports Section Overview of Static Reports Shared Savings Program reports have been developed to help participating ACOs understand and track their performance. These static reports have fixed formats and content that cannot be changed or modified by the user. The reports are prepared and distributed periodically to the ACOs who receive these reports in their Managed File Transfer (MFT) mailboxes, but they are also available online. The CDP provides a convenient, self-serve method for the ACOs to view and download these reports as needed. ACO users may only view reports and data for their ACO. The Reports section of the CDP Home Page provides access to program reports. Most static reports are formatted as Excel workbooks or PDF documents. However, some of the newer files listed (starting in 2015) are zip files containing multiple related reports. To download a static report that has been zipped, select the.zip file extension from the Save File dialog box. An application such as WinZip or 7zip is required to open these files. To assist users in locating static reports, a crosswalk spreadsheet is available to identify the period of time that a report covers, the date on which the report was released and other information. This crosswalk spreadsheet is available to all users in the Resources section. Search for the keyword crosswalk to locate this resource; the spreadsheet will be available as an attachment. Any report may be transferred by MFT, but reports containing personally identifiable information (PII) or protected health information (PHI) cannot be downloaded. 19 The CDP also provides some of these program reports in a slightly different form that allows side-by-side comparisons across quarters or across years. These are the dynamic (Cognos) reports that are described in Section Types of Static Reports The following static reports are available: The Annual and Quarterly Assignment List Report lists assigned beneficiaries and the number of primary care services at the ACO participant TIN level. Early in the agreement period, ACOs receive a preliminary prospective list of assigned beneficiaries based on the most recent benchmark year. Quarterly preliminary prospective assignment lists are based on a rolling 12 months of data ending in the current quarter. Finally, a list of retrospectively assigned beneficiaries for the performance year is provided at financial reconciliation. The Annual and Quarterly Assignment Summary Report is an aggregated report of the demographics and eligibility characteristics of assigned beneficiaries and provides counts of beneficiaries and percentages of annual assignment by classification. The Annual and Quarterly Expenditure/Utilization Report provides non-risk adjusted measures of expenditure and utilization calculated for the ACO s assigned beneficiaries. All available data is used for assigned beneficiaries, including care received from providers outside of the ACO. Annual reports are provided for each benchmark year and for each performance period and are based on a three month claims run-out. Quarterly reports are provided for each quarter during the agreement period and are based on a rolling 12 months of data ending in the current quarter with at most a seven day claims run-out. The Financial Reconciliation Report is provided to ACOs to specify the calculation of the ACO's updated benchmark and determination of shared savings/ losses. The Quality Reconciliation Reports include the following: 1. Consumer Assessment of Healthcare Providers and Systems (CAHPS) surveys ask consumers and patients to report on and evaluate their experiences with health care. These reports provide detailed results about your ACO s performance on the CAHPS for ACOs survey, including information on the Patient/Caregiver Experience measures and the survey questions that comprise them. 2. Quality Performance Reports enable you to see how your ACO performed on a set of quality metrics in a year, and compare how your ACO performed relative to the average performance of other SSP ACOs. This information will help ACOs identify areas for quality improvement in the next reporting period. This report is also available as a dynamic report; see Section 4.5, Using Dynamic (Cognos) Reports. Claims and Claims Line Feed Reports: The CCLF data files contain Medicare fee-for-service claims data for the ACO s assigned beneficiaries. Data provided includes Part A claims, Part B claims, Part D claims, beneficiary demographic data, and a beneficiary ID cross-reference file. Claims data provided is from both the ACO s participating providers as well as non-aco providers. Claims and Claims Line Feed Reports cannot be downloaded or viewed directly in your browser. These reports can be sent to your MFT mailbox where you can retrieve them. Historical Benchmark Report: To share in savings, an ACO must generate savings that meet or exceed the minimum savings rate (MSR) compared to its benchmark and meet the quality User Manual for ACOs-Version ACO MSSP Communications and Dissemination Portal

