5 1.0 Introduction to documentation Basic Module describes the scanning and modelling of crowns/bridges, and telescopes. You can use either a mushbite or a stone model as an antagonist - both versions are described. This documentation is aimed at dental technicians and dentists and serves as support documentation when working with CS Ultra Scanner and exocad software. It is assumed that you are familiar with Windows and typical Windows applications. 6 2.0 Introduction to CS Ultra Scanner Info: The illustrations in this manual may differ to those of your scanner or individual scanner components. This manual is constantly being extended and updated, deviations to your scan - or modelling software can therefore not be ruled out. If you have an older version of the software, please contact our support team for the latest software updates. 2.1 Setup As a setup location, a place should be chosen which has a firm footing and transmits no oscillations or vibrations. The work area should not be exposed to direct sunlight and any strong artificial lighting. Strong incidence of light and large fluctuations in temperature may affect the scan quality and/or accuracy. 2.2 Connections to the scanner The scanner has five ports on the back side to which the supplied cables must be connected to. 1 = Scanner power supply (black IEC connector) 2 = Scanner to PC connection (USB 3.0 printer cable A-plug/B-plug) 3 = Scanner to PC connection (HDMI/DVI) Page 5 of 65

7 2.3 Connections to the PC Connect the scanner and the monitor with the PC as shown in the picture. 1 = Power supply PC 2 = Scanner to PC connection (USB 3.0 printer cable A-plug/B-plug) 3 = Monitor connection PC (DVI cable) 4 = Scanner connection PC (HDMI/HDMI cable) Make sure that all cables are firmly attached, screw connections tightened (monitor) and cables cleanly lay out. A kink in the cable can damage it. Page 6 of 65

8 2.4 Scanner accessories The following accessories are included in standard scope of delivery: Calibration plate Model holder Multi-Die-Adapter Optional accessories can be found on our website at Calibration with Exocad Start the "Exocad" software from the desktop Click on Service Mode to start the Service mode Page 7 of 65

9 First you have to calibrate the detection head. Therefor put the calibration plate in the scanner an click on Recalibrate Head A new window opens and you can start the calibration via the Start button The program confirms successful calibration with the following message Page 8 of 65

10 After successful optical calibration with the calibration plate, you now also have the possibility to calibrate the mechanical axis of the scanner. To do this, place an unsawn stone model in the model holder and then place this into the scanner, and click "Yes" Use only stone models without a plastic base plate for the calibration of the mechanical axes You will receive the following message after successful calibration of the axis You can now remove the calibration plate or the stone model from the scanner, close the scanning software from the top right, and continue to Point 3.0 To maintain best possible scanner accuracy, we recommend undertaking weekly calibration. If you change the location of the device, or should the scanner be exposed to large variations in temperature, then calibration should take place more frequently. Page 9 of 65

11 2.6 Calibration without Exocad Turn the scanner on at the back Start the "ULTRA Scan" software from the desktop Click on "New free project" and create a new project Page 10 of 65

12 For calibration you have to switch to service mode Tools / Service Mode First you have to calibrate the detection head. Therefor put the calibration plate in the scanner an click on Recalibrate Head Page 11 of 65

13 A new window opens and you can start the calibration via the Start button The program confirms successful calibration with the following message Page 12 of 65

14 After successful optical calibration with the calibration plate, you now also have the possibility to calibrate the mechanical axis of the scanner. 15 3.0 Create new job order 3.1 Create customers, patients, technicians Start the exocad application from the desktop Click on "New" to create a new job order (date and time will be updated) To create new customers, click on "..." Enter the customer data and then click on "New" Page 14 of 65

16 As a result the customer is then selectable in the drop down menu the same process can be repeated for "Patient" and "Technician" 3.2 Select Indications Select the prepared tooth with the left mouse button The on-screen tips located top left above the date offer a little help. The question mark icon in the upper right corner will take you to the exowiki home page where you can find additional help. Page 15 of 65

17 Select the restoration, material and tooth color Click on "OK" to confirm the selection Repeat the process for other restorations To check, you can hover the mouse pointer over the tooth which will then display the selected restoration again Use the round buttons (red arrow) to determine which teeth are to be splinted together. Green: A connector is created Grey: If there is no connector applied, click on it to activate it if necessary Red: A connector cannot be created because the material of the adjacent tooth is different There is no connector button present: One of the selected construction types does not allow for connectors If you want to assign several teeth restorations, press and hold <Ctrl> and click on the desired tooth with the left mouse button Page 16 of 65

