1 EMAR: Reports and Service Reports This manual covers the available EMAR Reports and Service Reports. NOTE: If your software is hosted on the Eldermark server, to print you will need to have ThinPrint installed. Reports: The standard reports in EMAR are the Resident List, Census Reports, 24 Hour Report and Weight Report. Reports are accessed from the Report Menu. Click on the word Reports and a listing of available reports is displayed. Click on the report you would like to print. Screen Reports: Screen reports give you a preview of the report. You can sort on columns and/or modify the report criteria with the preview on the screen. Additionally, you can copy the data and paste it into Excel. Several of the reports allow you remove data. The specifics will be covered under each report. Resident List: This report lists the active residents in your community as of the current date. It also tracks residents on leave and the reason for the leave. When you click on Resident List, the following window will open. Column headers. The default sort is by resident name. You can change the sort by clicking on any of the other column headers. If you want to print this report in unit order, click on the Unit header. The report prints how it is sorted on the screen.
2 Copy: This report can be copied to excel by clicking on Copy. Click on Copy to copy the data. Click on Copy with Headers to copy the data and the headers. Once the information is copied, you can open Excel and paste the data into Excel. Double click / right mouse click for more options: If you double click on a line of the report, Resident Care Management will open to this resident s screen. You can also right click on the line and select Open Resident. Print: Click on the Print button to print the report. Done: Click on the Done button to close the report window. 3 Census Reports: There are two census reports available in EMAR. When you click on Census Reports the following window will open. The census reports are screen reports. Click on the desired report to highlight and click OK, or double click to open the screen report window. The Census Report by Head Count will give you a count of all active residents in your community. The Census Report by Units Occupied will give you a count of occupied units in your community. These reports do not always match. For example: If you have a couple in one unit, the Census Report by Head Count total will be one more than the Census Report by Units Occupied. When the report window opens, select the month you would like to print the report for. Use the up or down arrow to select the month. Then click on the Load button. Census Report by Head Count: This is a static report, all numbers are as of 12 midnight. Census changes will be reflected the following day. 4 Census Report by Units Occupied: This is a static report, all numbers are as of 12 midnight. Census changes will be reflected the following day. This report is a multi-facility report and shows all communities in your database. It lists the head count (HC) for each day, the average for the month, the monthly goal and variance. It is separated by regions if those have been setup. Move-In and Move-Out numbers are listed as well. Note: The head count goal and regions are setup on the Company Marketing Tab. Select: The Select feature is used to limit the rows displayed on the report. Highlight the rows you want to remove or keep. Click on. Click on Keep Selected Items if you only want to see the highlighted rows. Click on Remove Selected Items if you do not want to see the highlighted rows. Print: Click on the Print button to print the report. Done: Click on the Done button to close the report window. EMAR Direct Connect: Reports and Forms Revised 3/1/12 4
5 24 Hour Report: You must have at least one unit station set up for your company and you must have units assigned to a unit station to utilize this report. Click on 24 Hour report to open. The report defaults to the current date. Select the Station you are responsible for by clicking on the dropdown. Then Select the Shift by clicking on the dropdown. 6 The residents for the selected station will be listed and the shift you selected will be highlighted in yellow. You can adjust the height of the row by clicking on the drop down in front of Row Height and selecting a number up to 9. Note: The row height is set per employee. 7 This report has a code column and a comments column for each shift. To enter codes, type in the number from the key at the bottom of the report OR right click on the row for a pop up list. Click 1 line or multiple lines to select the code or code(s) desired and click on select. The codes are listed at the bottom of the screen report and will print out on the report. Use your tab key or your mouse to move to the next person. The information is saved as you type. Once you have completed the report for your shift, lock the report by clicking in the check box in front of the word lock. NOTE: If you press the enter key, the report will be closed. 8 Once the shift is locked, only the employee who locked that shift can unlock and edit if needed. Once locked, you will see the initials of the employee who locked the shift along with the date and time it was locked. You can view reports from previous days by using the calendar lookup, typing in the date or using the previous button by the date. This report can be printed by clicking on the print button. You will have to print each station separately. Done: Click on the Done button to close the report window. EMAR Direct Connect: Reports and Forms Revised 3/1/12 8
