This modification shall be shown in your authorized price list or supplemental price list and distributed to this office, the Schedules Information Center and Using Agencies. Ten (10) copies of this Modification are to be submitted to the Contracting Officer within 15 calendar days of approval. Failure to submit the required documentation or publish a supplemental price list will cause rejection of future modifications and/or cancellation of the approved modifications. The format for supplemental price lists shall include the Description of Schedule, Contract Number, Special Item Number, Modification Number and Effective Date as well as specific changes to the contract.
