7 Addiction Severity Index Multimedia Version Connect ASI-MV Connect Overview ASI-MV Connect is an interactive program that allows for client selfadministration of the widely used ASI. Clients, regardless of education level, reading ability and prior computer experience, can easily use this program and, because it requires no staff or training time to administer, ASI-MV Connect can be run for a fraction of the cost of the interviewer version. ASI-MV Connect is an engaging audio and video program that guides clients through a series of on-screen offices in which they meet with virtual interviewers. Questions and answer options are presented verbally as well as written on the screen, so that clients need not be literate to self-administer the ASI. Upon completion, reports are immediately available for staff members. In clinical studies, the ASI-MV Connect has proven to have excellent reliability and validity (Butler et al, 2001). It can save treatment centers money, ensure standardization, and become an indispensable part of substance abuse treatment and outcome planning. Research findings appear in: Butler, S. F., et al. 10 Installation Some ASI-MV Connect customers will be installing over previous versions of the program. Do not uninstall earlier versions. When ASI-MV Connect is run for the first time, the program will search your computer for previous ASI-MV data, and will migrate it to the new version. (See step 3 for more information on data migration.) IMPORTANT NOTE: ASI-MV Connect requires several software components that may not be installed on your computer. If your computer does not have these components installed, the ASI-MV Connect installer will prompt you to install them. After the additional components have been installed, you will need to restart your ASI-MV Connect installation. In order to install ASI-MV Connect properly, you must be a Windows Computer Administrator. Please ask your Information Technology administrator if you are not sure of your user type or privileges. It is also suggested that if you are running any other programs at this time to close them. Follow the steps below to install ASI-MV Connect. Step Action Associated Screen or Result Log in to the computer as a Computer Administrator. If you do not have Microsoft Word installed, it should be installed now before installing ASI-MV Connect. If you do not have an active connection to the internet, please connect now. You are logged in with Administrator privileges. Microsoft Word is installed and available for use. 11 Step Action Associated Screen or Result Log in to the ASI-MV Connect Data Center using the login and password supplied to you by Inflexxion, Inc. when you purchased the product. If you do not know your login information, please contact Inflexxion for assistance. On the left side of the screen, click on the link labeled ASI-MV under the heading Product Downloads. Click on the system requirements link and make sure that your operating system meets the requirements listed. You can close the pop-up box by clicking on the x in the top right hand corner. Before you install the ASI-MV Connect software you need to download the Business Associate Agreement. This PDF should be signed and faxed back to Inflexxion. 12 Step Action Associated Screen or Result Click on the button labeled DOWNLOAD KEY. This will download your facility s key, which activates the ASI-MV Connect software. Save the key to your desktop; do not open it. After the key has been downloaded, click on the button labeled DOWNLOAD INSTALLER. Please download this file to your desktop by selecting Save As if prompted. This download process could take 35 minutes or more. After the Installer has finished downloading, close your browser and locate the ASI-MV Connect Setup program that you downloaded. It will be called SmartClientSetup.exe Double-click the ASI-MV Connect Setup program (SmartClientSetup.exe). Click Next to continue. Inflexxion, Inc. 3/6/2008 6

13 Step Action Associated Screen or Result You may be prompted at this point to install Microsoft.Net 1.1 and/or 2.0 Frameworks to your system before continuing with the installation of ASI-MV Connect. In this case, after installing one or both.net package(s) from the Internet, please re-run the Smart Client Setup (SmartClientSetup.exe) that you previously downloaded to your desktop. Click on I Agree to accept the License Agreement. Choose Install Location: Accept the default location (unless you have a specific reason for placing the ASI-MV Connect Software in another location) Click the Install button to start the installation. 14 Step Action Associated Screen or Result Some users running Windows 2000 will be prompted to install MDAC at this point. If you are prompted to install MDAC, please turn to page 11 and follow the instructions labeled: Installing Microsoft Data Access Components (MDAC) v.2.8 (Windows 2000 users only). If you are not prompted to install MDAC, please continue. The progress screen shown at right will then be displayed. After installation has progressed, Microsoft SQL Server will be installed. This progress window will look a bit different. (see right) After SQL has been installed, and other components have been set up, if you do not have Adobe Acrobat Reader version 6 (or higher) installed on your computer, you will see a message to this effect. Click OK to dismiss this prompt. The Acrobat Download page will be loaded in your web browser. Please finish ASI-MV Connect installation Inflexxion, Inc. 3/6/2008 8

