1 PLEASE PRINT INSTRUCTIONS Dear Student Athlete, This is a requirement from the Athletic Training Department to complete all aspects log in and form completion in order to be cleared to play. Here are the instructions to help you log onto the University of Mobile s sports injury software. The system allows the student athlete to enter in their demographics and insurance information as well as print off required forms. 1. Go to New Athlete 2. If you are a new athlete at UM, you will click join sportsware at the bottom left of the page. (Move onto # 4 in directions) Returning Athlete 3. If you are a returner you can enter your address and enter your password. Click reset your password if you forgot it from last year.

2 NEW STUDENT ATHLETES ONLY: 4. You will need to put the school id UMobile in the blank

5. The next screen will ask you your First and Last Name, your email address, and your Group: University of Mobile.

The next screen will ask you your First and Last Name, your address, and your Group: University of Mobile. **Once you click send, you will need to wait until the athletic trainer accepts you into the system via an to begin filling out the information. ** This could take about 1 2 days to complete so be patient

5 6. The first page you will come to after having been accepted into the system is this page. ** Anything with a RED asterisk is a required field. If you notice your file being incomplete go back and make sure you have not missed one of these fields. You do not have to hit save after every link just at the very end.

6 7. The next page in the GENERAL page. It has information about YOURSELF. Everything with a red asterisk.

7 8. The next page is the ADDRESS page. Enter your parents or guardians information. Please put your PARENTS HOME address and PARENTS cell phone numbers. This is in the event of an emergency.

8 9. The next page is the EMERGENCY page, where you fill out your emergency contact information.

9 10. The next page is the INSURANCE page. This page is VERY important. Transfer all the information from your insurance card to this page. I will need the POLICY, ID, and GROUP number along with the person that holds the insurance plan. You will also need to upload a copy of your card front and back to the system on the bottom of the page.

10 11. The next page is the MEDICAL page. Here you you answer questions about ALERTS (medical issues). I do not need your immunization dates. Only if you are taking medicine and have any alerts.

11 12. The final link is the paperwork page. This is where you can PRINT and COMPLETE your forms. 1. Please do not print these forms simply fill them out using the PDF signature: ABC Form Medical Insurance Fomr Insurance Compliance Form Concussion Form Sickle Cell Form 2. Print only the New or Returning Athlete Pre participation form This form will need to be printed and neatly filled out with PEN. You can choose to receive a physical with your primary physician or local urgent care If you choose that option you must use this form any other form will not be accepted If you are unable to receive a physical back home, and will get one the first week of school at UM, then leave the first page(doctors portion) of the physical form blank and fill out all the history sections. Only mail completed (by you and the physician) forms back to: Jake Lewis 5735 College Pkwy Mobile, AL Completion of sportsware information is to be completed by August 1 st, 2016 of ALL athletes. Failure to do so will result in a delay of participation for your sport. ** Only fill out the forms that are required of you:

12 ** As for the physical forms, you are welcome to receive a physical at home prior to arriving at UM. It will need to be signed and stamped by the physician (M.D.) you are seeing. ** It is strongly recommended to have a physical with your primary care physician back home if you are a NEW athlete. ** If you have EVER been diagnosed with a heart murmur or any other type of heart condition you will need to have a follow up and EKG with a cardiologist prior to arriving at UM. ** If you have seen an orthopedic, athletic trainer, or physical therapist in the last 6 months you will need to supply UM s athletic training staff with physician s notes, MRI report, and/ or athletic trainer/ physical therapy notes.
