Each module is broken into sections based on specific tasks you can perform. Use this workbook as a reference. Introduction to SMART Forms SMART forms are Microsoft Word-based form and letter templates. You can use SMART forms to create electronic versions of standard forms, such as lab requisitions and government forms, which you can open and fill out in Wolf EMR. You can also use SMART forms as letter templates for consult letters and referral letters. Using SMART forms, you can: Pre-populate select patient demographic and other medical information from the EMR into an electronic version of a form or letter. 7 SMART Forms Workbook.book Page 3 Monday, October 26, :44 AM Introduction Wolf EMR contains SMART forms for the most common provincial and regional requisition forms. SMART forms are stored in the Wolf EMR data center, and are available to all Wolf EMR clients. If the Wolf development team creates a SMART form for other clinics in your region, the form is automatically available to you from your SMART forms list. Using Microsoft Word, you can modify SMART forms provided by TELUS Health. SMART forms you modify are available only to your clinic, and cannot be deleted or updated by TELUS Health. If you have other form needs, in this course you learn how to build the forms yourself in Microsoft Word. If a form is complex, and you need help, you can hire TELUS Health technical staff to build forms for you. For more information, contact the Wolf EMR support team. Wolf EMR SMART Forms - Issue

You explore clinical situations where order sets can be beneficial, and learn how to create your own order sets. Objectives Upon completion of this module, you will be able to: Describe what an order set is Create a basic order set Identify the location of the Wolf Master Templates Directory folder, and the location of your User Templates folder. What is an order set? If you fill out certain requisition forms with similar information for specific groups of patients (for example, you select the same core lab tests for most diabetic patients), you can create customized copies of these SMART forms with check boxes and text fields pre-populated. Customized, pre-filled versions of SMART forms are called Order Sets. Wolf EMR SMART Forms - Issue

House forms include all SMART forms that have been created or modified by your clinic. Order sets and other house forms are not maintained or updated by TELUS Health. When you use an order set for a patient, you can edit the pre-populated data as needed before you print and save the form. 6 Wolf EMR SMART Forms - Issue 01.02
11 SMART Forms Workbook.book Page 7 Monday, October 26, :44 AM Creating order sets Creating a basic order set You create an order set by copying the SMART form s Microsoft Word Template file from the Wolf Master Templates Directory, and then pasting the template in your User Templates folder with a new name. Copying template files from the Wolf Master Templates Directory folder To open the Wolf Master Templates Directory folder: On your data center desktop, click the Start menu, and then click Computer. In Windows Explorer, click Local Disk (C:) > Program Files (x86) > Wolf > Wolf Lair > Data > Templates To copy the file, right-click the file name, and then click Copy (<Ctrl><C>). There are four file types for each form. Ensure that you copy the Microsoft Word Template file. Pasting template files to your User Template folder To open your User Template folder: On your data center desktop, click the Start menu, and then click Computer. In Windows Explorer, click W: > Wolfdata > Templates To paste the file, right-click the folder window, and then click Paste (<Ctrl><V>). Save all order set SMART forms in the Order sets folder. If an Order sets folder is not available, create a new Order sets folder. From the User Template folder, open the form (right-click and select Open), fill in the form with the default information you want, and then save the form. The order set is now available to use in Wolf EMR. Wolf EMR SMART Forms - Issue
12 SMART Forms Workbook.book Page 8 Monday, October 26, :44 AM Creating order sets Tips for creating order sets Naming order sets: Use a name that describes the purpose of the order set (for example, Male Diabetic over 50 Lab req ), also if only one user will be using the order set, consider including the user s initials in the name. To open and edit the Word template, right-click the file, and then click Open. Avoid double-clicking the file to open it, or the changes you make will not be saved. You can create as many order sets as you want for one SMART form. If you just created an order set and it is not available on your SMART forms list, close the Workdesk and re-open it. You can make an order set into SMART form favourite. Practise: Creating an order set 1. Open the Wolf Master Forms Directory folder. 2. Open your User Templates folder. 3. In the Wolf Master Directly folder, find and copy the SMART form s Microsoft Word Template file. 4. Go to your User Templates folder, and open the Order sets folder. If an Order sets folder is not available, create the folder. 5. In the Order sets folder, paste the copied SMART form template file. 6. Rename the SMART form template file. 7. Open the renamed SMART form (right-click > Open). 8. Fill-in the SMART form with the default information you want. 9. Click Save ( ). 8 Wolf EMR SMART Forms - Issue 01.02