After installation has completed, you should return to the browser, and download and install Acrobat Reader by following the instructions on the web page. You may be prompted to re-start your computer after Adobe Acrobat has been installed. Do not re-start your computer until after the ASI-MV Connect installation has completed. ASI-MV Connect will then display the informational message shown at right, and then launch Windows Media Player to ensure that the multimedia content runs correctly. You should see a brief clip of narrators George and Angela within Windows Media Player. After the clip has finished, re-run it once to ensure good playback. Then close Windows Media Player to finish ASI-MV Connect installation. Click Next, and then Finish to end installation. The ASI-MV Connect program is now loaded on your computer. Some systems may require you to restart for new settings to take effect. If you are prompted to restart, please do so before continuing. Inflexxion, Inc. 3/6/2008 9

17 Installing Microsoft Data Access Components (MDAC) v.2.8 (Windows 2000 users only) This procedure should only be followed if you are prompted by the ASI-MV Connect installation program to install MDAC. This would have happened at Step 0 in the Installation instructions above. Step Action Associated Screen or Result 1 Click OK on the prompt to install MDAC. 2 Your default browser will be displayed with the Microsoft Data Access Components (MDAC) 2.8 download page loaded. Click the Continue button. 3 Wait a short while until you see the Security Warning pop-up window. Click Yes to install and run Windows Genuine Advantage. Inflexxion, Inc. 3/6/

This will take a short while to load. 6 Click on the checkbox to accept the license agreement, then click Next to begin installation of the MDAC software. Inflexxion, Inc. 3/6/

19 Step Action Associated Screen or Result 7 Click Finish to begin installation. 8 Make sure the first item Let setup restart the system now is selected, then click Finish to restart your system. 9 When the system restarts, log in to the computer as a Computer Administrator. You are logged in with Administrator privileges. 10 Look for the Setup ASI-MV Connect on your desktop. Double-click this icon to re-start your ASI-MV Connect installation. 11 Return to Step 0 in the above procedure table (Installation) and continue your installation. Inflexxion, Inc. 3/6/

20 Running ASI-MV Connect for the First Time Step Action Associated Screen or Result 1 Locate the ASI-MV Connect icon on your computer desktop. Double-click this icon to open the program 2 After launching the program, you will see the screen at right, informing you that you will need to enter the product key. ASI-MV Connect Click on the NEXT button. The pop-up window (shown below right) will then be displayed. From here you can browse to locate your product key. It should be in the same location as the downloaded software. The name of the software key file is: ASIMVConnect.key The default location for the product key is: C:\Program Files\Inflexxion\SmartClient Inflexxion, Inc. 3/6/

21 Step Action Associated Screen or Result 3 After ASI-MV Connect processes your product key, the User Name and Password Creation screen to your right will be displayed. Type in your desired user name and password, then re-type the password in the Confirm Password field. When you have finished, click NEXT. 4 You will then see a screen offering the option of migrating data from previous English versions of ASI-MV. If you had a previous English version of ASI-MV, click NEXT. The pop-up shown below right will be displayed as the system searches for previous English ASI-MV data. If you did not have a previous English version of ASI-MV, click EXIT. 5 Once migration has completed, you will see a screen indicating that English migration finished successfully! Click the EXIT button to scan for previous versions of Spanish data. If you do not have data, you can click EXIT on the next screen to log into the ASI- MV Connect. Inflexxion, Inc. 3/6/

