1 HOW TO DO BUSINESS WITH AMERICAN AMICABLE Submission: ALL new business applications MUST be ed or faxed directly to American Amicable. Fax: Phone: ext 1,1,1. When faxing or ing applications, agents are REQUIRED to keep the original application for no less than 90 days. Please note that the original may be requested during this period for audit purposes. The applications must be UNALTERED. New business may be scanned in and submitted at ALL APPLICATIONS WITH MONEY MUST BE MAILED WITH ORIGINAL APPLICATION DIRECTLY TO: American Amicable PO Box 2549 Waco, TX Checking Case Status: Agents are required to register on to check pending cases status. You may also or call American Amicable at opt 1, 1, 1 or policyissue@aatx.com. Requirements and Monies: Requirements MUST be faxed or ed to American Amicable. All initial premium checks should be mailed to the address above. Please be sure to check the website before calling and allow 1 business day for a response. Please be sure to provide policy number and client name when calling. Submission of Contracts: Contracts may be submitted electronically at

2 Contract Scan Instructions Go to Login by entering your agent number. Once logged in, click the Browse button to locate the file containing the contract and supporting documents you wish to send. Select the file and then click Open. The name of the document you are about to send will appear on the screen. Please make sure the correct file has been selected. Click the Upload button to send the file to the Marketing Administration Department You should now see a confirmations stating, Document was successfully uploaded. (If you do not receive this confirmation or receive an error message such as, File is empty, please attempt to upload the document again). The contract will be processed in the order in which it is received in the Marketing Administration Department. You can submit multiple contracts once logged in, simply repeat steps 3 through 6, for each file you wish to send. Requirements/Other Information: Each contract and the supporting documents for that contract should be sent in one file. We can accept file in either PDF or TIF formats. The file size should not exceed 3 Megabytes (MB). The scanner resolution should be set to at least 300dpi. Scanner setting should be for black and white and not color. Contract documents need to be in the following order: 1. New Agent Contracting Set Up Sheet, 2. Producer History Sheet, 3. Agency Agreement, 4. Copy of current license or completed license application, 5. Consumer Report Notification and Authorization, 6. W-9, 7. Compliance Guidelines Statement of Understanding, 8. Anti-Money Laundering Certificate, 9. Annualization Plan Supplemental Agreement, 10. Agent s ACH Direct Deposit Request, 11. any other supporting documents or explanations 12. If Contracting a Corporation include Articles of Incorporation that tie the individual signing the contract to the Corporation When feeding documents into a scanning device, make sure the tops of all documents are fed into the device first and all documents are facing in the same direction.

3 AppDrop Instructions Go to Login by entering your agent number. Once logged in, click the Browse button to locate the file containing the application and supporting documents you wish to send. Select the file and then click Open. The name of the document you are about to send will appear on the screen. Please make sure the correct file has been selected. Click the Upload button to send the file to the New Business Department. You should now see a confirmation stating Document was successfully uploaded. The application will be processed in the order in which it is received in the New Business Department. You can submit multiple applications once logged in, simply repeat steps 3 through 6, for each file you wish to send. Requirements/Other Information: Once sent, the status of the applications should be checked using Agent Efile. Each application and the supporting documents for that application should be sent in one file. Do not save multiple sets of applications and their documents as one file. We can accept files in either PDF or TIF formats. The file size should not exceed more than 3 Megabytes (MB). Application documents need to be in the following order: Cover Memo, Front of application, Back of application, any signed disclosure forms, HIPAA form, Payment (echeck, void check, deposit slip, check), and any other supporting documents. When feeding documents into a scanning device, make sure the tops of all documents are fed into the device first and all documents are facing in the same direction. Do NOT forward original application to Home Office unless instructed to do so by Home Office personnel. Keep the original application until the application has been approved and the policy delivered. Make sure to use the application with the correct state variations.