For your standard lab requisition, you create an order set to use for female physicals. When TELUS Health later updates the lab requisition with an updated version, is the order set updated too? 3. When you create an order set, can you: a) Modify the form itself? (For example, if you want to update the company contact information) b) Modify what data is pulled from the EMR into the form? (For example, if you want a patient s consultant MD information to pull instead of the patient s family MD) 4. You create a number of order sets that only you will be using. Upon completion of this module, you will be able to: 1. Create a SMART form for a letter 2. Program a SMART form to pull data from the EMR using bookmarks 3. Create a SMART form for a requisition form 4. Add user interface elements, such as check boxes and text fields, to SMART forms. 5. Restrict a SMART form to be fill-in only Configuring Microsoft Word for SMART forms development Before you create a SMART form, in Microsoft Word, you must activate the Developer tab, and enable bookmarks. To activate the Developer tab, click File > Options > Customized Ribbon, and then in the Main Tabs area, select the Developer check box. The Developer tab enables you to: Add user interface items, including check boxes, text fields, and dropdown lists Add editing restrictions To enable bookmarks, click File > Options > Advanced, and then in the Show document content area, select the Show bookmarks check box. 16 SMART Forms Workbook.book Page 12 Monday, October 26, :44 AM Creating SMART forms Creating SMART forms for letters You can create letter SMART forms to use as templates for consult letters, referral letters, or other types of letters. Simply write a letter in Microsoft Word (using the various Microsoft Word tools to format your letter), and then save the Microsoft Word document in your User Template folder as a Word Template file. Your User Template folder is located in the following location on the data center: W: > Wolfdata > Templates Save all letter SMART forms in the Letter folder. If a letter folder is not available, create a folder called Letter. Adding bookmarks You can use bookmarks to program a SMART form to populate patient, clinic, practitioner, and addressee data automatically. To add a bookmark: 1. Open Wolf online help, and then search for and select the Bookmark help topic. 2. In the Bookmark topic, find the bookmark name corresponding to the information you want to pull. 3. In the Word document, place your cursor where you want the bookmark, and then click Insert > Bookmark. Copy and paste the Wolf bookmark name. 17 SMART Forms Workbook.book Page 13 Monday, October 26, :44 AM Creating SMART forms Tips for creating a letter SMART form If you want your letterhead to display on all pages of the letter, click Insert > Header & Footer > Header, otherwise, enter your letterhead on the first page in the regular editing area. If you want to insert the date that the letter is used, click Insert > Quick parts > Fields > Create Date. If you want to see what a particular bookmark is, place your cursor to the right of the, and then click Insert > Bookmark. If you want some letter content to display in columns (for example, for patient information to display in a more compact manner in the Re line), enter the content in a table, and then hide the borders. To insert an MD s electronic signature, insert the MDSignature bookmark. For additional information on how to use tools in Microsoft Word to format your letter, you can ask a question and get help using Microsoft Word Help ( ). Leave letter SMART forms unrestricted (avoid setting them to be fill-in only). This enables users to add or modify text in all areas of the letter. If you want to set a SMART form to be your clinc s default referral letter template, name the SMART form Consult letter. Wolf EMR SMART forms - Issue

19 SMART Forms Workbook.book Page 15 Monday, October 26, :44 AM Creating SMART forms Scenario 1: Creating a letter SMART form You are creating a letter SMART form to be used as your clinic s referral letter template: a) Under what Bookmark category can you find bookmarks to pull the consultant s (addressee) name and contact information? b) If you want to enable users to CC a practitioner on the letter, under what bookmark category can you find bookmarks to pull a CC d practitioner s name and contact information? c) What name must you give the SMART form for it to become your clinic s default referral letter SMART form template? Creating SMART forms for requisition forms and other entry forms (fill-in only forms) You can create fill-in only SMART forms, where users can input information only in designated text fields, check boxes, and drop-down lists. Users cannot edit the content of the form itself. Fill-in only SMART forms are most often created for electronic versions of standard forms (such as requisition forms and government forms). To create a fill-in only form, build the form in Microsoft Word (using the various Microsoft Word form tools available), and then save the Microsoft Word document in your User Template folder as a Word Template file. Your User Template folder is located in the following location on the data center: W: > Wolfdata > Templates Save all requisition SMART forms in the Requisition folder. Using bookmarks, you can program fill-in only forms to automatically populate data, removing the drudgery of filling out forms manually. See Adding bookmarks on page 12. Users cannot edit information pulled in through bookmarks. Using tables to format SMART forms Many forms contain a variety of data that is separated into various organized columns and rows. Using Microsoft Word, the easiest way to create these forms is to build content within tables. Tables enable you to: Line up groups of content Wolf EMR SMART forms - Issue
