1 National Audit of Dementia (NAD) Guidance for online data submission NAD3 2016
2 About this guidance This guidance is provided to assist your hospital in submitting data online for the National Audit of Dementia (NAD) Data collection Data collection will commence online on 18 April 2016 and conclude on 28 October Please see a timeline of deadlines for each tool on the website. Accessing the online data entry form The NAD audit lead for your hospital will be mailed your username and password. You will need this information to access the online data entry form. Your username is unique to your organisation and enables the NAD team to link your submissions to your hospital. Please contact your local audit lead in the first instance for your hospital username and password. For further support or information please contact a member of the NAD team. 2
3 How to access the data collection form online Step 1: Link for the online data collection form is: Step 2: Click Login at the left hand side of the screen. (See Figure 1) Figure 1: Login This leads you to the Login page where you need to enter your username and password (See Figure 2). Your hospital username and password have been mailed to the NAD lead. Enter your username and password (password is case-sensitive). Step 3: Click on the Log In button located in the bottom right hand corner of the screen (See figure 2). Figure 2: Username and password entry This takes you to the Projects page (See Figure 3) where the link to the online data entry form is available. Your hospital code is displayed in the top right hand corner. 3
4 Step 4: Clicking on the survey link takes you to the online data collection form (See Figure 3). Figure 3: Projects page Navigational tools Links to help you navigate through the form are available at the top left hand corner (see figure 4a) of the survey and at the bottom of the page (See Figure 4b). 5 The navigational tools include options to go to the previous page, next page or to cancel, clear, save, and submit your data. Clicking cancel takes you back to the Projects page and wipes the form. Please note: that clicking clear deletes all the information you have completed on the form, not just on that specific page. The save option enables you to save partially completed forms for completion at a later date. N.B. This option is only available for the Casenote Audit and Organisational Checklist. Data missed or completed inaccurately on the form will be displayed in the Completion Errors box. Details include the page where the error(s) are, and what the error for that particular question is. To return to the question simply click on the link in the box (See Figure 5). N.B. Please ensure all completion errors for page are corrected before moving on to the next page. Figure 5: Completion errors PLEASE NOTE: THERE IS NO AUTO SAVE FUNCTION AVAILABLE. PARTIALLY COMPLETED FORMS WILL NEED TO BE SAVED AND ACCESSED AGAIN BEFORE DATA COLLECTION ENDS, FOR COMPLETION (SEE HOW TO ACCESS A PARTIALLY COMPLETED FORM ). 5
6 FORMS WILL TIME OUT AUTOMATICALLY AFTER 20 MINS. SHOULD YOU REQUIRE EXTRA TIME PLEASE ENSURE YOU SAVE PARTIALLY COMPLETED FORMS. ENSURE ALL COMPLETION ERRORS FOR A PAGE ARE CORRECTED BEFORE YOU MOVE ON TO THE NEXT PAGE. How to access a partially completed form A receipt number will be generated for each form you save for completion later (See Figure 6). You should save this receipt number in order to return to partially completed forms. Figure 6: Receipt number for partially completed forms To access the incomplete form, navigate back to the Projects page and click on the message You have partially completed copies of this form. Do not click on the title of the form as this will take you to a new blank version. (See Figure 7) A list of saved forms next to each survey will appear showing the date and time it was last saved. To access a previously saved form, either click on the date, time and receipt ID for the form you wish to access (see Figure 7) or return to the main Projects page, click on Receipt and enter the receipt ID (See Figure 8). You will be able to return to and edit previously saved forms until the end of the data collection period for that tool. 6

8 Support and guidance from the NAD team The NAD team is available to provide support Monday to Friday during office hours ( ). A printable sample version of the data collection tools, guidance documents and the link to the online form will have been sent to your NAD audit lead.
