1 504 PLAN APPLICATION INSTRUCTIONS BASICS: Recommended browsers: Chrome, Safari, Firefox To log in: 1. Open a Web Browser and enter your district specific address: a. [yourdistrictname].schooldata.net/v2/504-plan (Another way to log in is using a quick Login from Homeroom. Click on the App Selector ( waffle icon) and choose 504.) 2. Type in your username and password, click Sign In. 3. If you do not remember your password, click Reset Password To change password: 1. User clicks on their name at the top right of the screen 2. Choose My Account 3. Under Update, type in the new password twice and the current password. To log out: Click on your name in top right of screen and choose Log Out 1

2 1 st tab: Creating a Prior Written Notice form allows you to purposefully inform parents/guardians of the action-taking place, whether that s proposing or refusing to initiate or change an evaluation, eligibility, 504 plan, re-evaluation, disciplinary action, and/or other. It includes the following: Text field to enter in name of parent/guardian/ or other Select the date the Prior Written Notice is created Select Student Name (which will auto populate the printable with DOB) Select either proposing or refusing based on purpose Select either initiate or change based on purpose Select all applicable or select other and type in text for the purpose of this prior written notice Type in text for the description of purposed or refused action Type in text for the reason as to why you are proposing or refusing to take action 2

3 2 nd tab: This tab allows you to view all prior written notice forms you ve created as the 504 Coordinator. Student the Prior Written Notice was created for Date Prior Written Notice was created. Click on Date to sort the column by date edit the form print the form delete the form 3

4 3 rd tab: Creating an Eligibility Form is for the purpose of determining if the student has a mental or physical impairment which substantially limits a major life activity(ies), which meets or does not meet Section 504 eligibility criteria. Select Student Name using the student name selector Select the 504 Coordinator creating this eligibility form Select 1 of the 3 Eligibility Criteria and Determination Options Optionally, type in text for the Area of Disability. Select one or more accommodation types the student could possibly need. These can be customized per district. 4

5 3 rd tab (continued): Pick the date the eligibility form is created Pick the date the prior written notice was sent Pick the date the prior written notice was received for record purposes Pick the date of the meeting Enter text for the nature of the concern Enter text for the impairment description Enter text for the impairment limitation description Enter text for any other notes you deem necessary After the form is saved, it can be printed and/or converted into a 504 Plan 5

6 4 th tab: The 504 Eligibility plan will be saved with an automatic pre-determined label for consistency where you can view student s eligibility forms you ve created. 7 5 th tab: The 504 Plan is meant to guide and assist school teams in serving their students with disabilities in a way that assures a quality education that is in compliance with state and federal laws. 1 st section: Details Select student from the student selector who the plan is for Select the 504 coordinator for this plan Optionally, type in text for the Area of Disability. Select the specific impairments. These can be customized per district Once you select an Impairment, you have the option to add a note. To add a note, click on the chat bubble and a note box will pop up. These notes will display on the printable 504 Plan. Select the implementation date for which this plan will be put in place Select the review date for when you will review the plan. This auto populates to 365 days later, but you can change if needed with the date selector 7

8 2 nd section: Accommodations The Accommodations tab allows you to select multiple accommodations under multiple categories that will need to be put in place according to this 504 plan for the student. These are customizable per district, and should match the accommodation types selected on the Eligibility form. Once you select an accommodation, you have the option to add notes such as: how much extended time the student can have on the test. To add specific notes, click on the chat bubble and a note box will pop up. These notes will display on the printable as well as in the student spotlight widget. 8

9 3 rd section: Communications edit the form delete the form Add a communication to keep track of correspondences with guardians/parents etc. while the 504 plan is in place. 1) Select the communication method (customizable per district) Mail Parent/Teacher Conference Phone Send with Student 2) Select if it was sent to parent(s) or not 3) Type in the actual communication or summary of the communication This part of the 504 Plan does not print on the report. 9

10 4 th section: Files Upload a file such as a health file, or doctor s note into the 504 plan so that all information is in one place. Either drag and drop your files or choose a file to upload. Once your file is uploaded, it will appear on the uploads list. Here you can select three options: download the file edit the file to name it delete the file 10

12 7 th tab: All 504 Plan application documents created for students are accessible in this view, including ones not created by the logged in user. Click the type of document to review and a list of the available documents for that student opens. 12

13 504 PLAN APPLICATION DATA ACCESS & USE Once data is entered into the 504 Plan application, impairments and accommodations for those students can be utilized in other Applications by staff that have access to the students. This way all educators working with the students can have updated and quick access to the information. 1. Enter your district specific address: [yourdistrictname].schooldata.net and log in a. (or click on the App Selector ( waffle icon) and choose Homeroom.) 2. Go to the Lookup widget, search for a student or a class 3. Click the magnifying glass on the selected student or class Homeroom Student Spotlight The Student Spotlight 504 Widget is to display the 504 Application data for a single student. Homeroom Class Spotlight The Class Spotlight 504 Widget is on the Class Spotlight tab. It displays 504 Plan data for students in the selected class. 13

14 Homeroom Student Groups widget The Student Groups widget is located on your dashboard. It allows you to manage your collection of custom Student Groups, with Edit, Delete, Print, Download and Copy tools. Use this widget to make new static Student Groups or dynamic Student Groups. Add the 504 Plan filter group, then select either Impairments, Accommodation Types, or Accommodations to create a group that can be visable and used in other Homeroom widgets or Applications or to collect a list.