22 Step Action Associated Screen or Result 6 You will then see a screen offering the option of migrating data from previous Spanish versions of ASI-MV. If you had a previous Spanish version of ASI-MV, click NEXT. The pop-up shown below right will be displayed as the system searches for previous Spanish ASI-MV data. If you did not have a previous Spanish version of ASI-MV, click EXIT. 7 Once migration has completed, you will see a screen indicating that Spanish migration finished successfully! Click the EXIT button to log into the ASI-MV Connect. 8 Log in with the user name and password that you established in Step 3. Inflexxion, Inc. 3/6/

23 Step Action Associated Screen or Result 9 The ASI-MV Connect Main Menu will then be displayed Upgrading from Trial Version to Registered Version of ASI-MV Connect If you have upgraded from the Trial version to the Registered version of ASI-MV Connect, and you wish to activate your registered version of the product, follow the steps below. 24 Step Action Associated Screen 3 Log in to the ASI-MV Connect Data Center using the login and password supplied to you by Inflexxion, Inc. when you purchased the product. 4 On the left side of the screen, click on the link labeled ASI-MV under the heading Product Downloads. 5 Click on the button labeled DOWNLOAD KEY. This will download your facility s key, which activates the ASI-MV Connect software. Make a note of the location into which the key was downloaded. 6 Locate the ASI-MV Connect icon on your computer desktop. 25 Step Action Associated Screen 7 Log in with your user name and password. 8 The ASI-MV Connect Main Menu will then be displayed. From here, click on the button labeled: Administration. 9 From the Administration Menu, click on Product Activation Key Inflexxion, Inc. 3/6/

26 Step Action Associated Screen 10 Click on NEXT to browse for your new product key file. 11 Navigate to the location of the key, and click on the key to select it. Then click on Open 12 A message will be displayed indicating that your product key has been activated. 13 Click OK to continue. Your new key has been loaded onto this computer. Purchasing / Transferring Uses Immediately after installing the program for the first time, you will need to purchase or transfer a number of uses for ASI-MV Connect. Each use corresponds to a completed interview for your facility. Inflexxion, Inc. 3/6/

27 Follow the steps below to transfer uses to your newly installed computer: Step Action Associated Screen 1 From the ASI-MV Connect Main Menu, click on the button labeled: Administration. 2 You should now see the screen shown at right. Click on Manage Uses. 3 The screen shown at right will then be displayed. Choose to either Transfer uses from your Facility Bank, or Purchase additional uses for your facility. 4 If you select Purchase Uses, a toll-free number will be displayed, at which you can call Inflexxion to purchase uses for your facility. 28 Step Action Associated Screen 5 If you select Transfer Uses, you will see a screen displayed (at right) that prompts you to select the number of local computer uses you would like to transfer to this particular computer. Use the up and down arrows to select the appropriate number of uses and then click on Confirm. 6 You will be asked to confirm the transfer of uses from your facility to your local computer. Click Yes to confirm. The appropriate number of uses will then be transferred and available for use on your local computer. Please see the Computer Settings section on the following pages for important details regarding the program before administering to clients. Inflexxion, Inc. 3/6/

