1 ELECTRONIC FILING U.S. Fish and Wildlife Service Federal Form 3-186A INSTRUCTION GUIDE If you need assistance: Telephone: (503) Step 1. Before submitting a report, a unique federal ID number needs to be created for each valid permit holder. To create a federal ID number, a falconer has to register into the system through the electronic form as shown in the image below. All (*) indicates a required field. Falconer will also need a valid Oregon Falconer License number to establish an account. (Note: If permit (license) is expired, falconers will need to contact ODFW to renew before registering into the system.) Step 2. Click on Federal ID Issuance System. This will navigate you to the registration form. (See below) 1

2 Step 3. o Please fill in the form (below) in order to generate the Federal ID #. o Fill in all required fields. o Make sure all information is valid. If it does not match what is in the Federal database, you will not get a Federal ID #. o o o IMPORTANT: o Enter you name as it appears on your Oregon Falconry License. o Your State Permit Number is the number on your Oregon Falconry License. When entering your State License/Permit Number add the letters OR to the beginning of your number; this is an Oregon identifier. It is case sensitive: Be sure to use capital letters and the letter O and not the number 0. o If you make a typing error and enter someone else s number by mistake, you will get the message already been issued a Federal ID and a login. Start over and re-enter your Oregon permit number. o If after you enter your correct State Permit Number and you still get rejected, you probably have not entered all of your information exactly as it is in the database, such as using a nickname by mistake instead of a full name. o NOTE: All new permits (licenses) and renewals issued after 2012 will contain the letters OR (for Oregon) in the permit/license number If you fill in an address (optional but recommended) the system will your Federal ID number to you so you will have a record. You do not have to submit a 3-186A to create an account. 2

3 o o After successful registration, a screen will come up with your Federal ID, User ID, Falconer Name and Falconer Address. Record this information; close window, and log back in with those ID s. Step 4: Log On Click on Federal Login 3

4 o Fill in the boxes with the Federal ID assigned and given to you in the Successful Registration box (the one you were just allocated) by the USFWS, and with user ID and password you just created. Step 5. Bird Acquisition and Disposition Report Successful login will direct you to the Acquisition and Disposition data entry form shown in the image below. o Before entering any information, Type of Reporting must be selected (Transfer, Release, Acquire, etc). o Select the appropriate button at the top of the page for the action you are reporting. o The correct form will reload to show only the section you need to fill out. o Your personal information will be added automatically (this data is not editable.) o When you finish filling in your data, click the Save Data button. It will display the information on the next page. Verify that everything is correct. Acquisition and disposition data entry form: 4

5 Example. Released" is selected in the image below. Fields that are required to enter information are enabled. NOTE: Transfer with the Breeder or Propagator If you purchase a bird from a breeder or propagator and want to report to the U.S. Fish and Wildlife Service system, you need to have the breeder permit number. Breeder s permit number has to be entered as MB-###### with correct last name and first name; otherwise the system will not work. (Do not use any nick names.) Step 6. o Click the button to send the information to USFWS; you are finished submitting your 3-186A. Step 7. Print out the page with this information and save in your file. 5

6 OTHER INFORMATION AND INSTRUCTIONS: List of Submitted Reports: o You can access a list of all your submitted reports by clicking the Acquisition and Disposition Submission Report link in the top green bar. o To print the report click Print at the right side of the report. o Retain for your records. 6

7 Change of Personal Details: To update the contact information click Change of Personal Details. This will allow you to change the contact information and the facility address only. To Change Password: To change password, click Change Password link. Enter Federal ID, User ID and then click Next. 7

8 You will be asked to enter the answer to the hint questions selected at the time of registration. If the answer to the hint questions matches in the database, then you will be directed to a new page to enter your new password information. 8

9 Forgot Password or User ID: Click the link Forgot Password or User ID and you will be directed to the page to select an option. If you forgot your user ID click the Forgot User ID button or, if you forgot your password click the Forgot Password button. 9

10 To retrieve your user ID, you need to have your: Federal ID # that was assigned to you by the system, and A valid address Type your Federal ID as FED###### and a valid address. User ID that matched with the entered Federal ID # will be ed to the address that is entered in the box. 10

11 To change your password, you need your Federal ID #, User ID and the answer to the security question. Errors To edit the submitted 3-186A report. State personnel will have the option to edit the 3 186A record submitted by falconer if there is any correction to the information they entered. Falconers do not have the option to edit their record.