20 performance standard. A historical benchmark is established for each ACO based on the expenditures for its historically assigned patient populations for each of the three years preceding the start of the agreement period. When an ACO changes its participant list, an adjusted historical benchmark based on the same 3 benchmark years is calculated for the performance year in which the participant list change becomes effective. For each performance year, the historical benchmark report is updated each performance year using a national update amount and then adjusted for changes in health status. Beneficiary Data Sharing Exclusion File Report: All Beneficiary Data Sharing Exclusion Files distributed since February 2016 are available in XML form for download by ACOs. These files allow ACOs to reference previous exclusion files in one place to aid in ongoing analysis regarding beneficiaries who have opted out of data sharing. Exclusion files may also be re-sent to your MFT mailbox by selecting the desired file and clicking, Send to MFT Searching for a Static Report Unlike the announcements, events, and resources, the reports section does not show any reports initially. To see a list of reports, you must use the search option. To see a list of all the reports for your ACO, you can leave the start and end date fields blank and select All Reports for the category then click the Search Reports button. Figure 14. Static reports may be downloaded. The list of static reports can be filtered by limiting the date range and/or selecting a report category. Follow these steps to search for a static report. 21 1. Enter a start date for the search in the Start Date field. 2. Enter an end date for the search in the End Date field. 3. Use the drop-down menu to select the type of report. 4. Select the Search Reports button. 5. Use the navigation aids at the bottom of the section to view additional pages returned in the search. Figure 15. Limit the date range and select a category to filter the list of static reports. To send a report to MFT, follow these steps: 1. Check the box adjacent to the report(s) you want to send. 2. Select the Send to MFT button. Note: This sends the report to the ACO s MFT mailbox. Note: Multiple reports may be selected but the user must make selections on an individual screen and click on Send to MFT before moving to the next screen. To download a report: 1. 22 2. Depending on your browser settings, the report will download automatically to your computer or a prompt will appear to ask you whether you wish to open or save the file. Reminder: As discussed earlier in this section, some of the newer reports (starting in 2015) are zip files containing multiple related reports. An application such as WinZip or 7zip is required to open these files. Figure 16. The reports section features static reports that may be downloaded. 4.5 Using Dynamic (Cognos) Reports Overview of Dynamic Reports Dynamic reports are created through an IBM product called Cognos and can be viewed, downloaded and printed by the user. The dynamic reports are based on the same data as the static reports but they provide additional capabilities such as side-by-side comparisons across years or quarters and charts and graphs for visualizing the trends and comparisons. Dynamic reports may be customized by the user by selecting, for example, the data and graphs to be included. These reports are not prepared in advance and distributed to the ACOs periodically; the ACO users must use the CDP to generate these reports. The data sets for the dynamic reports are updated periodically on approximately the same schedule that the static reports are delivered. The content of each report is determined by parameters selected by the user. ACO users may view reports and data only for their own ACO. 23 Quality Measure Performance Full Quality Report Report descriptions and how to access, specify parameters, run and save a report are described in detail in the following sections. User guides for the Annual and Quarterly Aggregate Expenditure Utilization Report and Assignment Summary Report are available as Resources in the CDP. Search for the phrase User Guide in the Resources section to locate these helpful guides. A quick-start guide for dynamic (Cognos) reports is located in Appendix C Accessing the Dynamic (Cognos) Reports Cognos, which is used to run the dynamic reports, has many features, but only a portion of those are used by the CDP. It is important to follow instructions and use the commands as directed. (Note: To ensure access to the system and proper function of the application, please use Internet Explorer. 24 6. On the CMS Enterprise Portal use the ACO SSP drop-down menu (as seen below in Figure 30) to reveal the link to the CDP (ACO MSSP Reports), and select the link. (The user must access the CDP Home Page prior to accessing the Cognos reports. Going directly to the Cognos reports will result in an error when the user attempts to run a dynamic report.) 7. Users who work with multiple ACOs should select one ACO and click the Continue button. Users who work with only one ACO do not see this screen. 