29 Computer Settings Screen Appearance The computer s display settings must be set to 1024x768 or higher in order to view the ASI-MV Connect screens properly. Check the current settings on your computer in the control panel and adjust if necessary. Changing the Display Settings: 1 Click on Start in the lower left toolbar. 2 Scroll up and select Settings and then select Control Panel (or Settings and then Control Panel on Windows 2000). 3 Double-click on Display (or Personalization and then Display for Vista editions). 4 Click on the Settings tab. 5 Move the shuttle under Screen Resolution to the left or right depending on current setting until it is set at 1024x768 or higher. 6 To save settings (or to simply close window if settings are already correct) click OK and close all open windows by clicking on the X in the upper right corner of window. 7 You may be prompted to restart the computer for new settings to take effect. If prompted to do so, restart your computer now. Font Size Should the display settings appear incorrect when running the program (for example, pictures and/or buttons are not aligned) please check your display settings to ensure your computer is displaying at 96 dpi. 30 2 Scroll up and select Control Panel (or Settings and then Control Panel on Windows 2000). 3 Double-click on Display. 4 Click on the Settings tab. 5 Click on the Advanced button 6 Set Display/Font Size to Normal size - 96 dpi. 7 To save settings or to simply close window if settings are already correct click OK and close all open windows by clicking on the X in the upper right corner of window. 8 You may be prompted to restart the computer for new settings to take effect. If prompted to do so, restart your computer now. Screen Savers & Scheduled Programs Some ASI-MV Connect users have experienced problems with audio and/or video playback on computers running screen savers, anti-virus software, and/or scheduled programs (for example, system scans). If you encounter similar problems please try temporarily disabling these programs while administering the ASI-MV Connect interview. Remember to enable them when the interview is complete. For further clarification, or to report problems, contact Customer Support toll-free at Audio & Video As specified in System Requirements (page 3), a sound card is required for the ASI-MV Connect program. Without one, the audio and video files will not play and the program will display an error message. If you are experiencing any problems with the audio or video and are not sure if you have a sound card, please consult the documentation that came with your computer. If you are having problems with audio or video and are sure that it is not sound card-related, check your computer connections to ascertain that: 1. The external speakers are both plugged into the system as well as to electrical outlets; Inflexxion, Inc. 3/6/

31 2. The headphones if applicable are plugged in correctly; 3. The volume is turned up. If you cannot resolve the sound problem or are having any problems with video playback, please contact Customer Support for assistance, toll-free at Inflexxion, Inc. 3/6/

32 General Description of ASI-MV Connect The ASI-MV Connect program is a self-administered, multimedia interview that consists of seven sections or domains: 1 Medical 2 Education & Employment 3 Alcohol 1 4 Drug 5 Legal 6 Family & Social 7 Psychological The program s video narrators, George and Angela, introduce the program as well as each domain and reappear throughout to provide positive feedback to the client. Each domain has its own video interviewer, appropriate for the section (i.e., doctor for Medical; lawyer for Legal, etc.), who introduces the topic and provides voiceovers for the questions within. NOTE: Clients may only change answers within the domain/section that they are currently working. It is not possible to go back to another domain/section to change answers. Questions and answers are read aloud within the ASI-MV Connect interview so that a client need not be literate to use the program. Answer options also blink or flash when read so that it is clear which option is being read. Repeat and Back buttons on each screen allow a client to repeat the question and answers as well as change answers when necessary on previous screens. While previous experience with a computer is not necessary for a client to successfully use the program, manipulating the mouse may be difficult at first for some users. It is suggested that a staff person sit with the client through at least the General section to show him or her how to point and click when necessary. Depending on a client s reading ability as well as previous computer experience with a mouse, as noted above completion of the ASI- MV Connect interview should take approximately minutes. 34 Introducing the ASI-MV Connect Interview to Clients Asking clients to use computers to enter information about themselves may be difficult for some counselors and staff. Some worry that it might be too impersonal, or not confidential enough. There are also concerns that clients may not know how to use a computer mouse, or may be afraid of failure. In clinical trials with clients representing a broad range of population demographics, Inflexxion found that: Client acceptance of ASI-MV and the computer was very high; Clients found the program engaging and easy to use; Clinicians were able to spend more face-to-face counseling time with clients and less time data gathering. As with any assessment, there will always be some clients who will not or cannot take the ASI-MV Connect interview. However, the following tips may be helpful when introducing the client to ASI-MV Connect: Explain that all clients are asked to take this questionnaire. Assure them that information gathered is confidential, as is the rest of the information in their file. Tell them that ASI-MV Connect is an interview they complete on a computer that will talk them through each section. It will present them with videos of people who will guide them through the program and that any time they need a question repeated, they can click the speaker icon button. Ask if they would like some help in learning how to use the mouse. Explain that they will be presented with mostly multiple-choice questions and will be asked to select the best answer. After answering each question, they must click the NEXT button at the bottom of each screen. Explain that the interview asks questions in different areas: medical; legal, employment; family/social; alcohol/drugs; and psychological. Inflexxion, Inc. 3/6/