20 SMART Forms Workbook.book Page 16 Monday, October 26, :44 AM Creating SMART forms Line up individual items, such as check boxes Force inputted data to stay within defined regions You can hide table borders where you don't want table lines to display, or modify border properties to highlight specific areas. You can also split or merge table cells, and adjust individual cell widths. To insert a table, click Insert > Table. 16 Wolf EMR SMART forms - Issue 01.02
21 SMART Forms Workbook.book Page 17 Monday, October 26, :44 AM Creating SMART forms Tips for using tables to format a SMART form If the form you are creating is wide, narrow the document margins. Click Page Layout > Margins > Narrow. To center the table on the page, right-click the table, and then click Table Properties > Table > Center. If you want to change a column width, click and drag the column border. If you want to change a row height, click and drag the bottom border of the row. To hide table borders, highlight the cells you want to modify, and then right-click the highlighted area. Click Borders and Shading ( ). To merge (combine) table cells, highlight the cells you want to merge, and then right-click the highlighted area. Click Merge Cells ( ). To split a table cell into one or more cells, right-click the cell, and then click Split Cells ( ). To add a row to the table, right-click the row above or below where you want the row to insert, and then click Insert > Insert Rows Above (or Insert Rows Below) To prevent table columns from expanding when users enter data, turn off auto-sizing. Right-click the table, and then click Table Properties > Table > Options. Clear the Automatically resize to fit contents check box. To maximize the amount of space you can use in a cell for content, narrow the cell margins. Right-click the table, and then click Table Properties > Table > Options. Decrease margin sizes in the Default cell margins area. Adding static elements to a SMART form When you create a fill-in only form, any content you add, that is not a text field, check box, or drop-down list, cannot be edited by the user and is therefore considered to be a static element. Commonly used static elements include text, bookmarks, pictures, shapes, and digital signatures. Wolf EMR SMART forms - Issue
22 SMART Forms Workbook.book Page 18 Monday, October 26, :44 AM Creating SMART forms Tips for adding static elements to a SMART form Static text: Use the Arial Narrow font with a size of 10pt or 11pt. If, in the middle of a sentence, you want patient-specific or practitionerspecific data to pull automatically, insert a bookmark. Pictures: The image file must reside on the data center. You can copy and paste the picture file from your local computer to the data center. The file size should be as small as possible. The size of your entire SMART form should not exceed 200kb. To ensure that the picture quality is sufficient, test print the form. For clinic logos and other non-photographic images, use.gif or.png files (avoid using.bmp files). Electronic signatures: To insert a practitioner s electronic signature, use the MDSmallSignature bookmark. Adding user interface elements to a SMART form When you use a fill-in only form, you can enter data only in designated text fields, check boxes, and drop-down lists. 23 SMART Forms Workbook.book Page 19 Monday, October 26, :44 AM Creating SMART forms To insert a user interface item, click the Developer tab, and then in the Controls area, click Legacy Tools ( ). Use only tools that are available in the Legacy Forms area. CAUTION: Do not select items in the ActiveX Controls area. You can use only Legacy Forms items for SMART forms. The following table describes the more commonly used Legacy Form items: Item Description Tips Text Form Field Insert where you want users to If users may enter a large amount of manually enter text. 24 SMART Forms Workbook.book Page 20 Monday, October 26, :44 AM Creating SMART forms Item Description Tips Drop-Down Form Field ( ) Insert where you want users to select only one option. Users cannot select multiple items in a drop-down list. To define what items are to display in the list, right-click the item, and then click Properties. Tip: Make the first item on the list an instruction to the user. For example, Choose an option..., or Select a gender Restricting a SMART form to be fill-in only A SMART form becomes fill-in only when you add editing restrictions to the form. If you later want to modify the base form, you must remove editing restrictions first. To set a SMART form to be fill-in only: 1. Click the Developer tab, and then click Restrict Editing ( ). 2. In the Restrict Formatting and Editing pane, select the Allow only this type of editing in document check box and then, in the drop-down list, click Filling in forms. 3. Click Yes, Start Enforcing Protection. When you are prompted to create a password, leave the password fields blank. You are not required to create a password for a form. Recommendation: Avoid adding a password to your SMART forms or you risk losing editing access to the forms if you forget the password. 20 Wolf EMR SMART forms - Issue 01.02

