5 What Is a CMS-2744 Form? The ESRD Facility Survey forms (CMS-2744) are used to capture and report facility and patient information related to the operation of each federally approved dialysis or transplant facility. CMS-2744A is completed by dialysis units. CMS-2744B is completed by kidney transplant centers Some facilities may fill out both forms. The CMS-2744 Form summarizes the number of: Patients receiving care at the beginning of the reporting period Losses during the reporting period Patients at the end of the reporting period Patients enrolled in Medicare Hemodialysis patients dialyzing more than four times per week Vocational rehabilitation status Staffing RETURN TO INTRODUCTION Page 5

6 What Is a CMS-2744 Form? (continued) The CMS-2744 report contains patient and facility activity data between January 1 and December 31 (Survey Period). The CMS-2744 is created after the survey period ends. Patient and facility data entries used to calculate activity must be available after the survey period ends AND data is available in CROWNWeb. The deadline to have a FINALIZED 2012 CMS-2744 form in CROWNWeb is May 1, RETURN TO INTRODUCTION Page 6

7 Generate the CMS-2744 Form The following interactive tutorial will guide you through adding (generating) and saving a CMS-2744 form in CROWNWeb. RETURN TO INTRODUCTION 1. Select Form 2744 From the CROWNWeb Home Page, click Form The Form 2744 Search Results screen displays. 2. Click Add New 2744 Click Add New 2744 in the sub-menu. The Add a New Form 2744 screen displays. 3. Enter Facility Criteria In the Facility DBA Name and Survey Year fields, use the drop-down lists to select the facility and survey year, and then click Add. The Add 2744 for <facility> for Survey Year <yyyy> screen displays. NOTE: You cannot create more than one CMS-2744 form for a facility for a specific year. 4. Treatment and Staffing Section In the Treatment and Staffing section, click in each enabled field and manually enter your treatment and staffing counts. You may need to manually obtain this data, since it may not be in CROWNWeb in its entirety. 5. Remarks/Comments Section (Optional) Enter any comments you need to include in the Comments field. 6. Click Generate Click Generate to generate the counts in the form fields. The message Form 2744 GENERATED displays. 7. Click Save Click Save to save the form. The message Form 2744 SAVED displays. At this point, you can no longer modify data in the displayed fields, but you can view the counts that were generated on the screen. Page 7

8 Review a CMS-2744 through Status Change Updates The CMS-2744 Form moves through several stages as it is completed. When statuses change, facility and Network users are notified in CROWNWeb. Statuses can change from: Draft to Final Final to Accepted Final to Rejected Accepted to Finalized Finalized to Reopened The next slide reviews the descriptions for each status; or click on Bypass Review Status to navigate to the Editing Treatment and Staffing Counts section. 9 CMS-2744 Status Descriptions DRAFT Indicates that a new CMS-2744 Annual Facility Survey has been generated. This status includes any errors or warnings when generated from facility data in CROWNWeb. FINAL Indicates the facility has generated the survey and submitted the survey to the Network for review and acceptance/ rejection. REJECTED Indicates the Network has found uncorrected errors or unaddressed warnings on the survey and returned the survey back to the facility for correction. ACCEPTED Indicates the Network has reviewed the survey s data and accepted the survey in its current state. FINALIZED Indicates that the Network has accepted the submitted survey from the facility, and has clicked FINALIZED to complete the survey process. REOPENED Indicates that an error was found on the CMS-2744 form after it was FINALIZED, and that the form has been reopened via a request to the QualityNet Help Desk. The reopened form must be corrected by the facility and resubmitted to the Network for acceptance. RETURN TO INTRODUCTION Page 9

10 Review a CMS-2744 through Status Change Updates (cont.) The following interactive tutorial will help you review status change updates for a CMS-2744 in CROWNWeb. 1. Click Form 2744 From the CROWNWeb Home Page, click Form The Form 2744 Search Criteria screen displays. 2. Click Status Change Updates Click Status Change Updates from the sub-menu. The Form 2744 Status screen displays. 3. Status Categories Display The latest status of each CMS-2744 displays under a status category: Accepted Rejected Finalized Re-opened 4. Click ID Number Click on the ID number for the selected 2744 under the appropriate status section. The View 2744 screen displays the facility s 2744 form. 5. Review Counts and Comments Review the counts, then scroll down to view the status and comments in the REMARKS/COMMENTS section. You can also view any comments entered by the user who completed the action that resulted in that status change. RETURN TO INTRODUCTION Page 10