18 3.3 Assign antagonist, adjacent teeth, missing teeth Antagonist, adjacent teeth or missing teeth must be assigned to the corresponding teeth in the same way as restorations (see Point 3.2) Select antagonist type Select "Mushbite" (in Point 4.5, scanning a stone model as an antagonist is described ) Page 17 of 65

19 If all has been selected, click on "Save". The "Scan" button which was previously inactive can now be selected The demo case should now be as shown below If you want to continue at a later date, you can do so once the job order has been saved. The next time you open exocad, at the top of the tool bar click on "Load". You can then load or delete your job orders Page 18 of 65

20 4.0 Scanning (3-digit bridge) 4.1 Scan Stone Model Start the scan software The scan software starts automatically and the welcome screen is displayed Click on the blue icon to load the previously created project A diagnostic message will be shown in the middle part of the dialogue window If the scanner has started correctly, the message "Scanner has started correctly" will appear in green. If an error occurs, a description of the error will be shown in red. Most of these messages are self-explanatory and allow the user to independently remedy the error. Page 19 of 65

21 The wizard is launched by clicking on the "Next" button This wizard will guide you through the scanning process The prepared jaw is placed in the model holder (labial side of the model to the scanners camera or the model holders locking screw) Make sure that there are no reflective points on the model as this can lead to holes in the scan. For reflective surfaces (wax, metals, etc.), we recommend spraying the surface with our "CADstar scan spray". Never spray directly into the scanner! Page 20 of 65

22 In the Project Wizard on the left hand side, you will find the individual scanning steps to be processed Click on "Scan" to initiate the first model scan To rotate the model press right mouse button, to move the model, hold down the right mouse button and the mouse wheel button. Rotate the mouse wheel to zoom. Page 21 of 65

23 In order to obtain better scan results, the scanner can be tuned to the color of the plaster. For conventional laboratory plasters the "Default" setting can be left on. Especially by very dark objects we recommend setting the object color. Make sure that the first model scan is of good quality as the subsequent scans are referenced on this. Click on "Next" when the quality of the scan is sufficient Check the scan and crop with the slider if necessary We recommend cropping only a small portion of the model so that the program has enough data available for subsequent synchronisation Click "Next" to continue with the antagonist Page 22 of 65

24 4.2 Rescanning If gaps appear in a specific area and a rescan is desired, you can click on "Integrate" to perform a rescan To do this, position the model in the direction of the camera, the scanners axis responds directly to your mouse movements Rescanning can be repeated any number of times for all scans (except articulator) CAUTION! DO NOT use the "Integrate" function with articulators and inadequately fixed models. The articulator and /or the models could fall out of the scanner and become damaged! An additional recording can be started by clicking on the camera icon The yellow crosshair shows the recording direction Page 23 of 65

26 4.3 Mushbite as Antagonist Place the complete model (with Mushbite) into the scanner and click "Scan" After the bite has been scanned, there is again the possibility to perform a rescan e.g. using "Integrate" Click on "Next" when you are happy with the scan Page 25 of 65

27 Skip the "Cut Height" step and do not crop the mushbite The program requires a sufficient amount of scan data to be able to synchronize automatically Only crop the bite in the next step Skip this window and click on "Next" After clicking "Next", the below message appears... This message confirms the successful automatic alignment of the scan Page 26 of 65

28 4.4 Cropping the Mushbite After the automatic alignment of the model has taken place, the mushbite may be cropped. Clicking the right mouse button opens the extended context menu where you can choose various tools for cropping and trimming the scans Page 27 of 65

29 Using the "Free closed line" tool by pressing and holding the left mouse button, allows you to draw a boundary line The area is then selected when releasing the left mouse button Then, the selected area can be reversed by clicking "Invert Selection" The marked areas can then be deleted with "Delete Selection" Click on "Next" when you have finished cropping the mushbite Page 28 of 65

