1 BenefitFocus Open Enrollment Instructions To view or make changes to your benefits, you will need to use the BenefitFocus online enrollment system. Listed below are login instructions and helpful tips to navigate through the system. You will use your Active Directory credentials to log in the system. If you have difficulty logging in, please contact To log in to BenefitFocus: 1. Go to Benefitfocus supports the following browsers: Microsoft Internet Explorer: 11, 10, 9, 8 Google Chrome: 40, 39, 38 Mozilla Firefox: 35, 34, 33 Apple Safari: 8.0, 7.0, Enter your Mesa Public Schools as your user and your network password. If you have any issues logging in, please call the HELP DESK at Enter your MPS & Network Password 1

2 3. Once you have logged in the following screen will be visible. Please click the Enroll Now button indicated on the screen below. Click here 4. Please click on the Get Started button to begin your enrollment. Click here 5. You may add your dependents to your profile by selecting Add Dependent or click next to continue. ** This section ONLY adds your dependent(s) to your profile NOT your health plans. You must click Edit Coverage to Add/Delete your dependent(s) ** Click here 2

To make benefit selections for the 2017/18 school year, click on the Open Enrollment tab. **Make sure you are ALWAYS selecting benefit options under the Open Enrollment Tab for each section*** Click Open Enrollment tab 7. Please review the benefits selected and if any changes are needed use the Edit coverage to select where the change needs to be made. Click on edit coverage to make changes. 3

4 8. IMPORTANT MESSAGE REGARDING YOUR DENTAL DHMO BENEFIT: You must select a Provider for the new DHMO plan OR one will be automatic assign to you. To add your provider click on Show Plan Details Click on Show Plan Details a. Then under Dental Care Provider click Edit Click on Edit under Dental Care Provider b. Enter your provider s code under PCP CODE and provider s name under PCP Name, to search for a provider click Search for Dental Providers Click to search for providers 4 Enter 6-digit PCP code and PCP name

5 c. One you complete your selection, Click Next. Click here d. Review your selection and click Next Click here e. If the selection is accurate, click the Save button. Click here. 5

6 9. To complete your enrollment, you must complete Section 5 Heath FSA and Section 6 Dependent Care FSA. Click on Begin Enrollment Click Begin Enrollment 10. To select the plan click on Select Plan or to decline click on Decline Coverage. Either select or decline coverage 11. If the selection is accurate, click the Save button. Click here. 6

7 At this time, please review the beneficiaries for the life insurance and make any changes needed. 12. Scroll down to Your Life Coverage to review your beneficiary. If you need to revise your information click on the Edit next to the beneficiaries section. Click here. 13. Update informaton on file or click Next to continue. Click here 14. Repeat steps 12 & 13 for each Life Insurance Section, once the selections are accurate, then click Save Changes at the bottom left hand corner. 7

8 15. To complete your enrollment click Complete Enrollment Click here. 16. Once all sections are complete and saved, a confirmation screen will appear with a confirmation number.