11 Edit Treatment and Staffing Counts If you find the counts in the Treatment and Staffing section are incorrect, they can be easily updated in CROWNWeb. The following interactive tutorial will help you make updates to the Treatment and Staffing section of the CMS-2744 Form in CROWNWeb. 1. Click Form 2744 From the Home page, click Form 2744 in the main menu. The Form 2744 Search Criteria screen displays. 2. Search for Survey Year In the Survey Year field, enter the correct survey year, then click Search. The Form 2744 Search Results screen displays. 3. Select Current Survey Click the ID link for the most current survey. The View 2744 screen displays. 4. Click Edit Click Edit in the gray sub-menu. The Edit 2744 screen displays. 5. Make Edits The Treatment and Staffing section fields are now enabled. Make any edits as applicable. 6. Click Generate/Save After edits have been made, scroll down the screen and click Generate; then click Save. 7. Saved Message After clicking Save, the message "Form Saved" displays. 12 Regenerate the CMS-2744 Form After making edits to patient records (see Errors) you must regenerate the CMS to update the counts: 1. From the View 2744 screen, click Edit in the gray sub-menu. RETURN TO INTRODUCTION BYPASS REGENERATE TO SUBMIT/FINALIZE Page 12

15 Submit and Finalize the CMS-2744 Form Now that all errors have been resolved, you must submit a final version of the CMS-2744 Form to your Network. Click Form 2744 From the CROWNWeb Home page, click Form The Form 2744 Search Results screen displays. 2. Search for Facility/Enter Survey Year Enter your facility CCN or NPI to search for a specific facility, or just enter the Survey Year and click Search. The View 2744 screen displays with any facility that has created a CMS-2744 for that survey year. 3. Click Submit for Acceptance Scroll to the bottom of the screen and click Submit for Acceptance. A confirmation window displays. 4. Click Yes Click Yes to confirm that you wish to submit the form. The View 2744 screen displays. 5. Confirmation Message The message "Form 2744 has been successfully submitted to your in-scope network." displays. 6. Print the Form (OPTIONAL) If your Network has requested a printed copy, scroll to the bottom of the screen and click Print. The form displays in PDF format in a new window. 7. Click Print Icon In Adobe Reader, click the print icon to send the form to your printer. Page 15

16 Finalized CMS-2744 If all counts are correct, the Network marks the status of the CMS-2744 as Finalized, and the facility can view the status via the Status Change Updates screen. NOTE: A CMS-2744 cannot be edited after the form has been finalized. RETURN TO INTRODUCTION Page 16

17 Resolve the Most Common Errors Most of the errors facilities will see after generating the CMS-2744 are related to: Duplicate patient records, and Patients that have a System Discharge instead of a Transfer. The following interactive tutorials will help you find these patients so you can quickly resolve these errors. At the end of this tutorial, you will find information on all CMS-2744 error and warning messages, including directions on how to handle them. If you are having trouble with opening any of the CMS-2744 Reports, click on Troubleshoot Opening Reports to open a PDF file with instructions; or click the NEXT arrow to continue with resolving errors. 18 Search for Duplicate Records The following interactive tutorial will help you search for duplicate records in CMS-2744 reports. 1. Launch the Patients Receiving Care at the End of the Survey report with columns ready to filter/sort. 2. Select the date row, right click and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Select the Filter/Sort button (down arrow) in Column F (Patient Name). The Filter/Sort dialog box displays. 5. Select Sort A to Z from the menu and click OK to run the filter. 6. Locate any duplicate patients. If you are a Batch Submitting Organization, contact your Corporate Help Desk to delete these duplicate patients. 7. If the Patient ID (CROWN UPI) is the same, check to see if the patient was discharged and then re-admitted. RETURN TO INTRODUCTION Page 18

19 Search for System Discharge Patients The following interactive tutorial will help you search for patients with a System Discharge included in CMS-2744 reports. 1. Launch the Additions During Survey Period report with columns ready to filter/sort. 2. Select the date row, right-click and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Select the Filter/Sort button (down arrow) in Column N (Discharge Reason). The Filter/Sort dialog box displays. 5. Click the Select All box to deselect all items, then select the System Discharge box. 6. Click the OK button to run the filter. 7. When the filter is run, only patients with a Discharge Reason of System Discharge display. 8. Note the Patient ID (CROWN UPI) for these patients, and then review the patient's Admit/Discharge Summary record. 9. System Discharges occur when a patient is transferred in to a new facility without being discharged from the Home facility. Correct the Discharge Reason to Transfer. 10. For non-batch facilities, if a transient patient was not admitted as a Transient, the Admit/Discharge Summary displays a System Discharge. RETURN TO INTRODUCTION Page 19

