2 HOW TO REGISTER When you go to myhealth.hooperholmes.com, you will see a LOGIN screen to your left. IF YOU ARE A NEW USER Step 1. Click on New User Registration. Step 2.The next screen will take you to My Account. This is where you select your role: Kit Ordering Customer, Client Medical Review Officer, or Client Nonmedical Review Officer. A Kit Ordering Customer is someone who orders blue dye tablets from Hooper Holmes. For example, someone with a hospital or government agency. A Client Medical Review Officer is a client who is authorized to view all lab results and lab images including results with sensitive information. A Client Nonmedical Review Officer is a client who is authorized to view lab results and lab images that do not contain sensitive information. This is most users. Step 3. You will need to complete some basic information about your account before your account is activated and you are able to log in. Kit Ordering Customer users please go to the next page. Client users please go to Page 5. 2

3 If you are a Kit Ordering Customer, follow the steps below: Enter all the required information as indicated by a red asterisk (*). Enter your User ID. It must contain at least one alphabetical letter with a range of 6 to 15 characters (no spaces). Enter your Password. It must include at least one alphabetical character, one numeric, one capital letter, and one symbol with a range of 8 to 15 characters. Example of a secure Password: Anpw@mHc2. Enter your First Name, Last Name, Address and Phone Number. Answer all four security questions. This will aid in the recovery of your User ID and Password if you forget them. Note: Kit Ordering Customers must enter the Business or Client Name and Address information for billing. Click the orange button Submit Registration. 3

4 You will receive a confirmation notice. Click on the orange button Continue. You will first receive an initial from Hooper Holmes, No-reply@hooperholmes.com, to let you know that you have successfully registered for the myhealth.hooperholmes.com site. Click on the activation link in the . The link will take you to the Account Activation screen as shown below. Click Continue. You will also receive a confirmation from Hooper Holmes to let you know that your account has been successfully activated. You can also click on the link to access the site and begin placing your orders. 4

5 If you are a Client Medical Review Officer or a Client Nonmedical Review Officer, follow the steps below: Enter all the required information as indicated by a red asterisk (*). Enter your User ID. 6 Enter the Account Name(s) and/or Account Number(s) for which you are requesting access. Add additional comments. Optional - Add any access approval documents using the Upload button. Click the orange button Submit Registration. You will receive a confirmation notice. Click on the orange button Continue. 6

7 You will first receive an initial from Hooper Holmes, to let you know that you have successfully registered for the myhealth.hooperholmes.com site. You will also receive a confirmation from Hooper Holmes to let you know that your request to access the site was activated. Click on the link to access and begin navigating the site. IF YOU ARE AN EXISTING USER An Existing User means that you were registered for the old web site and want to start using the new myhealth.hooperholmes.com site. Step 1. Click on Existing User Registration. Step 2. Enter your old User ID and Password. Click the orange Log In button. 7

8 Step 3. If your old Password does not meet the new requirement, you will be required to change your Password. You will need to enter your new Password in twice. Your new Password must span 8 to 15 characters and include at least one alphabetical character, one numeric character, one capital letter and one symbol. Example of a secure Password: Anpw@4m!2. All users will be required to answer four security questions. This will aid in the recovery of your User ID and Password if you forget them. Click on the orange Migrate button. You will receive a User Migration confirmation notice screen. This means you have successfully registered for the myhealth.hooperholmes.com site. Step 4. If you click on the orange Continue button in the confirmation notice screen, you will return to the landing page and can begin accessing the new site, which will allow you to access all data you were able to view on the old site.
