1 Transient Ischemic Attack (TIA) Referral Online HOT referral User Manual TIA is a medical emergency. Significant time is wasted in administrative delays of FAX and letter TIA referrals. To improve the referral pathway a new online HOT referral has been designed and tested. It is an Easy Online form which automatically calculates ABCD risk and submits the referral with a receipt to your practice . This system also provides guidance on what the next steps are for the patient (information sheet) dependent upon the risk calculation score. Important Note The TIA Service is NO longer Fax or Letter based due to the urgency of referral The TIA service will operate on a paperless system of Bleep based referral or Online HOT referral only. There is no operating Fax number and Letters will not be regarded as TIA referrals. Notes for Practice Managers: 1. Please remove all old stationary of TIA referral forms. TIA Services are NO longer Fax or Letter based, there is no operating Fax number and Letters will not be regarded as TIA referrals. 2. Please ensure all GP s in practices are aware of changes Please clearly display the poster attached in clinical areas. 3. Please ensure the link is saved on all desktops or as a favourite in internet browsers for quick use. 4. Please ensure GP s are aware of the practice account for receipt of referral. 5. Please ensure GP s have easy access to a printer so the patient information sheet can be printed and given to the patient. 6. If GP s prefer to refer by telephone, please ensure a copy of the attached what to do checklist and patient information sheet is visible to them. 2 Please click and add to favourite as TIA referral form You can also crate shortcut for future use Filling the form Filling this simple one page form is user friendly and it automatically advise you next step. In patient details section please fill the details specifically patient telephone as this the emergency contact we are going to make.you can fill more than one like mobile number as well.
3 In Referrer details section please fill the details specifically practice or your own as this the only receipt or proof that your patient has been referred.it is important for medico legal purpose as well that you have receipt attaché to patient s notes. However we will not use the for any personal communications. Once you have completed the details please click Assess patient button Depend upon patient risk it will advise you next step in patient management. If the system calculated risk means patient is high risk automatically next window will prompt you to refer this patient urgently.
4 It will also give you list what do you need to do in between or if patient refused to go to hospital. If you need any help or have any question with this process you can contact the Acute Stroke Team on Bleep 1120 via the hospital switchboard Likewise if patient is low risk you will see window like this Click on what to do list and file will open with two page document. One is for patient for his information and second is check list for your reference. Print the document and give first page to patient and read and follow the checklist for all the things patients need from you.
5 That is all, your patient details have been passed to Stroke team and you will receive receipt on provided. If you need any help or have any question with this process you can contact the Acute Stroke Team on Bleep 1120 via the hospital switchboard Note For security Reasons this will be available only on then3 network, so only machines connected to that network can use it. With the current set up people will need to be at work to log these events. But that applies to many workplace, fax and patient encounter place.
