1 Logging in to the National Healthcare Safety Network (NHSN) 1. Log in to the SAMS Grid Card on the left-hand side of the screen. 2. Using the CDC Grid Card assigned to you, enter the corresponding characters into the blanks. April 2018 Page 1 of 6

2 3. Once logged in, click on the NHSN Reporting hyperlink. 4. You will be brought to the NHSN Landing Page. April 2018 Page 2 of 6

3 Accepting the Healthcare Personnel Safety and Patient Safety Components Acute care hospitals are required to accept two components: Healthcare Personnel Safety and Patient Safety. First, we will show you how to accept the Healthcare Personnel Safety component, and then how to accept the Patient Safety component, all in just a few easy steps. Healthcare Personnel Safety Component 5. On the NHSN Landing Page, select the Healthcare Personnel Safety component from the Select component drop-down. 6. Next, select your facility from the Select facility/group drop-down. Then, click Submit. April 2018 Page 3 of 6

4 7. You will be brought to the NHSN Healthcare Personnel Safety Component Home Page. 8. This screen will display an alert at the top that the NHSN Agreement to Participate and Consent has NOT been completed. A hyperlink will be available to click here to complete the form. 9. Once the consent form displays, review the contents, navigate to the bottom of the page, and select the checkbox to Accept the Healthcare Personnel Safety component. Then, click Submit. 10. A pop-up box will appear, informing you that the consent is complete and the component activated. Click OK. 11. At this point, you have successfully consented to Healthcare Personnel Safety component. However, you will need to return to the NHSN Landing Page to consent to the Patient Safety component. 5 Patient Safety Component 12. To switch components, return to the NHSN Landing Page by clicking on the NHSN Home tab on the left-hand side of the screen, at the top of the navigation list. 13. Once you have returned to the NHSN Landing Page, select the Patient Safety component from the drop-down, as well as your facility/group. Then click Submit. 14. You will be brought to the NHSN Patient Safety Component Home Page. April 2018 Page 5 of 6

6 15. Again, this screen will display the alert that the NHSN Agreement to Participate and Consent has NOT been completed. A hyperlink will be available to click here and complete the form. 16. Once the consent form displays, review the contents, navigate to the bottom of the page, and select the checkbox to Accept the Patient Safety component. Then, click Submit. 17. A pop-up box will appear, informing you that the consent is complete and the component activated. Click OK. The process for accepting the updated Agreement to Participate and Consent is now complete.