8. 26 HTML reports will return data quicker than PDF, but users will have to navigate through the report pages with the links at the bottom left of the page. (The user may also specify the output format when selecting parameters on individual reports.) Click on the name of a report to open the report screen and select parameters for the report Running a Dynamic (Cognos) Report When using the IBM Cognos Connection to run and view dynamic reports, you may see the message Your report is running This message will usually appear when a selection is made (or shortly after a selection is made); no action is required. Dynamic reports are based on the same data as the static reports, and, like static reports, the dynamic reports are available as annual or quarterly versions. The parameters for the different dynamic reports are usually specified in the same order: select an ACO, annual or quarterly, report years or quarters (if quarterly is selected instead of annual), and charts or graphs to be included. Most reports have a cover page with the name of the report, ACO ID number, performance year, and report year; this page is followed by the glossary page(s), parameters page(s), the report and footnotes. For most reports, the graphs appear at the end of the report. The exception is the Quality Measure Performance Report, which is a set of stand-alone graphs. The following sections describe the content of each report and when it should be used as well as step-bystep guidance on setting parameters Financial Reconciliation Report, Tables 2 and 3 This tabular report is based on the information in the static Financial Reconciliation Report, Tables 2 and 3. Table 2 shows the calculation of an ACO s updated benchmark for the selected performance year. Table 3 shows the calculation of an ACO s shared savings or losses, if any, for the selected performance year. This dynamic report allows users to include and directly compare data from multiple performance years in the same table. Use this report to understand how your ACO s updated benchmark (Table 2) and savings performance (Table 3) are determined and how they have varied over time Specifying Report Parameters To specify the report parameters: 1. 35 4.5.6 Expenditure Utilization Report All ACO Distributions This graphical bar chart report is based on the information in the static Aggregate Expenditure/Utilization Report, allowing for the selection of annual or quarterly data. Unlike the static report, the dynamic report displays ranked values for all ACOs in the program for a selected performance period and measure. Use this report to graphically compare your ACO s expenditures and utilization to all ACOs. ACOs in Track 1 and Track 2 are shown together with other Track 1 and Track 2 ACOs while ACOs in Track 3 are displayed together with other Track 3 ACOs. The percentile ranking is also calculated based on these groupings. The percentile ranking for a Track 1 or Track 2 ACO is calculated from the group of ACOs in Track 1 and 2. The percentile ranking for a Track 3 ACO is calculated from only the group of other Track 3 ACOs Specifying Report Parameters To specify the report parameters: 1. Select an ACO. This dynamic report reveals trends by allowing direct comparisons for selected annual or quarterly periods. Optional line graphs can be generated for selected measures. Use this report to view your own ACO s expenditures and utilization over time in tabular or graphical form and to compare your ACO s trends with trends in the all-aco median and the national fee-for-service population. The HTML version of this report also provides links that allow you to drill down into certain available report categories to show the beneficiaries included in the selected category. Time periods where drill-down data are available can be found on the report prompt page. Annual expenditures are computed from the benchmark year, allowing for a three-month claims run out and use of a completion factor to complete claims to 100 percent. Total expenditures are annualized and weighted for months of beneficiary ACO eligibility, and extreme outliers are truncated. Reported expenditures are the per beneficiary per month amount multiplied by 12 (adjusted for outliers). Total expenditures take into account individually identifiable payments made for beneficiaries under a demonstration, pilot or time-limited program. Annual component expenditures (inpatient, outpatient, etc.) are calculated in the same manner as total expenditures. All hospitalizations include short-term acute care, psychiatric, rehabilitation, long-term care, and critical access hospitals Specifying Report Parameters The Expenditure Utilization Report Comparative Trends Report allows the selection of annual or quarterly data, the comparison of ACO specific, all ACO median and Fee for Service values according to the selected performance period, and, if annual, comparable report years with the same participant list. The resulting report is generated in HTML format and contains measure level Quality data for the