20 Resolve Errors on the CMS-2744 Form This section outlines the cause and resolution to all errors users may encounter when generating a CMS-2744 Form. Users will also learn to use the sort/filter feature in the Microsoft Excel reports that is needed to research and resolve errors on the CMS-2744 Form. 21 Error Messages Click on an error message below to find specific information on the error. RETURN TO INTRODUCTION BYPASS ERRORS TO WARNINGS The number of frequent hemodialysis patients cannot be greater than the total number of hemodialysis patients (the sum of fields 30A, 31A, 30B, 31B cannot be greater than the sum of fields 14, 16, and 21). The number of patients receiving VR services (field 33) cannot be greater than the total number of patients (field 26). The number of VR employed patients (field 34) cannot be greater than the total number of patients (field 26). The number of VR employed patients (field 34) cannot be greater than the number of patients aged (field 32). Total Patients (field 26) cannot equal 0 if the facility has not closed. The Total number of patients receiving care at the beginning of the survey period (field 03) cannot equal 0 if the facility has not closed. The number of Total Patients (26) must equal the sum of the total number of incenter dialysis patients (field 20) and the total number of home dialysis patients (field 25). The number of total incenter dialysis patients (field 20) must equal the sum of fields 14 through 19. The Total number of patients (field 26) must equal the sum of the patients receiving care at the beginning of the survey period (field 03) plus the additions during the survey period (fields 04A through 07B) minus the losses during the survey period (fields 08A through 13B). 23 Error Message This error occurs when the total number of HD patients dialyzing more than four times a week is greater than the total number of in-center and home HD and self HD-trained patients. After all errors are resolved, regenerate the CMS-2744 Form. 1. Launch the Hemodialysis Patients Dialyzing More Than 4 Times Per Week report with columns ready to filter/sort. 2. Click on the date row, right-click and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Click the Filter/Sort button (down arrow) in Column K. 5. Click the Select All box to deselect all items, then check the box for Dialysis Facility/Center. Click the OK button to run the filter. 6. Click the Filter/Sort button (down arrow) in Column L. 7. Click the Select All box to deselect all items, then click the Daytime and Nocturnal boxes. Click the OK button to run the filter. 8. When the filters are applied, only patients with a Dialysis Setting of Dialysis Facility/Center and Dialysis Time Period of Daytime and Nocturnal will display. 9. Locate any possible duplicate patients and note their Patient ID (CROWN UPI). Batch facilities should contact their Corporate Help Desk to delete any duplicate patients. 10. For non-batch facilities, if the Patient ID (CROWN UPI) is the same for duplicate patients, go into CROWNWeb and edit the Admit/Discharge Summary to delete the duplicate admit record. 24 Error Message This error occurs when the number of Vocational Rehabilitation employed patients is greater than the total number of patients. Click View Slideshow to learn how to resolve this error. Click Online Help to view instructions for finding duplicate treatment records or System Discharge patients. After all errors are resolved, re-generate the CMS-2744 Form. 1. Launch the Vocational Rehabilitation report with columns ready to filter/sort. 2. Select the date row (bottom row of the spreadsheet), right-click to bring up the menu, and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Click the Filter/Sort button (down arrow) in Column L (Vocational Rehabilitation Status). The Filter/Sort dialog box displays. 5. Click the Select All box to de-select all items, then select the Completed VR and Currently in VR boxes. Click OK to run the filter. 6. When the filter is applied, only patients with Completed VR or Currently in VR will display. 7. Note the number of patient records in the lower-left corner receiving Vocational Rehabilitation Services. 8. If this number is greater than the Total Patients (Field 26), review the Excel spreadsheet to locate duplicate patients. 9. Make note of the duplicate patient's Patient ID (CROWN UPI). 10. Go back into CROWNWeb, review the patient's Admit/Discharge Summary record, and delete any duplicate admits. Then, re-generate the Return to Error Messages Page 24

