1 Dear Student- Athletes, Prior to participating on a team from Embry-Riddle Aeronautical University athletes must provide the Sports Medicine team with the following: current address, emergency contact, insurance, medical alert and health history information. To expedite this process Embry-Riddle Aeronautical University uses an online data entry system. To enter your information, visit The first time you visit the website you will need to enter your Embry-Riddle issued address and click Get Password. Join SportsWare Online: Instruction Go to Scroll to the left of the screen and click the Join SportsWare button. Example Click Join Sportsware Enter your School ID: ERAU Enter the school ID: ERAU. This is required to join the correct school. SCHOOL ID is ERAU NOT YOUR ID GIVEN TO YOU BY THE UNIVERSITY Enter your First Name, Last Name, address and click the Send button. 2 Your request to join SportsWare will then be sent to the Athletic Trainer for review. Once your request is accepted you will receive an with the Subject SportsWare request accepted. Open the and click the link to continue to SportsWareOnLine. Make sure you check your SPAM folder, if you haven t received SportsWare requested accepted in Setting Your Password Instruction Go to Example Enter your Address and click the Reset Password button. You will receive and with the Subject SportsWareOnLine Password Request. Open the and click on the link to reset your password. Enter your address, new password and click the Save button. Updating Your Information Instruction Go to Example Enter your Address and click the Login button. At the top of the page is the Menu Bar. My Info: Update your address, emergency contact and insurance information. Med History: Complete a Medical History questionnaire. Forms: View/complete required paperwork. 4 Emergency Contacts: Insurance Information: You are required to have insurance to participate in intercollegiate athletics. PLEASE ATTACH your insurance card directly into sportsware by scanning or taking a picture with your device and directly adding it under the insurance tab. Medical: Allergies, Medications and personal physician NOTE: HMO POLICIES If you have an HMO insurance: We strongly encourage the policy holder to verify, with their insurance provider, that there is available coverage in Cities of Prescott and Prescott Valley as well as the State of Arizona. The policy holder needs to be aware of costs associated with innetwork and out-of-network providers, physicians associated with their policy, and the potential for additional costs associated with referrals. If they insurance provider cannot provide coverage for Cities of Prescott and Prescott Valley as well as the state of Arizona then the athlete will have to return home for an appointment with HMO participating physicians. If you think or know that your insurance doesn t cover the cities of Prescott and Prescott Valley as well as the state of Arizona please look in the school insurance.
