1 Correct completion of the Module 4 Practical Test Assessment Report Background This document sets out the methodology for assessing and recording individual question scores and calculating the overall mark (pass / fail). It is essential that Examiners listen carefully to the replies and actions of the candidate to the question(s) asked. This is crucial to obtaining a correct assessment of the candidate s ability and knowledge and therefore the correct test result. Assessing individual questions Under each question, there are performance criteria or outcomes. These are the key knowledge indicators (KKI) for the assessor to record the candidate s response(s). If the candidate makes acceptable reference to the performance criteria listed, a mark is placed in the left side box. At the end of the question, the examiner totals the number of KKI boxes marked to produce the individual question score. The resultant score to each question will be in the form of a single mark placed in one of the scoring boxes on the right side, which equates to; 5%, 10%, 15% and 20% Percent. Note that a candidate can score 0% i.e. nothing. A zero percent would result in the scoring boxes being left blank. To summarise: 1 KKI box marked equates to a 5% question score 2 KKI boxes marked equates to a 10% question score 3 KKI boxes marked equates to a 15% question score 4 KKI boxes marked equates to a 20% question score The 5 th question on all forms has ten KKI boxes. Each box equates to 2%. Examiners should sum the number of KKI boxes and place a mark in the appropriate question score box, rounding up or down as necessary. For example: 7 KKI boxes marked (7 x 2 = 14) therefore mark the 15% score box. 6 KKI boxes = 10% etc. Overall assessment Individual question marks are added together to produce a total score which is marked in the appropriately numbered box at the bottom of the page (Blue circle pg5). A pass / fail decision is made based on the following criteria: (Red circle pg5) The candidate must attain a minimum of 15% in all of the question score boxes and a total score of at least 80% to pass. This means in addition to attaining 15% in four subject topic areas, they must also attain at least one 20% to achieve the overall pass mark of 80% 1

2 Completion of test report prior to the assessment. Ensure you have the correct CPC4 relevant to the category of test (LGV or PCV) This number is available when you book the test via the gateway, make a note before completing the booking. This is your unique organisation code This is your unique assessor number Actual category of vehicle presented for test The combination number of the questions you are asking. Category of test will be C or D Transfer the question numbers from the combination selected. 2

3 Write the candidates name and assessors name on the B copy only This is for data protection, all the information written on the CPC4A will transfer via the carbon. 3

4 Ask the candidate to read and sign the declaration keeping the signature within the box. If the candidate refuses to sign the declaration the test must not go ahead. 4

5 Sup to be recorded if the assessment is supervised by DVSA, Int is marked if the test required sign language (for deaf candidates only) C is a late change if you recognised the candidate as a friend or relative. The resultant score to each question will be in the form of a single mark placed in one of the scoring boxes on the right side, which are equal to percentages; 5%, 10%, 15% and 20% Remember Q05 is worth 2% for each answer and is rounded off to the nearest 5%. Individual question marks are added together to produce a total score which is marked in the appropriately numbered box at the bottom of the page (Blue circle). 6 Complete the reverse of the B copy to include the weather, the type of vehicle by ticking the boxes, a brief description of the candidate, ID presented for test (PC photo card licence, PP Passport and paper licence) A brief report on the test including the question and prompts given and finally sign the bottom. 6

7 If the candidate passes the assessment, record the pass certificate number in the appropriate boxes and ask the candidate to sign to acknowledge receipt of it. (Black circle) Record the result (Red circle) The correct activity code is then completed (Green circle) refer to your list of codes. The candidate must attain a minimum of 15% in all of the question score boxes and a total score of at least 80% to pass. CPC4B is completed as above, but there is no requirement to write a report on the questions which were not answered. 7

8 Complete the DVSA11 as shown below and transfer the serial number from the DVSA 11 onto the CPC4 remembering to include the check digit 8