30 4.5 Stone Model as Antagonist If you want to use a Stone Model as an antagonist, you must when you create the job order in CADstar order management, first select the scan mode "Two Stone Models, in occlusion". By doing this, you have the opportunity to use your physical articulator or to place two together pinned models into the scanner. Start the application by clicking on "Scan" CAUTION! Only place your articulator or fixed models in the scanner after the initial initialisation of the axis! The articulator and the fixed stone models could fall out and damage the scanner! In order to place the articulator into the scanner, you need our optional ArtiBase Plate. Contact our sales team regarding price and availability. 31 Place the articulator or fixed models in the scanner and then click on "Scan" If you are using an articulator, if possible, remove the supporting pin The articulator scan should, as shown on the image, display enough scan data for both jaws Page 30 of 65

32 If you wish to perform the scan again, then re-click on "Scan" The program automatically overwrites the previous scan CAUTION! DO NOT use the "Integrate" function with articulators and inadequately fixed models. The articulator and /or the models could fall out of the scanner and become damaged. Click on "Next" Place the lower jaw into the scanner and click on "Scan" Click on "Next" after a successful scan Page 31 of 65

33 Skip the "Cut Height" step and crop the lower jaw in the next step The program requires sufficient scan data to be able to synchronize automatically Only crop the antagonist in the next step Skip this window and click on "Next" After clicking "Next", the below message appears... This message confirms the successful automatic alignment of the scan Page 32 of 65

34 After the automatic alignment of the model has taken place, the antagonist may be croppedt. You can now crop the antagonists as described in Point 4.4 Click on "Next" when the step has been completed In the next step, you are prompted to scan the upper jaw Repeat the steps as described previously for the lower jaw When finished, click on "Next" and continue to the scanning of the stumps (Point 4.6) Page 33 of 65

35 4.6 Scanning the stumps on the model Place the model with the two stumps in the scanner WITHOUT the mushbite and WITHOUT direct adjacent teeth In the wizard, select the stumps to be scanned. Not yet captured stumps appear red, whilst captured stumps are later shown in green Click on "Scan" to start scanning the stumps If required, the stumps can then be rescanned (see Point 4.2) Click "Next" when the scanning is complete Page 34 of 65

36 In the next step, the stumps need to be segmented Using the selection tool, select the stumps (left mouse button) Repeat these steps for all stumps Click "Next" to crop the stumps on demand We recommend cutting away only the bottom portion of the stump so that there are enough reference points available for the subsequent synchronisation Click on "Next" until you come to the point, stump alignment (Point 4.8) Page 35 of 65

37 4.7 Scan stumps on the "Multi-Die" adapter Instead of scanning the stumps on the model, there is also the possibility to scan the stumps on supplied "Multi-Die" adapter. To do this, in the Wizard window, set the checkmark on "Use Multi-Die support" and then click "Next" Set the stump on the Multi-Die adapter as requested, and click on "Scan" Click "Next" to crop the stumps on demand Page 36 of 65

38 We recommend cutting away only the bottom portion of the stump so that there are enough reference points available for the subsequent synchronisation Click on "Next" until you come to the point, stump alignment (Point 4.8) Page 37 of 65

39 4.8 Synchronise the stumps on the model In the next step, the software will try to automatically synchronise the stumps with the reference model. This can be visually controlled and, if necessary, be manually corrected by using "Manual alignment". Should the software fail to synchronise the stump, then manual alignment will automatically be started. The reference model is shown in the upper two Windows. On the left you see the reference model, and on the right is the stump scan from the on file library. Set one reference point on the left hand side and then one more at the same point on the right hand side. We recommend indicating at least two points. The result is then displayed in the lower window. CAUTION: Then click on "Best fit" to perfectly position the stump. Click "OK" to continue. Repeat these steps for further stumps, if necessary Page 38 of 65

40 4.9 Capturing adjacent teeth You can now scan the adjacent teeth or clone the reference model to save time by using "Copy From Reference". We recommend to first copy the reference scan and checking the approximal contacts to the adjacent teeth. If these are available, continue by clicking "Next". Otherwise place the model, without the die, into the scanner and click on "Scan", the scan will subsequently replace the reference copy. Page 39 of 65

41 Next, you have the option to manually crop the adjacent teeth again. Cropping the scans reduces the amount of data, resulting in slightly faster calculation times at the end of the wizard and exocad. Once again click on "Next" when the step been concluded Page 40 of 65