9 Weight Report: This report will pull weights from the Medication Delivery History if you are using EMAR and recording weights in the system during medication passes. If you are not recording weights during the electronic medication pass, you can enter weights directly into the report. Note: This report is divided by Unit Stations if you want to use this report you must have stations setup in your database. Select the Weight Report from the Report Menu. The Report Month defaults to the current month. To change the month, click on the previous or next buttons. Click on the drop down to select a station. After you have selected your criteria, click the Load button. EMAR Direct Connect: Reports and Forms Revised 3/1/12 9
10 The report will list all of the active residents on the station selected. Completed weights will have a white background. You can enter weights directly into the report in any cell that is still yellow. The report shows weights for an entire year. You can enter weights into any cell that is still yellow. Click into a cell with a Yellow background to type in the resident weight for the month. Again, if you have entered weights into the EMAR passing screens they will pull into the report for you. 11 Once you have entered a weight, if you press the tab key the focus will move to the next yellow cell in the row. Once you have entered the last weight on the current row, when you press tab you are moved to the first field of the next row. The tab or enter/return keys will move you through the fields. If you press the enter key on the number pad, the record will be saved and this window will be closed. Shift + tab will move you back a field. You will be able to edit any weights that are in cells with a yellow background. As you are entering the weights the Percentage of Change will be calculated. 12 The Percentage is calculated using the following formula. Current Weight minus (-) Prior Weight divided by Prior Weight 1 month: [(Current Weight Month Prior Weight)/Month Prior Weight] 3 month: [(Current Weight 3 Month Prior Weight)/ 3 Month Prior Weight] 6 Month [(Current Weight 6 Month Prior Weight)/6 Month Prior Weight] 12 Month [(Current Weight 12 Month Prior Weight)/12 Month Prior Weight] Once you click on the Done button, the background turns white and you will not be able to edit the weights on this screen. 13 Service Reports Resident Specific Reports: The Service Reports available in EMAR are the Face Sheet, Kardex, Medication Sheet, Physician Order Sheet and Progress Notes. These are the resident specific reports. To print these, you MUST access the Service menu. The Service menu is only available when Resident Care Management is open. From the Resident listing screen or from an opened resident record in Resident Care Management, you will see the option of Service in the menu. If you have a resident record open, when you click on the Service menu, you will be able to print any of the reports for that resident only. From the Resident listing screen, you can select an individual resident or a group of residents to print. Click on the resident name to highlight, prior to clicking on Service. If you want to print the report for multiple residents, hold down your Ctrl key and click on all of the residents you want to print. Note: If you do not highlight any names from the Resident listing you will be printing for ALL residents. Face Sheet: Select the resident or residents you would like to print the Face Sheet for by highlighting the name(s) on the Resident listing screen. Click on Service. Click on Face Sheet. Note: If the title of the report is changed in the company settings, the Service Menu will still show Face Sheet. If you highlight one resident, the following window will open. 14 To print the Face Sheet for the selected resident, click on 1 Highlighted Record. To print the Face Sheet for all residents, click on, <#> Listed Residents. The Print Options window will open. Your printer will be listed in the name field. If you want to preview the report prior to printing, check the Print Preview option. Note: You do not have to preview prior to printing. Click on the printer icon to print. Note: This only prints the page you are viewing if this is a multiple page report. Click on the down arrow to view the next page. This will close the report if it is only one page. The up arrow will be grayed out if you are on the first page. In a multiple page report, you can click on the up arrow to go to the previous page. Click on the magnifying glass to make the preview larger. Click on the X to close the preview. EMAR Direct Connect: Reports and Forms Revised 3/1/12 14
