1 STEP-BY-STEP USER GUIDE Print it Share it Save it INTRODUCING THE FIRST EVER Anthem Mobile Provider Finder Finding a Doctor Just Got Easier Find a Doctor, Get Directions... FINDING A DOCTOR JUST GOT EASIER Now you can search for a health care provider from your Web-enabled cell phone, BlackBerry TM, PDA or similar hand-held device. It s called the Anthem Mobile Provider Finder, and with it, you can search for a doctor or hospital by name or by location. And you can further refine your search by provider type, specialty, gender, other languages spoken, whether the provider is accepting new patients, or whether the provider can serve as PCP. Once you find your health care provider, Anthem Mobile Provider Finder can automatically call your health care provider, get directions to the office, or add your health care provider to your contacts list. 1
2 STEP 1 From Your Web-Enabled Device: Visit Choose the Anthem Mobile Provider Finder link. Mobile Provider Finder STEP 2 Select Your State: Use the dropdown menu to select your state. Choose Next State selection: Connecticut STEP 3 Choose Your Plan: Use the dropdown menu to select your health plan. Choose Next Plan selection: Select a plan NOTE: When making an appointment, always ask if the provider is still participating in your plan. STEP 4 Select the Type of Search you d like to execute: Search for providers by location or name by selecting one of the links. Search type selection: Location search Name search 2
3 STEP 5 Search by Location: OR Search by Name: * Provider type: * Last or facility Name: a minimum of a city and state OR ZIP is required minimum of two characters required First Name: Address: City: Address: City: State: Zip: -or- -or- State: Zip: Mileage: Mileage: Provider Type: *required Select a provider type from the dropdown menu. Enter an address (optional). Enter a city and state or enter a ZIP code. Use the mileage dropdown menu to select the radius of your search (optional). Choose Search to find providers that meet your criteria. Or choose More Options to enter more search criteria. Enter at least the first two letters of a provider s last name or a facility name. Enter the provider s first name (optional). Enter the provider s address (optional). Enter a city and state or enter a ZIP code. Use the mileage dropdown menu to select the radius of your search (optional). Select a provider type (optional). Choose Search to find providers that meet your criteria. Or choose More Options to enter more search criteria. 3
4 STEP 6 Search Results: The Search Results display the providers that match your search criteria. Choose a provider s name for details. Choose the More button to go to the next page of results (if available). Choose New search to start a new search. Results: 1 Smith, Samuel T. MD 2 Jones, Michelle S. MD 456 Any Name Rd. Any Town, ST [5.4] STEP 7 Provider Details: The Provider Details page displays the provider s phone number and details such as specialty and if a doctor is accepting new patients. Choose the phone number to automatically call the provider. Choose the Directions link to get driving directions to the provider s location. Choose the Add to Contacts link to automatically add this provider to the contact list on your mobile device. Provider Details: Smith, Samuel T. MD Specialty: Endocrinology Accepting New Patients (123) Call Provider at (123) Directions Add to Contacts NOTE: Adding providers to the contact list may not work on all mobile devices. 4
5 STEP 8 Get Directions: Enter your starting location. A minimum of city and state or ZIP code is required. To get more accurate results, enter a full address. Choose Get Directions Starting address for directions: a minimum of a city and state OR ZIP is required Address: City: State: -or- Zip: The Directions Result Page: The result page provides step-by-step driving directions from your location to the provider s location. Directions: Smith, Samuel T. 6 MORE OPTIONS The Anthem Mobile Provider Finder allows you to hone your search even further with the following options: Accepting new patients: Check this box if you want to find providers who are accepting new patients. This checkbox will only be displayed for some plan and provider type combinations. Able to serve as my PCP: Check this box if you only want to find Primary Care Physicians. This option will only display for certain plans and if Physicians or No preference was selected as the provider type on the Search page. Gender: Narrow your search by looking for only male providers or only female providers. Search for all providers by selecting the default value. Additional Search Criteria: Accepting new patients Able to serve as my PCP Specialty category: Gender: Other languages spoken: Other Languages: You can search for providers who speak a language other than English. Select a language from the dropdown menu to narrow your search. 6