25 Error Message This error occurs when the number of patients receiving Vocational Rehabilitation Services is greater than the total number of patients. Click View Slideshow to learn how to find this error using the MS Excel Reports. After all errors are resolved, regenerate the CMS-2744 form. 1. Launch the Vocational Rehabilitation report with columns ready to filter/sort. 2. Select the date row (bottom row of the spreadsheet), right-click to bring up the menu, and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Click the Filter/Sort button (down arrow) in Column P (Employment Status). The Filter/Sort dialog box displays. 5. Click the (Select All) box to de-select all items, then select the Employed Full Time and Employed Part Time boxes. Click OK to run the filter. 6. When the filter is applied, only patients with Employed Part Time or Employed Full Time will display. 7. Note the number of patient records in the lower-left corner that are Employed Full Time and Part Time. 8. If this number is greater than the Total Patients (Field 26), review the Excel spreadsheet for duplicate patients. 9. If duplicate patients are found, note the Patient ID (CROWN UPI) for the duplicate patient(s). 10. Go back into CROWNWeb, review the patient's Admit/Discharge Summary record, delete any duplicate admit records. Return to Error Messages Page 25

Launch the Vocational Rehabilitation report with columns ready to filter/sort. 2. Select the date row (bottom row of the spreadsheet), right-click to bring up the menu, and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Click the Filter/Sort button (down arrow) in Column P (Employment Status). The Filter/Sort dialog box displays. 5. Click the (Select All) box to de-select all items, then select the Employed Full Time and Employed Part Time boxes. Click OK to run the filter. 6. When the filter is applied, only patients with Employed Part Time or Employed Full Time will display. 7. Locate patients that are under age 18 or over age 54, and note their Patient ID (CROWN UPI). 8. Also locate any duplicate patients and note their Patient ID (CROWN UPI). 9. Go back into CROWNWeb, and edit the patient's Attribute History in the Employment Status History section by selecting the correct status; -OR- 10. If you find there is a possible duplicate patient, then delete the duplicate with the assistance of the Help Desk. Return to Error Messages Page 26

27 Error Message This error occurs when the number of total patients (field 26) is zero (0) if the facility is a dialysis facility and is open at the end of the survey period. This error could occur if the facility is a seasonal facility or has been disasterstricken. For facilities that appropriately have a zero (0) in field 26, the CMS-2744 Form must be approved by CMS. You can submit the CMS-2744 form with this error, but provide a comment to explain this situation. Return to Error Messages Page 27

28 Error Message This error occurs when the total number of patients receiving care at the beginning of the survey period equals zero (0) and the facility is open during the survey period. NOTE: This error will only display for facilities that open during the survey year. No correction is required. Return to Error Messages Page 28

29 Error Message This error occurs when the total number of patients is greater than the sum of the total number of in-center patients (field 20) and the total number of home patients (field 25). Click View Slideshow to learn how to find this error using the MS Excel Reports. Click Online Help to view instructions for finding System Discharge patients. After all errors are resolved, regenerate the CMS-2744 Form. 1. Launch the Patients Receiving Care at the End of the Survey report with columns ready to filter/sort. 2. Select the date row (bottom row of the spreadsheet), right-click to bring up the menu, and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Select the Filter/Sort button (down arrow) in Column O (Primary Dialysis Setting). The Filter/Sort dialog box displays. 5. Click the Select All box to deselect all items, then select the (Blanks) box to find patients with no assigned Primary Dialysis Setting. 6. Click the OK button to run the filter. 7. When the filter is applied, only patients with no Primary Dialysis Setting will display. 8. Note each patient's Patient ID (CROWN UPI) number. 9. Go back into CROWNWeb, open the patient's Edit Treatment Information screen, and update the patient's Primary Type of Treatment field. 10. You can also search for duplicate patients by selecting the Filter/Sort button (down arrow) in Column F (Patient Name). The Filter/Sort dialog box displays. 11. Sort the records from A to Z and click OK. 12. Locate any duplicate patients. If you identify any duplicate patients, contact the Help Desk to have these patients deleted. 30 Error Message This error occurs when the total number of in-center patients does not equal the sum of fields 14 through 19. Click Online Help to view instructions for editing patient treatment information or creating a new treatment record for any missing patients. After all errors are resolved, regenerate the CMS-2744 Form. 1. Launch the Patients Receiving Care at the End of the Survey report with columns ready to filter/sort. 2. Select the date row (bottom row of the spreadsheet), right-click to bring up the menu, and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Select the Filter/Sort button (down arrow) in Column P (Primary Type of Treatment). The Filter/Sort dialog box displays. 5. Click the Select All box to deselect all items, then select the (Blanks) box. 6. Click the OK button to run the filter. 7. When the filter is run, only patients with no assigned Primary Type of Treatment display. 8. Note the Patient ID (CROWN UPI) for these patients. 9. Go back into CROWNWeb, open the patient's Edit Treatment Information screen, and correct the Primary Type of Treatment field. Return to Error Messages Page 30