26 SMART Forms Workbook.book Page 22 Monday, October 26, :44 AM Creating SMART forms Scenario 2: Creating a requisition SMART form You are creating a SMART form to be used as an electronic version of a diagnostic imaging clinic form: a) The form contains required fields that are indicated by an asterisks (*). Can you program the electronic version the form to force users to fill in those fields (for example, to not allow the user to print or save the form until they complete those fields)? b) The form contains two columns of instructions at the top. In Microsoft Word, how should you create the instruction area? c) After you finish and release the SMART form, a user complains that when they enter information in the form, the entire form starts to skew (the column expands, and other columns become narrow). In fill-in only forms, why should the font for bookmarks and entry fields be different than the font used for the form s static text? 2. You have created a fill-in only form that contains a problem list bookmark. 29 SMART Forms Workbook.book Page 25 Monday, October 26, :44 AM Module: 3 Modifying SMART forms Introduction to this module Purpose This module provides an overview of how to modify your own SMART forms, and SMART forms provided by TELUS Health. Objectives Upon completion of this module, you will be able to: 1. Modify a SMART form. 2. Remove any editing restrictions on a SMART form template. 3. Combine multiple forms into one order set. 4. Use the same bookmark multiple times on a single SMART form. Modifying SMART forms You can edit any SMART form, even if it is provided by TELUS Health. Common edits include: Changing bookmarks (for example, to pull Family Practitioner details instead of Consultant details) Updating the base form (for example, a diagnostic imaging company changes their location and you want to update the company s form to have the correct contact details) When you modify a form that is originally provided by TELUS Health, you save the modified form as a "house" form. You cannot modify the centralized form on the Wolf EMR data center. Please be aware: Because modified forms become house forms, these forms cannot be maintained or updated by TELUS Health. 30 SMART Forms Workbook.book Page 26 Monday, October 26, :44 AM Modifying SMART forms If you are modifying a form provided by TELUS Health, copy the form Word template file from the Wolf Master Templates Directory and paste it to your User Templates folder. See notes on page 7. From the User Templates folder, open the form you want to modify, remove editing restrictions, and then modify any content as needed. Tips for modifying SMART forms: If you want to modify only certain check boxes, text fields, and drop-down lists so that they are pre-filled with information, create an order set. see Creating order sets on page 5. To remove editing restrictions, click the Developer tab, and then click Restrict Editing > Stop Protection. SMART forms typically have four files in the template folder. Always modify the Microsoft Word Template file. CAUTION: Do not double-click to open and modify the form. Always rightclick and then click Open. If you double-click the form to open it, any modifications you make are not saved. Practise: Modifying a SMART form 1. Open the Wolf Master Forms Directory folder. 2. Open your User Templates folder. 3. In the Wolf Master Directly folder, copy any SMART form s Microsoft Word Template file. 4. Go to your User Templates folder and paste the copied SMART form template file. 5. Rename the SMART form template file to Test modify. 6. Open the renamed SMART form (right-click > Open). 7. Remove form restrictions. 8. Modify some static text on the form (for example, the clinic/program contact details). 9. Delete and add a new bookmark in any field on the form. 10. (Optional) Open the form in a test patient s record to ensure your modified form is available and that the changes you made are present. Combining multiple forms into one order set If you often use a complete set of forms for certain types of patient visits (for example, prenatal tests), you can combine multiple forms into one order set. Order sets and other house forms are not maintained or updated by TELUS Health. To create a multi-form order set, you copy and paste the contents from multiple forms into one Microsoft Word template. High-level steps: 1. For the SMART forms you want to combine, copy the Microsoft Word Template files from the Wolf Master Templates Directory, and then paste the files into the Order sets folder in your User Templates folder. 2. Open the first form to display in the order set. Remove any editing restrictions on the template, and then save the template with a new name, using a name that describes the purpose of the order set (for example, Prenatal exam forms ). 3. Open the Word template for the next form to display in the order set. Remove any editing restrictions on the form, copy the contents of the entire form, and then, paste the form s contents in the first template. Repeat this step for all other forms to be included in the order set. Wolf EMR SMART forms - Issue
32 SMART Forms Workbook.book Page 28 Monday, October 26, :44 AM Modifying SMART forms Tips for combining multiple forms into one SMART form When you open a Word template, always right-click the file, and then click Open. Avoid double-clicking the file. In Microsoft Word, ensure you enable the Developer tab and set bookmarks to show before you make modifications to the SMART form. When you copy and paste contents from one template into another template, use the following shortcuts in Microsoft Word: <Ctrl><A>: to select a document s entire contents. <Ctrl><C>: to copy the selected content. <Ctrl><End>: to take you to the very bottom of a document. <Ctrl><V>: to paste copied content. If a form is built in a table, ensure you place your cursor below the table before you paste the contents of the next form. If there are identical bookmarks used between the combined forms (for example, patient last name [PatientLName]), only the first bookmark remains and all remaining instances disappear. Re-add the bookmarks, but with a unique number following the bookmark name (for example, PatientLName2).