35 Emphasize that this assessment will provide an overall view of problem areas as these may relate to their use/abuse of alcohol/drugs. Tell them that for each of the areas, they will be asked how serious they feel the problems are and whether they want help with those problems. Tell them that some of the questions may not apply, or may sound unusual, but instruct clients to "please bear with the program and answer as best you can." You may find it useful to tell clients that some people find answering questions on the computer is more comfortable than with a counselor. Emphasize that they should make a real effort to answer all questions. Questions that are skipped may make it difficult for the assessment to provide an accurate picture of the problems with which they are struggling. Skipping questions may also result in some domain severity ratings and composite scores not generating. If there is no Severity Rating or Composite Score generated for a Domain, the clinician can go over the questions in that Domain with the client in a face-to-face meeting. Inflexxion, Inc. 3/6/

36 Administering an ASI-MV Connect Interview The following table outlines instructions for administering an ASI- MV Connect interview with a client. Inflexxion suggests that a staff person sit with the client through the first section, the demographic section, to ensure that the client is comfortable with the mouse and understands how to answer questions and advance to the following questions/sections. Step Action Associated Screen 1 Double-click the ASI-MV Connect icon on the Desktop to start the program. 2 Enter your User Name and Password in the corresponding fields, then click NEXT.. ASI-MV Connect 3 The Main Menu shown at right will be displayed. Select Run Interview at the Main Menu. Inflexxion, Inc. 3/6/

37 Step Action Associated Screen 4 The Start a New Interview screen shown at right will be displayed. Select the appropriate Version of ASI-MV Connect (English or Spanish). 5 The Registration screen shown at right will be displayed. Enter client s information on the Registration Form screen. If the client does not have, or does not want to use his or her Social Security number, please enter Click NEXT to begin the interview. 6 A Welcome screen (shown right) is then displayed, and George and Angela will present an introduction. Click NEXT to continue. NOTE: At this point, you should step away, and the client should complete the interview on his/her own. Inflexxion, Inc. 3/6/

38 Step Action Associated Screen 7 When the client has completed the six ASI- MV Connect sections, he or she will be presented with the option of printing a personalized Certificate of Completion NOTE: There is only one opportunity to print this certificate for each client and Inflexxion suggests doing so. Facilities using the ASI-MV Connect program have found that overwhelmingly, clients enjoy receiving the certificate. If you are unable to print the certificate, a blank copy is included in Appendix A of this Program Guide for you to copy and distribute as needed. The ASI-MV Connect interview is now complete. Staff may now open ASI-MV Connect to generate and view reports for this client (see page 37 for more information). Discontinuing an Interview If a client needs to stop the program during an interview, staff may shut it down by pressing these keys simultaneously: ALT and CTRL keys on the left side of the keyboard SHIFT and F10 keys on the right side of the keyboard Click OK when you see the Escape sequence activated message. As discussed above, when the program is restarted, the client s name will appear in the list of incomplete records. Returning to Complete an Unfinished Interview NOTE: Because of time sensitive 30-day questions, any incomplete client records will be deactivated automatically from the program after 7 days. Because the ASI-MV Connect uses a database to store program data, client data is saved throughout the administration of the interview automatically. In the event that the program closes either because of a system failure or by a client s own desire to stop ASI-MV Connect interview data are saved. The client may continue where he or she left off when the program is started again within seven days. If an interview is left in an incomplete state, the following actions are available for this interview: Inflexxion, Inc. 3/6/

