1 June 2018 Dear Student Athlete & Parent/Guardian: Prior to participating on a team at Coe College, student athletes must provide Athletic Training Services with medical information such as personal info, emergency contacts, medical history, medical alerts, health insurance, and a copy of a physical examination with clearance to participate in intercollegiate athletics. To expedite this process, Athletic Training Services utilizes online data entry via SportsWare, an online medical record system, where student athletes will need to enter their information. Additionally, there are four (4) required forms for student athletes to review, complete, and electronically sign. These forms are required annually and must be completed by August 1 st, First year and transfer student athletes to Coe will also need to submit proof of a physical, conducted by a licensed physician (MD or DO), completed within 6 months of the start of the academic year to Athletic Training Services. A copy of the physical must be uploaded to SportsWare prior to participation. Hard copies will not be accepted. This information should have been received with your orientation materials. Please note that student-athletes MUST provide Coe College Athletic Training Services with a copy of the physical (via SportsWare) separate from Coe College Health Services requirements. All student athletes must update their entire contact, medical, and insurance information each year and a front and back copy of your current insurance card needs to be on file. These can be uploaded under the insurance tab when updating your information. To enter your information, visit To Join SportsWare you will need to request access to the system by clicking the button Join SportsWare, entering the School ID (see below), and providing full name and Coe College . A request will be sent to Athletic Training Services and as soon as the Athletic Training Staff approves the request, you will be notified by with a link to set a password and begin entering your information. Please use your assigned Coe College address as that is the we will use to contact you while you are enrolled. Joining SportsWareOnLine Go to Scroll to the middle of the screen and click the Join SportsWare button. Enter your School ID: Coe1851

2 Enter your First Name, Last Name, Coe College address and click the Send button. Your request to join SportsWare will then be sent to the Athletic Trainers for review. Once your request is accepted you will receive an e- mail with the Subject SportsWare Online Password Request. Open the and follow the instructions to click the provided link to continue to SportsWare Online and set a password. Setting Your Password Go to (only if you need to reset your password!!!) Enter your Coe College Address and click the Reset Password button.

3 You will receive and with the Subject SportsWare Online Password Request. Open the and click on the link to reset your password. Enter your address, new password and click the Save button. Updating Your Information Go to Enter your Address and click the Login button. At the top of the page is the Menu Bar. My Info: Update your personal info, address, emergency contact, medical alerts, and insurance* information. ***The insurance section will have a place to upload a front and back copy of the insurance card. Med History: Complete a Medical History questionnaire. Forms: View/complete required paperwork. There are 4 required forms. Note: SportsWare will also display You have forms to complete/download. Physicals should be uploaded in the Forms section. Select Add from the left hand side. Upload by selecting a file. The last page of this document includes a checklist of the required medical items that each student athlete must complete. There is a separate checklist for Returning student athletes and First Year/Transfer student athletes. Please complete all this information by August 1, All information and forms must be completed and signed before student athletes are cleared for participation in any intercollegiate activities. Additionally, a copy of a physical exam and front and back copies of a health insurance card must be uploaded to SportsWare before student athletes may participate. In My Info ; Insurance tab, scroll down to bottom of the page, upload files. 7) Complete ImPACT & SCAT5 Testing on campus (see below*) * New (first year and transfer) student athletes participating in sports with a risk for sustaining a concussion will need to complete baseline concussion testing when they arrive on campus before they may participate in team activities. Coe College has designated the following sports as having a risk for concussion with participation: Football, Men s & Women s Soccer, Men s & Women s Basketball, Wrestling, Baseball, Softball, Men s & Women s Pole Vaulting, Men s & Women s Diving. Coe College Athletic Training Services utilizes ImPACT software as a cognitive baseline test to guide return to play decisions following a concussion. Student-Athletes will also have baseline symptom, cognitive, and balance testing completed utilizing the SCAT5 test protocol. Your team s assigned staff Athletic Trainer will schedule the testing when you arrive on campus.