42 5.0 Acquisition Check and export to exocad In this section you can see all the scans collated together: Stumps, adjacent teeth, bite/opposing jaw Please recheck the scan data for complete accuracy. Should a scan still show holes or be misaligned, you can simply click the back button in the wizard and repeat the respective scan or the alignment. All other scans will be saved and do not need to be rescanned. If all of the data is correct, click "Next" The software generates a net over all the scanned data and prepares it for exocad In the last step, the data can be exported to exocad. Please check the final scan data one last time and then click the "exo" icon Page 41 of 65

43 6.0 Modelling of crowns and bridges The following steps describe how modelling is carried out with the help of the exocad Wizard. A 3-digit bridge is modelled with an anatomical coping, a reduced crown and a reduced pontic. A mushbite serves the purpose of an antagonist. The job order should now be created as shown. To start modelling, click on "CAD" in CADstar order management. Three default Windows in the basic screen will always be opened Toolbar (right) Wizard Show/hide groups Page 42 of 65

44 The Wizard serves as an assistant which guides you step by step through the modelling. Take note of the displayed text in the Wizard window. This shows you the currently required action. Explore the options that are available in each Wizard window, and click "Next" to continue in the next step. If you want to repeat a step, you simply press on the "back" button. The Wizard offers you predefined default values (depending on the type of restoration and the selected material) for all design parameters. To prevent possible problems with the construction or the fit, we recommend you only change construction parameters in consultation with our support team. Parameters that you modify in the Wizard are applied to all teeth on the construction. Hover the mouse over various buttons in the Wizard to see the HOTKEYS For additional information and support during the modelling, you can click on the question mark in the top right of the Wizard. You will then be automatically redirected (with an enabled internet connection) to the exocad Wiki site where you can get additional information about the current step. Page 43 of 65

45 6.1 Margin Line Detection Start the modelling by clicking on "Design" in order management Set the margin line with a left mouse click on the preparation of tooth 16 The bubble shows a cross section of the preparation and serves as an aid to avoid undercuts Page 44 of 65

46 After setting the first point, an automatic proposal is created by the programme In some difficult cases, after setting the first point, the border is not automatically detected. To help the program, please set more points. Then click on "Start" to restart detection If the point was incorrectly set it can be removed by clicking on "Delete" in the programme proposal, and a new point can be set For individual changes of a line, go to the "Modify/Draw" tab Individual points can now be moved Simply click on the green line to add points To remove a point, click and hold the left mouse button on the point, then click the right mouse button If the margin line is OK, click on "Next" and repeat the same thing with tooth 14 Then press "Next" again Use the "Undo" and "Redo" buttons if you make a mistake Page 45 of 65

47 6.2 Set Insertion Direction For bridges, the "Unique insertion direction for bridges" option is set by default. In other words, the construction is optimised for 3-axis milling machines. The bridge s stumps will all therefore have the same direction of insertion As an aid, the program will automatically calculate an initial proposal. Depending on the situation and the complexity, the initial proposal can be calculated with either better or worse results. In any case, check the proposal and change it if you wish. To manually change the insertion direction, rotate the view to see the occlusal view of the model. Then, click "Set current view as insertion axis" In occlusal view the green edge should be seen everywhere Take care to avoid undercuts If you do have any undercuts, the program will visualise this (like the one shown below) Change the direction of insertion so that any undercuts are avoided as much as possible Page 46 of 65

48 If both stumps are to receive an individual direction of insertion, then the option "Unique insertion direction for bridges" must be unchecked. Each individual tooth must then be selected and edited using the drop-down menu Click on "Next" when this step has been completed 6.3 Crown Bottoms The fit parameters are set out in this first step. As initially mentioned, these parameters are predefined by CADstar. You are free to adjust and change these parameters. Above all, customers are advised to send the milling jobs to us and to only make changes to the parameters in consultation with our support team. If you agree with the parameters click on "Next" Page 47 of 65