39 1 Resume an interview for the selected client 2 Start a new interview 3 Deactivate the selected client 4 Return to Main Menu Follow the steps below to resume an interview for a selected client. Step Action Associated Screen 1 From the Main Menu, select the Run Interview button. This will open a screen that lists the clients that have not completed their interviews (if any). 2 To continue an interview, Select the client from the list of incomplete interviews. Then click on Resume Interview. A confirmation screen showing the ASI-MV Connect video narrators, George and Angela will be displayed and the user will be presented with the last screen viewed and user can choose to start at this screen or restart from the beginning. 40 Deactivating a Client Follow the steps below to deactivate a selected client. Step Action Associated Screen 1 From the Main Menu, select the Run Interview button. This will open a screen that lists the clients that have not completed their interviews. (Shown at right.) 2 To deactivate a client, select the client from the list of incomplete interviews. Then click on Deactivate Client. A confirmation message will be displayed. Click Yes to deactivate the client The client will be deactivated within the program. Certificate of Completion At the conclusion of the ASI-MV Connect interview, clients are presented with option of printing a personalized Certificate of Completion. This certificate announces that they have successfully completed the ASI-MV. Inflexxion has learned that the certificate is very important to most clients, and some clinicians have said that their clients feel a sense of accomplishment after completing the assessment. Type a new Client ID into the Client ID field, and then click NEXT. The ID number is now changed and will be reflected in all outputs generated from this point forward. Password Protection The ASI-MV Connect software requires a username and password to access all features of the ASI-MV Connect program, including running an interview, viewing client information, generating reports and accessing reports that have previously been generated. Once an interview has been completed and uploaded to the Data Center, a new Client ID will be assigned for use within the Data Center. Report Descriptions The following paragraphs describe the different reports, what they contain, and the use of the reports. Narrative Report The Narrative Report inserts the client s responses within a narrative that describes the answers to the questions within the various Domains. The Narrative Report contains the following information: Severity & Composite Score tables Domain summaries Detailed drug and alcohol use history Psychiatric risk table Summary of the most severe domains Score Discrepancy Table DSM IV Diagnosis Template (for clinician completion, if desired) Consistency of Response Summary NOTE: Once a report has been edited, you should open it from the location in which you saved it. Using the Report function to create a Narrative Report will generate a new, non-edited copy of a client s report. Narrative Report: Consistency of Response Summary The ASI-MV Connect software has a built-in consistency check of 27 pairs of questions, which, if not answered in the same way, suggest a client may misunderstand or be deceiving the program. If a client has inconsistent responses, this is indicated at the end of the narrative report and should be addressed during counselor/clinician follow-up. Inflexxion, Inc. 3/6/

