1 Topic: Facility: Audience: Overview: Clinical IS Job Aid QPath / Point of Care Ultrasound (POC US) Education IU Health Facilities QPath - Credentialed Providers Effective Date Current Functionality Contact IU Health Help Desk helpdesk@iuhealth.org QPath is a point-of-care ultrasound (POC US) workflow manager. QPath allows for storing, reporting, retrieving and associating orders. This will meet billing requirements for POC US and allow providers to bill for tests, generating a new source of IU Health revenue for IUHP providers. The following education describes how to enter patient information, document images, split and merge exams and make changes to documentation in QPath for the Cerner electronic medical record. Set Up QPath 1 Turn on POC US machine at the patient bedside. 2 Place cursor in ID field on POC US device. 3 Scan patient wristband with bar code reader. Pt. MRN populates in the field. Note: Some hospitals have placed a code on employee badges for scanning that will auto-populate the user s name in the Reading Dr. field. If so, place cursor in Reading Dr. field and scan badge. If no code is present on the badge, place the cursor in the Reading Dr. field and manually type the dictation number in the field. 4 Press New/End button on the screen and begin the POC US on the patient. 5 Press New/End button on the device when to end the exam. 2 Clinical IS Job Aid Complete the QPath Template 1 Utilizing a computer/ workstation, enter the QPath address in the browser window to complete the QPath POC US report template: The Sign In window displays. 2 Sign in using IU Health Network ID. Note: To add QPath Link to PowerChart Toolbar, see Add QPath Link Button to PowerChart Toolbar section on page four. 3 Locate (but do not highlight) Patient Name on the screen. 4 Click on dropdown arrow in the Exam Type column. Dropdown list of available tests displays. 5 Select appropriate Image Report Template from test options. Page 2 of 11

3 Clinical IS Job Aid Complete the QPath Template, (continued) 6 If User s Name does not auto-populate in the Operator column, manually enter it, using the following directions: a. Click the in the Operator column. b. Click the by the appropriate User Name in the dropdown User list. 7 Highlight Patient s Name on the Patient list. 8 Click Images/ Worksheets button. 9 Complete Exam Information on the Images/ Worksheets tab. Note: Current Patient information appears on the top line, above the report. 10 Mark exam as Clinically Indicated. 11 Complete template(s) associated to images. Note: If all report information has been entered and there is not a need for additional actions (such as splitting/ merging exams), the user may log out by clicking on their name and signing out in the upper right corner of the screen. 4 Add QPath Link Button to PowerChart Toolbar 1 Locate Explorer Menu button in PowerChart Toolbar. 2 Right-click Explorer Menu button. Customize dropdown option displays. 3 Click Customize dropdown option. Customize Tool Bars window displays. 4 Click Add. Add User Links window displays. 5 Type QPath for Display Name. 6 Type for URL Name. 7 Click OK. Qpath displays in box under My Custom Links. 8 Click Close. Customize Tool Bars window closes. QPath button will appear in the PowerChart Toolbar. Page 4 of 11

5 Split and Merge Exams Exams can be split into one (1) or more additional exam(s). Exams previously split can be merged back together. Icons on the left hand side of the Exams-List screen indicate the parent (original) exam and the split exam(s). Images split into the new exam can be cut (removed) from/ copied to the original exam. The original exam will retain a copy of the images. Note: Only users with appropriate access rights can split/ merge images. Split an Exam into Additional Exams 1 Open the exam in either the Images/Worksheets or Images screen. 2 To split image(s) into another exam place a checkmark in the box(s) below the thumbnail image(s). 3 Click Split button at the top of the screen. Note: The Split button will be greyed out and cannot be selected until at least one thumbnail image is checked. Page 5 of 11

6 3 (continued) Split an Exam into Additional Exams, (continued) The following screen appears: 4 Images can be copied or cut (removed) from the original exam by selecting or deselecting the check box on the Split screen. Leave the checkbox unchecked on each image to cut the images from the original exam and move to the Split exam. Check the checkbox of each image to copy the images from the original exam to the Split exam. 5 Select the Exam Type for the new exam from the dropdown list. 6 Check/ uncheck the Copy attachments (from the original exam) box to copy/ not copy to attachments for the new exam. 7 Click OK. A new exam is created and a window indicating both the Original and Split exams opens. Note: Patient demographics are duplicated for the new exam. 8 Click Refresh button at the top of the screen to return to the List screen. Page 6 of 11

7 Clinical IS Job Aid Merge Exams Back into Previous Exam (Reverse Split Exam) 1 On the Exams-List screen, select the Split exam to be merged. Note: Select the line but do not open the exam. 2 Click the Merge button at the top of the screen. Note: The Merge button cannot be selected until a split exam has been selected. Select Show related exams to display a list of all the exams related to the split exam selected. OR Select Merge the exams to complete the exam merge. 3 Select the exam to be merged. 4 Click Merge button. The exam no longer appears on the list; the exams and images are merged back together. 5 Click Sign in the Physician Signature section. The Accession Number field populates after the Physician Signature is signed. Page 7 of 11

8 Merge Exams Back into Previous Exam (Reverse Split Exam), (continued) The Physician s signature triggers a Time Stamp in Cerner. Four icons populate in the All Column in QPath. Icon Key Order transferred to Cerner Image storage EMR documentation complete Image bill transferred Note: The All field will be icon- free initially. Icons populate in the field as information is entered and transferred to Cerner. Submit a QPath ticket to Helpdesk if the icon turns red. This signals that an error occurred during transmission of the report. POC US orders auto-generate as Routine with a Date and Time in the Orders band under Other Departments. Location of POC US Cardiac Report The POC US Cardiac Report may be found in two separate bands within the Patient chart: Radiology Reports folder within Clinical Notes band Page 8 of 11

9 Within the Results Review band in the Radiology tab Location of POC US Cardiac Report AND the Diagnostics tab Note: Add POC US documentation to PowerNote by choosing Radiology Interpretation last 24 hours. This is located within the Radiology tab of the Results Review band. 1 Log in to QPath. 2 Locate and open the exam to be modified. 3 Edit exam. 4 Click Unsign in Physician Signature section. Make Changes to Documentation 5 Click Sign in Physician Signature section. An updated Time Stamp populates on the QPath template. Page 9 of 11

10 Clinical IS Job Aid Make Changes to Documentation, (continued) 6 Submit a Helpdesk ticket, stating Edits/ Changes have been made in QPath. Changes appear in Results Review with a ( c ). Additional Learning Resource on QPath: Overread Physicians For Quality Assurance (QA), Overread Physicians will receive an notification via their IUHealth.org when an exam has been submitted by the Learner Physician. The Overread Physician reviews the QA exams in QPath to determine if the images should be used for education only or are clinically indicated. If the Overread Physician changes the exam type to Clinically Indicated and signs the signature field, the images and report will populate to Cerner, and technical fees will be charged. If the exam type remains as Education, the images and report will not populate to Cerner and no technical fees will be charged. Opening and changing exam to Clinically Indicated: 1 The Overread Physician receives an to review the QA exam in QPath. 2 Click Open this exam link. 3 Log into QPath with User ID and Password. 4 The Overread Physician completes all applicable sections (Image Quality, Accuracy of Interpretation, Exam Information, etc.). Note: Education is selected as the exam type in the Exam Information section. The Overread Physician determines if the images remain as Education or Clinically indicated. Page 10 of 11