31 Error Message This error occurs when the total number of patients receiving care at the beginning of the survey period plus the additions during the survey period minus the losses during the survey period does not equal the total number of patients. Click View Slideshow learn how to find this error using the MS Excel Reports. Click Online Help to view instructions for editing patient records that have a System Discharge. 32 Error Message This error occurs when the number of recorded Medicare Enrollment Statuses does not match the total number of patients (Field 26). Click View Slideshow learn how to find this error using the MS Excel Reports. Click Online Help to view instructions for editing patient attributes and assigning a Medicare Enrollment Status. After all errors are resolved, regenerate the CMS-2744 Form. 1. Launch the Patient Eligibility Status Report with columns ready to filter/sort. 2. Select the date row (bottom row of the spreadsheet), right-click to bring up the menu, and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Select the Filter/Sort button (down arrow) in Column K (Medicare Enrollment Status). The Filter/Sort dialog box displays. 5. Click the Select All box to deselect all items, then select the (Blanks) box. 6. Click the OK button to run the filter. 7. When the filter is applied, only patients without an assigned Medicare Enrollment Status will display. 8. Note each patient's Patient ID (CROWN UPI) number. 9. Go back into CROWNWeb, open the patient's Edit Patient Attributes History screen, and add the patient's Medicare Enrollment Status. Return to Error Messages Page 32

33 Error Message This error occurs when the total number of patients receiving in-center other dialysis is greater than the total number of in-center other dialysis treatments including training. Field 15 is incorrect. Click View Slideshow learn how to find this error using the MS Excel Reports. Update the Staffing and Treatment section / Other field to the correct count (see Make Edits). After all errors are resolved, regenerate the CMS-2744 Form. 1. Launch the Additions During Survey Period report with columns ready to filter/sort. 2. Select the date row (bottom of the spreadsheet), right-click to bring up the menu, and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Select the Filter/Sort button (down arrow) in Column P (Primary Type of Treatment). The Filter/Sort dialog box displays. 5. Click the Select All box to deselect all items, then select the Other box. This will isolate patients with an Other treatment type. 6. Click the OK button to run the filter. 7. Only patients with an Other Primary Type of Treatment display. 8. Select the Filter/Sort button (down arrow) in Column S (Dialysis Type of Training). The Filter/Sort dialog box displays. 9. Select the Filter/Sort button (down arrow) in Column S (Dialysis Type of Training). The Filter/Sort dialog box displays. 10. Now, only patients with an Other Primary Type of Treatment and no Dialysis Type of Training display. 11. Note the Patient ID (CROWN UPI) for these patients. 12. Go back into CROWNWeb, open patient's Edit Treatment screen, and update the treatment as necessary. Once all errors are resolved, re-generate the CMS-2744 Form. Back to Error Messages Page 33

If the facility certification date is in the current survey period, then fields 01 and 02 must equal 0. Please add a comment. The total number of patients receiving care at the beginning of this survey period (field 03) must equal the total number of patients receiving care at the end of the last survey period for this facility (field 26). Please enter a comment. The total number of incenter patients receiving Other dialysis treatments (field 37) may be too high. Please verify this total. 39 Warning Message This warning occurs when the total number of in-center patients receiving other dialysis treatments is greater than 100. You must verify this total prior to submitting the CMS-2744 Form, since field 37 should not be greater than 100. RETURN TO WARNING MESSAGES Page 39

40 Warning Message This warning occurs when patients with a discharge reason of System Discharge that should be included in the beginning counts but are not counted as a loss or are in the end of survey period counts. Finding and fixing System Discharges was covered earlier in this tutorial, in the Errors section. To view the System Discharge tutorial again, review the instructions below: 1. Launch the Additions During Survey Period report with columns ready to filter/sort. 2. Select the date row, right-click and select Delete to delete the row. 3. Select Row 3, click on Sort & Filter in the main menu, and select Filter. 4. Select the Filter/Sort button (down arrow) in Column N (Discharge Reason). The Filter/Sort dialog box displays. 5. Click the Select All box to deselect all items, then select the System Discharge box. 6. Click the OK button to run the filter. 7. When the filter is run, only patients with a Discharge Reason of System Discharge display. 8. Note the Patient ID (CROWN UPI) for these patients, and then review the patient's Admit/Discharge Summary record. 9. System Discharges occur when a patient is transferred in to a new facility without being discharged from the Home facility. Correct the Discharge Reason to Transfer. 10. For non-batch facilities, if a transient patient was not admitted as a Transient, the Admit/Discharge Summary displays a System Discharge. RETURN TO WARNING MESSAGES Page 40