49 6.4 Tooth Placement The program now loads teeth from the library and automatically positions these on the model. By using the tools,"move", "Rotate" and "Scale" you have the opportunity to optimise the position of the teeth. We recommend not spending excessive time in the positioning, a rough alignment here is sufficient. Fine tuning can be done later in "Free-Forming". For this step please make sure that the axial position of the teeth, the alignment of the row of teeth, as well as the distance to the antagonists are rough set To help you work more efficiently use HOTKEYS to switch between the three tools. The "Move" tool remains the activated tool. By pressing and holding <Ctrl>, you can now rotate the teeth, and by holding <SHIFT> the teeth can be scaled For inlays, more accurate positioning at this point is extremely important. The more precisely the library tooth is positioned, the better the result will be. Page 48 of 65

50 In order to control the distance towards the antagonist, additional helper windows can be displayed With left mouse button, click on "Show Distances" in the right toolbar You will now see a visualisation of the spacings Based on the colour scale and the display you can see the existing spacing, or to what extent the teeth are already pressing by means of the antagonist After positioning is completed, click on "Next" Page 49 of 65

51 6.5 Free-Forming There are four selectable tabs available in the Free-Forming window: "Free" "Anatomic" "Adapt" "Attachment" With the individual tools in the "Anatomic" tab, you can finely adjust the previously placed teeth With the left mouse button, click on the tooth to change its shape Use the buttons "Cusps", "Tooth Parts", "Entire tooth" and "Ridge" to select the areas to be changed A useful HOTKEY is available in the "Entire tooth" mode. Hold the <Ctrl> key to rotate the tooth and <SHIFT> to scale the tooth. The model remains anchored to the preparation boundary. The program tells you about the HOTKEYS when you hover your mouse over the corresponding buttons. Page 50 of 65

52 In the "Free" tab, you have the possibility to fine tune the model with a virtual wax-knife. You can choose between the tools "Add/Remove" and "Smooth/Flatten" To apply material click the tooth with the left mouse button, to remove hold the <SHIFT> key Using the sliders enables you to set the desired amplitude Change the brush shape for example to "Point of knife" to work on fissures During activated "Smooth/Flatten" and whilst holding the <SHIFT> key, you can apply strong flattening of the areas. Pressing and holding the <Ctrl> key allows you to gently smooth out the tooth. During this, consideration is taken on the anatomical shape. The "Smooth/Flatten" tool can be used very well in order to remove disruptive model fragments Page 51 of 65

53 There are three submenus available under the "Adapt" tab Pontic Under the "Pontic" submenu, you can adjust the pontics relative to the gingiva With "Spacing (negative = intersection)", you select the value of the distance from the Pontic to the gingiva A positive value means that the Pontic still has a certain amount of spacing to the gums, a negative value means that the pontic is pressing into the gingiva. A 0 value sets the pontic directly onto the gingiva Checking the "Pull down to gingiva" offers the possibility to pull down the pontic to the gingiva, that have too much spacing. Best results are attained when the pontic is already roughly aligned via the "Anatomic" tab After you have entered the desired value, click on "Adapt to gingiva" so that the change is applied Page 52 of 65

54 Occlusal This submenu allows you to align the restoration to the antagonist (if scanned). Use the slider to adjust the distance to the antagonists (for anatomical and reduced parts) With the "Cut intersections" button you can grind into the anatomy Applying the "Shape-preserving adaption" button, attempts to preserve the shape of teeth. The tooth is reduced accordingly to achieve the desired value. Approximal In this submenu, you can perform the alignment for approximal contact Page 53 of 65

55 In the "Attachment" tab, different movable Attachments can be added to the construction When selecting "Add" or "Remove", this first action decides whether the attachment parts are added or removed With "Move", "Rotate", and "Scale" you can move and changes the size of the attachment part The insertion direction of the attachment part is changed by using the buttons "Top,"View or "Surface" Page 54 of 65

56 Attachment parts must first be placed and then set using the left mouse button Click on "Apply" to confirm the selection With the option "Cut on gingiva" the distance of the attachment part can be aligned to the gingiva Page 55 of 65

57 6.6 Shrinking If your order contains anatomical coping, reduced pontics, reduced wax-up or overpress parts, the wizard will show you the "shrink" window This step enables you to reduce the anatomical whilst the applied full crown is not changed. The values have already been predefined by us and should only be changed by experienced users. To create a lingual band, check the "Exclude selected parts" option, or use the Brush tool to manually mark the areas which should not be reduced Hold down the <SHIFT> key to delete selected points Page 56 of 65