15 Face Sheet Example: Note: The Face Sheet is a customizable report. The title and items that print on your face sheet may be different than what you see in this example. The title of the Face Sheet and the items that print on the face sheet are setup in File Setup Company Clinical Tab Face Sheet Setup and can only be changed by an employee who has access to the Company Table. 16 Kardex: The Kardex contains Diet, Diagnosis, Orders, Advance Directives, Allergies, Family Contacts, Religion, Date of Birth, Age, Gender, Marital Status, Pharmacy, Hospital Preference, Health Plan, Physician, Unit, Admit Date, Medical Record Number, Resuscitate status, Medication listing and Services (if your site uses the full clinical module). To print, select the resident or residents you would like to print the Kardex for by highlighting the name(s) on the Resident listing screen. Click on Service. Click on Kardex. 17 Medication Sheet: Select the resident or residents you would like to print a Medication Sheet for by highlighting the name(s) on the Resident listing screen. Click on Service. Click on Medication Sheet. Note: If the title of the report is changed in the company settings, the Service Menu will still show Medication Sheet. The Choices window will open. To print a Medication Sheet, click Print Medication Sheets for Selected Residents. To print a blank Medication Sheet or Med Note Sheet, click on either of those options. Note: The blank sheets from this menu will have the Month/Year, but no resident demographic information. If you want resident demographic information completed on the bottom of the form, with no medications, select the first option. When you select Print Medication Sheet for Selected Residents, the following window will open: The title you have selected for your Medication Sheet will be shown on the Options window. The medication sheet title in this database is setup to print Medication Observation Record. The options selected are setup at the master level. The options can be modified before you print. The next time you print a medication sheet the options will return to the facility default settings from the master. Details on the options shown to the left will follow. 18 Report Range: The range does not have to be a calendar month. The report date range defaults to the next calendar month. Edit the date if you want to print for a prior month. If you print for a future date you will have a medication sheet to record medication passes on. If you print for a past date, you will have a completed medication sheet. Select Include Med Notes Sheet if you want to print this. The medication notes sheet is where PRN medication given reason/results and medications held reasons print. If you print for a future date you will have a medication note sheet to record activity on. If you print for a past date, you will have a completed medication notes sheet. Select Print Meds, to have the resident medications print on the form. Select Don t Print Meds (Blank Med Sheet) to have a form with the resident information and the month/year and days, but no meds. This could be used if you need to manually add a new medication to a paper medication sheet. Order Meds By: This is used to set the sort order of the medications on the print out. If you select Medication Name, the resident s medications will print in alphabetical order. If you select Passing Time, the resident s medications will print in passing time order, with an entry for each passing time of the medication. Meds Included: Select Scheduled & PRN to print all medications on one form. If you always want PRN medications to start on a new page, you will need to Print Scheduled only and then Print PRN only. Select this if you want the Effective Date, End Date, Prescription Number, Prescribed by, and Pharmacy for each medication to print on the medication sheet. Select this if you want the resident s Date of Birth (DOB) to print on the medication sheet. sheet. 21 Example of Completed Medication Notes: Note: On the Medication Sheet and the Medication Notes, the employee name, title and initials will be completed. 22 Physician Order Sheet: Select the resident or residents you would like to print a Physician Order Sheet for by highlighting the name(s) on the Resident listing screen. Click on Service. Click on Physician Order Sheet. Note: The Physician Order Sheet is customizable. The options are configured in File Setup Company Clinical Tab Physician Order Sheet. If you have access to the Company table you will also be able to edit the setup from the print window. Once you have completed the options, click on Print. 24 Progress Notes: Select the resident or residents you would like to print a Progress Notes for by highlighting the name(s) on the Resident listing screen. Click on Service. Click on Print Progress Notes. Note: Progress Notes can also be printed from the Notes tab or from File Notes. The default selection is the selected resident. You have the option of selecting all active residents by clicking on Use < # > Residents in List. If you mark Since Last Printed you will print new notes entered after the last printing. If you have the Risk Management module, you will not mark Include Incident Notes. Entered or last Modified: Complete the date range you want to print for.