49 Narrative Report: Psychiatric Risk Questions In addition to your face-to-face assessment of clients mental status, ASI-MV Connect provides you with valuable information about their self-reported risk factors. It is recommended at a minimum that clinicians review the Psychiatric Risk Questions immediately after an ASI-MV Connect interview is completed and take appropriate action, if indicated. Psychiatric Risk Questions Listed below, are eleven questions of the Psychiatric Status section along with Mr. Client s answers to them. This gives the clinician important information, which may require immediate face-to-face intervention or followup if answered YES. In the past 30 days, have you had times when you heard voices or saw things that other people couldn t see and hear? Other times in your life? In the past 30 days, have you had times when you had trouble controlling your temper or violent behavior? Other times in your life? In the past 30 days, have you seriously considered a plan for killing yourself? Other times in your life? In the past 30 days, have you attempted suicide or tried to kill yourself? Other times in your life? Have you had trouble thinking clearly today? Have you had trouble concentrating today? Have you been hearing voices today? No No No No No No No No Not at all Not at all Not at all Editing Narrative Reports Clinician Impressions/Information (e.g. more specifics on current alcohol/drug use amounts and patterns, problems with past sobriety attempts, problems with past treatment episodes and attitude about wanting help at this time.) Once the Narrative Report is generated in Microsoft Word on your computer screen, you can edit it, add data and include your clinical impressions. In fact, at the end of each Domain Section, there is a heading labeled Clinician Impressions/Information with suggestions about topics you might want to cover in this area. See the example at left (from the Alcohol and Drug domain). Remember to click on the Save icons in Microsoft Word or Edit/Save on the toolbar to save any changes that you make to the document after it generates (e.g., clinical comments). 50 Treatment Plan Tool The Treatment Plan Tool summarizes the client s self-reported data from ASI-MV Connect across all domains, and arranges it in a useful manner for assistance with treatment planning including Problem Lists and the following Key Clusters: Client s perception of problems Client s motivation to get help for problems Possible psychiatric risks Potential strengths / supports Recovery environment assets & liabilities The summary also includes an Outcome Comparison Chart, which the clinician can use to track progress, a blank Treatment Plan Form or template, and a sheet listing the Client Handouts and Worksheets. Client Placement Worksheet (ASAM Dimensions) The Client Placement Worksheet contains the six dimensions of the ASAM Patient Placement Criteria. It is populated with data from ASI- MV Connect within each dimension and the clinician is prompted to add further information and rate the Risk of each dimension. This tool is intended to assist with effective client placement. Brief Screening Report The Brief Screening Report is a single page summary of key elements of a client s ASI-MV Connect interview responses, including answers that may indicate potential psychiatric risk. It can be used to assist staff in deciding whether or not to refer for a full evaluation. This report is created using Microsoft Word and is automatically saved by client name and date and time during generation. This report is also saved in the default location noted in the Narrative Report description, but can be saved in another location and/or moved to another computer. Domestic Violence & Sexual Assault Screening Summary The Domestic Violence & Sexual Assault Screening Summary is a single page summary of key elements of ASI-MV Connect interview responses that may be useful indicators of domestic violence and/or sexual assault. 54 Administration The Administration menu allows the user to select from various administrative or housekeeping functions. These include: Downloading/transferring uses to this computer from your facility s bank Importing or exporting data from ASI-MV Connect, including backing up your data Adding user names and passwords to the computer Performing client list management functions Backing up Your Database File Each time the program is closed, the ASI-MV Connect database is automatically saved on the computer s hard drive. The database is a Microsoft SQL Server database and is stored in the Microsoft SQL server folder. It is also possible to manually backup the data either to the computer s hard drive or to another electronic media (diskette, CD-ROM or DVD). 62 Report Terminology This support is primarily for clinicians who want to know such things as how to present ASI-MV Connect to clients and how to use the Severity Ratings and Composite Scores. Severity Ratings are the clinical summary scores most commonly used by clinicians. They are useful for identifying problem areas that may need attention. These ratings are clinically meaningful and are often used for treatment planning. Severity Ratings are normally not used for research or outcome evaluation because they factor in client responses to "lifetime" questions, which should not change in subsequent administrations. Composite Scores are generated from a number of items in each Domain that refer to the client's behavior, mostly within the last 30 days (not "lifetime"). Therefore, Composite Scores are useful for measuring change in problem status and can be effectively used in research and outcome evaluation. NOTE: Questions that are skipped may make it difficult for ASI-MV Connect to provide an accurate picture of the clients problems. Skipping questions may also result in some domain Severity Ratings and Composite Scores not generating. If there is no Severity Rating or Composite Score generated for a Domain, the clinician can go over the questions in that Domain with the client in a face-to-face meeting. Perception Scores are generated in each domain from the client s answer to the question, In the past 30 days, how troubled or bothered have you been by alcohol problems (or drug, medical, employment legal, family, social, or psychological problems). The answer options are: 0 = Not at all; 1 = Slightly; 2 = Moderately; 3 = Considerably; 4 = Extremely. Motivation Scores are generated in each domain from the client s answer to the question, How important is it to you now to receive treatment for alcohol problems (or drug, medical, employment legal, family, social, or psychological problems). The answer options are: 0 = Not at all; 1 = Slightly; 2 = Moderately; 3 = Considerably; 4 = Extremely. Domains There are six sections (or destinations) to which the client is guided after the main introduction. Each section correlates to a domain, with the exception of the Alcohol and Drug domains they are combined into one section within the program. 63 generated for Alcohol and Drug domains.. Each section offers a slightly different virtual environment designed to make the interview experience less redundant and boring to the client. However, the basic structure of the question and answer interface remains consistent throughout all seven domains. The domains and associated questions are derived from the content of the standard ASI interview. The seven domains are outlined in the following paragraphs. Medical Status These questions ask about hospitalizations, chronic medical problems, medications and perceptions of illness. The purpose is to identify medical problems that need attention. Clients rate their level of concern for these medical problems in the last 30 days, and how important it is to get treatment for them now. Employment/Support Status These questions collect information on occupation, employment status and means of support. The purpose is to establish clients' level of concern over employment problems and how important they feel it is to get counseling for these problems now. Alcohol Use These questions ask about alcohol use and treatment. Clients are asked about their use and asked to indicate their level of concern over their use. Clients are also asked how important receiving treatment or counseling is at this time. Drug Use These questions ask about drug use and treatment. Clients are asked about their use of a range of substances and to indicate their level of concern over their use. Clients are also asked how important receiving treatment or counseling is at this time. Legal Status These questions ask about arrests, convictions and incarceration for a variety of illegal activities. Clients are asked to indicate their level of concern over legal problems, such as arrests, charges and convictions. Inflexxion, Inc. 3/6/