58 If you want to create a so-called ¾ crown by which the vestibular side of the tooth is less reduced, then set the slider under "Lingual Shrink" to the desired value. If you wish to accept the changes, then click on "Apply" Once this step has been completed, press "Next" The next step allows you to edit the reduced or anatomical form with "Free-Forming" tools. Proceed here as in Step 6.5 and click on "Next" Page 57 of 65

59 6.7 Connectors In the "Connectors" window, you can edit the connectors. Connectors are automatically applied anywhere you have defined splinting in the job creation. You can customise individual connectors in a different ways. The minimum thickness can be set either through the cross section or the height/width. You can toggle between the two modes and adjust the value using the slider You can also choose from different pre-defined forms To apply the adjustments, click on "Apply cross-section/shape change" To change the location of the connector, simply click on the connector and drag it in the desired direction Page 58 of 65

60 By dragging the connector, both sides are changed. To change the connector on one side only, press and hold the <Ctrl > key and then click on a tooth to change the position of the point of attachment. The "Free" tab offers the possibility to customise the connector. Grab each point using the mouse in order to move it in the desired direction To add more control points, press and hold the <Ctrl > key and click on the line you want to add the point to. To simultaneously move several green dots, hold down the <SHIFT> key and move a point Once you have moved all the connectors to their desired positions click on "Next" to move on to the final step Page 59 of 65

61 6.8 Merge and Save restorations In the last window, all parts are put together and merged into one element. There are two tabs to choose from. 62 Under the second tab, "Files", you can delete already merged parts by clicking the "Remove Existing Merged Parts" button The "Optimize for better free-forming and 3D printing" option gives you the possibility to optimise your work for selective laser melting. Constructions can be better edited afterwards with the "Free-Forming" tool. For selective laser melting, the output data must have special properties: No holes or open edges Elements must be completely connected No inner surface or intersections Caution: CADstar does not currently offer laser melting Now click on "I'm done" and then click "Next" The construction will now be stored as an.stl data set in the project folder and the modelling window closed If you still want to make small changes such as smoothing or adding/removing material, you can set this option in advance and make those finishing touches to your work by clicking on Free-form restorations finished parts". 63 7.0 Telescopes With exocad Basic Module it is also possible to make telescopic crowns. Create a new order as described in DentalDB, points 3.0 to 3.3. As restoration, choose "Telescopic crown" You can now go through and apply points 4.0 to 5.0 because they do not differ in the approach of the bridge modelling described above. When point 6.5 has been carried out (Free-Forming) click on "Next" Page 62 of 65

64 7.1 Telescope Insertions Direction As previously described, you must initially (Point 6.2) adjust the stumps direction of insertion. The insertion direction of the telescopes will be set at this point. To determine the direction of insertion, this window basically looks just like the previous window. Set the insertion direction of the telescopes as you wish You may proceed just as in Point 6.2 It is also possible to set the insertion direction by grabbing and pulling the green arrows If you set the insertion direction using the arrows, you do not subsequently need to click on "View direction = direction of insertion"! You do not need to 100% set the direction here as it can be changed later if necessary Page 63 of 65

65 Click on "Next" when this point has been completed In this window, the individual parameters of the telescopes (angle, etc.) can now be set Try various parameters to see the resulting effects The diameter of the milling tool has already been set by us and should not be changed if the job order is to be created by CADstar By clicking each point with the left mouse button you can align the modelling Page 64 of 65

66 To change the telescopes direction of insertion again, click the blue background with the right mouse button. You can then select "Telescope Insertion Direction ". Change the direction of the insertion as desired and confirm again by clicking "OK" There are useful HOTKEYS for telescopes to work faster and more efficiently. Additional modelling points can be set by holding the <Ctrl> key down and clicking the left mouse button. Click the point with the left mouse button and click the right mouse button to remove a point. Press <Ctrl> and click on a point to change all the points of this group simultaneously. When you are done with this step, click "next" to continue. You can fine tune your modelling in the next step by using "Free-Forming" tools such as Smooth/Flatten or "Add/Remove". Once you are satisfied with the modelling you can continue and complete the process just as described in point 6.8 Page 65 of 65