Domain and Report Year corresponding to the selected Points value. The sub-report has columns for Measure Number, Measure Name, Numerator, Denominator, Your ACO Performance Rate, Pay for Performance/Pay for Reporting Indicator, Actual Points Earned Per Measure, and Possible Points Per Measure. A summary shows the Total Points Earned in the Domain as well as the Total Points Possible in the Domain. A Domain Score is included as a percent. Use this report to graphically compare your quality measure performance rates with the ACO mean and 30th and 90th percentile benchmarks. This section describes the steps involved in saving your report in PDF and Excel format. Reports in HTML should be converted first to PDF or Excel format and then saved in that format. The format of a report be set on the Report Descriptions page, but this function is available only prior to running a report To Save a PDF Report To save a PDF report, the user must have the report open in that format. You have a choice of two options for saving your PDF report: 1. Click on the report and use Shift+Ctrl+s to save a copy of the report in PDF to your computer. 2. Or click the save icon (the floppy disk image) in the floating tool bar as seen in the figure below. The tool bar may disappear but will reappear when the user moves the mouse cursor over the report. Select one of these three methods: Method 1: Save it immediately when you convert it to Excel format, rather than opening it, as seen in the figure below. If you are downloading the report and the Save File dialog box appears, select the icon to save the report and chose the folder where you want to save the report. 4.7 Converting the Format of a Dynamic (Cognos) Report The default format for viewing most reports is PDF, but other formats are available, including Hypertext Markup Language (HTML) and Excel 2007 Format. Reports that default to other formats may also be converted to a different format (e.g., HTML to PDF and/or Excel). XML or Excel formats other than Excel 2007 are not useful and are not recommended. The format of a report may be set on the Report Descriptions page, but this function is available only prior to running a report. Additional provisions have been made to allow the user to determine the format for each report on the parameters page for the specific report. Reports appear in a new window; return to the Cognos window to access tools to change the format. The following guidance is offered for users who have a report run in one format but would like to view it in another format Convert PDF to HTML Format To change the view of a PDF report to HTML, follow these steps: 1. Select the PDF icon at the top of the screen as seen in the figure below. 2. Select View in HTML Format in the drop-down menu. 57 Quick Start Guide Using the Communications and Dissemination Portlet Log In 1. Go to 2. Select Login to CMS Secure Portal. 3. Accept terms and conditions. 4. Log in with CMS user ID and password. 5. Select ACO MSSP Reports in the drop-down menu under ACO/SSP. Home Page The CDP Home Page has four sections: (1) Announcements, (2) Events, (3) Resources and (4) Reports. Click on a section header to open a section. Announcements The default setting for CDP Home Page is an open tab for announcements and a display of 10 announcements. 1. Click the arrow/triangle icons at the top of columns to sort announcements. 2. Select a date range or category, and click the Search Announcements button to filter announcements. 3. 60 Log In Quick Start Guide Dynamic (Cognos) Reports 1. Go to 2. Select Login to CMS Secure Portal. 3. Accept terms and conditions. 4. Enter your 4-character CMS User ID and click Next. 5. Enter your password and click Log In. 6. Select ACO MSSP Reports in the drop-down menu under ACO SSP to access the CDP Home Page. (The user must be on the CDP Home Page to proceed.) 7. Select Cognos under ACO/SSP to access the Dynamic (Cognos) Reports. Note: To run a different type of report in Cognos, select the Return button at the top of the screen; to run another report of the same type, select the Run Report button:. Run a Report Financial Reconciliation Report, Tables 2 and 3 This tabular report is based on the information in the static Financial Reconciliation Report, Tables 2 and 3. Table 2 shows the calculation of an ACO s updated benchmark for the selected performance year. Table 3 shows the calculation of an ACO s shared savings or losses, if any, for the selected performance year. This dynamic report allows users to include and directly compare data from multiple performance years in the same table. Use this report to understand how your ACO s updated benchmark (Table 2) and savings performance (Table 3) are determined and how they have varied over time. 1. Select the Financial Reconciliation Report, Tables 2 and 3, on the IBM Cognos Connection screen. 2. Select your ACO; for those with multiple ACOs, select only one ACO. 3. Select a Performance Year(s). 4. Click Finish as PDF to generate the report in PDF format, which is the recommended format. Other formats are available.