64 Clients also rate how important it is for them to receive counseling or referral for these problems now. Family/Social Relationships These questions collect information on marital status, living arrangements and social relationships. Clients are also asked about their satisfaction in these areas and if treatment or counseling is desired. Psychiatric Status These questions ask about the occurrence of psychological or emotional problems such as depression, anxiety, psychotic symptoms and suicidal ideation. Clients are asked about inpatient and outpatient treatment for these problems, as well as psychiatric medications. 65 Treatment Planning As stated above, Severity Ratings can assist clinicians with treatment planning. High Domain ratings help clinicians target those deficit areas for intervention. It is understood that most substance abuse treatment settings do not have vocational or legal resources. However, an effective treatment plan would address how a client could access those needed resources. The following are samples of treatment plans in different problem areas. These samples are adapted from the National Institute on Drug Abuse publication, "Assessing Client Needs Using the ASI: A Handbook for Program Administrators" (NIH Publication No , 1993, page 15): 1 Medical "Refer to medical clinic for evaluation and treatment. Provide HIV counseling and encourage testing." 2 Employment "Reassess after medical and psychiatric treatments have begun," or "Gather more history to identify reasons why client cannot hold a job. Refer to vocational rehabilitation program." 3 Alcohol Use "Initiate alcohol counseling, refer to AA and support AA attendance." 4 Drug Use "Client abstinent, continue counseling," or "No action." 5 Illegal Activity "Refer to legal aid for consultation," or "Talk with lawyer to clarify legal requirements of Driving While Intoxicated conviction." 6 Family/Social "Refer to social worker for evaluation of family problems and recommendations for treatment." 67 Program Limitations NOTE: The ASI-MV is intended to be a significant part of a complete psychosocial evaluation, however, by itself, it is not a complete psychosocial evaluation. The ASI-MV Connect program has been developed for use with adults seeking substance abuse treatment and is not designed for use with adolescents or children. There are a number of other assessment tools for adolescents. As with any assessment or intervention, there may be some clients for which ASI-MV Connect is not suitable. Clinicians need to determine if a client's current mental status is appropriate for sitting down for an extended session at a computer. If the client is disorganized, agitated, intoxicated or otherwise not in an appropriate state of mind to take the ASI-MV, alternative assessment approaches will be necessary. ASI-MV Connect Trial Version comes with 30 days of unlimited uses. It should be noted that since this is a trial version, these uses cannot be uploaded to the ASI-MV Connect Data Center. Setting Internet Explorer as your Default Browser The ASI-MV Connect installation process is much less complex for Windows 2000 users if Internet Explorer is the default browser. Please follow the instructions below to set Internet Explorer as your default browser, if you are running Windows 2000, and would like to streamline the installation process.
