Step 1: Joining SportsWareOnLine (first time users only)

1 Dear St. Norbert College Student-Athlete: Prior to participating on a team for St. Norbert College, athletes must provide the Athletic Department with current address, emergency contact, insurance, medical alert and health history information. To expedite this process, St. Norbert College Sports Medicine uses the online data entry system SportsWare Online. Since this information will be signed electronically by the student-athlete, it needs to be completed by the student-athlete and not his/her parents. All student-athletes must update/enter their information annually, in summer before reporting to SNC (football by July 1 st, all others by August 1 st ). New SNC studentathletes (incoming freshmen/transfers) also need to complete and submit a Pre-participation Exam (PPE) Form, which can be printed from the SNC Sports Medicine website at: Physical exams from high school are NOT valid, and new PPE s must utilize the SNC form. This form includes a health history which needs to be completed before visiting the physician, as well as sickle-cell trait status form. Completed PPE s need to be submitted to the Sports Medicine staff via mail at: SNC Sports Medicine, 100 Grant Street, De Pere WI or ed as a PDF to sportsmedicine@snc.edu. To enter your information into SportsWare, visit If you haven t used SportsWare previously, you will need to join SportsWare; Do not use your personal you must use your SNC address. For all returning student- athletes, you can skip to Step 2: Updating/Entering Your Information. If you encounter any difficulties and are using Google Chrome, please switch to another browser. Step 1: Joining SportsWareOnLine (first time users only) 1. Go to and select Join Sportware on the right. 2. Enter the school ID: Prevea College and select Next. 1

Enter your legal first name, middle initial, followed by preferred name in parenthesis if different; last name; SNC address (do not use your personal it must be your SNC address); and select St. Norbert College as your Group, then select Send. Your request will be sent to the SNC athletic trainers for approval. 4. Check your SNC for updates. You will receive an from with the subject SportsWare Online Password Request when the SNC athletic trainers have approved your request to join SportsWare. Open the and select the link provided to set your password. Use your SNC address for online access and a 4 character password. Select Save. You are now ready to enter your information, see Step 2. Step 2: Updating/Entering Your Information (all users) Go to and log in using your SNC and password. Once logged in, you will be taken to the athlete Dashboard. The Dashboard is the main menu. 2

3 Under Status, your Athlete Information and Medical History will be listed as INCOMPLETE and you are prompted to click here to complete. You can also get to those sections by selecting My Info or Med History on the black menu bar. Please upload a profile pic ( mug shot only) as well for identification. Updating Your Athlete Information Complete information for each of the tabs: General, Address, Emergency, Insurance, and Medical. Fields with * are required. ID is your SNC ID number. If you are a multi sport athlete, be sure to include each sport. Once all information is entered into ALL of the tabs, select Save. If you try to Save and fields are empty, SportsWare will prompt a notice informing you of which fields are incomplete. Under the Emergency tab, you are able to list both parents together under Primary Emergency Contact if their addresses are the same. If more than 1 cell phone is listed, please indicate the name associated with each cell number. 3

4 Under the Insurance tab, fill out all fields and upload a pic of the front and back of your insurance card where indicated at the bottom of the page (select Browse then Add after selecting the pic for front and back). If your insurance changes during the academic year, you must update it in SportsWare and notify the athletic training staff. Be sure the uploaded pic of your insurance card is always current. If your insurance requires you to see a Primary Care Physician before seeing a specialist, enter Primary Care Physician in the See First field. Policy = the policy/group number, Plan = the insurance plan (example: United Healthcare Choice Plus). This is incomplete unless all fields are filled. Under the Medical tab, enter any allergies or medical conditions in the Alert section. Select from the dropdown menu or type into the field(s). Enter any medications or supplements in the Drugs Taken notes section. Entering/Updating Your Health History Answer all questions yes or no. All yes answers need to be explained in the Comment field. Leave the Evaluator field empty. Save when completed. Again, all new incoming student athletes (freshmen or transfers) also need to complete and submit Pre Participation Exam (PPE) forms in addition to entering information into SportsWare Online. Submitting Forms Please review the Notices and Handbooks BEFORE completing the required forms. Select open for each attachment to view. After viewing, either click on the blue number indicating the number of forms needed to complete or by selecting Forms on the black menu bar. You are able to save your form after starting it, but do not Save and Submit it until it is completed. 4

5 Two forms are completed annually: 1) Statement of Insurance and Medical Access Form, and 2) Consent, Medical Release, & Concussion Info Form. Click select next to each and open. Statement of Insurance and Medical Access Form This section should not be left blank. The check boxes at the bottom of the page indicate which local hospitals/clinics/providers are considered innetwork by your insurance company. You will need to check with your insurance to determine this. Do not leave this section blank! Leaving this section blank will risk additional out of pocket expenses to you since the sports medicine team will refer you to the nearest medical facility in an emergency without knowing if your insurance will cover it. 5

6 Consent, Medical Release, & Concussion Info Form Check those that you permit the SNC Sports Medicine Staff to discuss your injury or condition with. You are responsible for communicating your injury/condition with your parents or coaches; this merely gives the sports medicine staff permission to discuss it with them if they ask about it. The sports medicine staff is unable to discuss your injury/medical condition with anyone unless checked off on this form. Save and submit when completed. Once all information is saved and submitted, the Athletic Trainers at SNC will review and update your participation status on the Dashboard. Your coach will be notified regarding if all information has been completed and received. Concussion baseline testing (for new student athletes and selected returners) is completed at SNC when reporting for your sport, which is the last Sports Medicine requirement before being cleared to participate.
