1 WELCOME TO THE UW LEND eportfolio As a UW LEND trainee you are required to maintain a UW LEND eportfolio. This tool is designed to help you track your training activities, reflect on your experiences and new knowledge, and provide a mechanism for sharing your accomplishments with your faculty mentors, future employers, and others. Your UW LEND eportfolio will be created using the UW Google Apps Service. UW Google Apps provides a UW gateway to Google s online services, allowing you to take advantage of Google s , productivity, and collaboration tools. Follow the instructions below to setup your eportfolio. STEP 1: COPY THE TEMPLATE 1. Go to UW CommonView 3. On the home page, click on "Copy to my UW Google Sites account"* 4. A window will pop-up asking you to create a unique URL. Keep the default URL that the system assigned for you ( trainee-portfolio_uwnetid ). ***It is very IMPORTANT that you keep the default. You will you this naming convention to log in in Step2.*** 5. Click on copy site. 6. Click view *NOTE: You will be redirected to a page to sign up for a UW Google Apps Account if you do not already have an account. Follow the instructions on the page to sign up for a UW Google Apps Account. Read through information and guidelines and grant access for your UW NetID to bridge with your new UW Google Apps Account. 2 STEP 2: PERSONALIZE YOUR eportfolio 1. Go to UW Google Sites (in place of UWnetID with your own UW NetID. 3. There are 7 sections to your eportfolio. Go to each section and follow instructions for tips to personalize your eportfolio with your own UW LEND training information. The following is an introduction to each of the 7 eportfolio sections: Tip: To assure success logging into your UW eportfolio it is important to log out and close out of all Google programs ( or other personal Google accounts). Home page This is your portfolio's homepage. On this page you will establish an introduction to your eportfolio. Attach an image of yourself and enter your trainee information. 3 Leadership Self Assessment 1. Take the leadership self assessment quiz at the beginning of your UW LEND training and at the end of your training Copy the three charts which depict your results into your eportfolio by replacing the example charts in the eportfolio template. Here are step-by-step instructions to copy and paste charts from the Self Assessment Tool results page: Right click on the chart you want to copy. Choose "Save Target As..." Save the image to your desktop. Insert the image into your eportfolio. Replace all charts, until you have 6 charts total (3 for the beginning and 3 for the end of our training). 3. Optional: If you have Adobe Acrobat Writer, you can also save a copy of your results in PDF format and attach it to this section of your eportfolio. Quarterly Learning Objectives 1. Learning Objectives: Use this section to record your individual learning objectives. At the beginning of each academic quarter, determine your individual learning objectives for that quarter and determine the activities you will pursue to meet these objectives. Type your learning objectives here. 2. Summary of Accomplishments: At the end of each quarter write a brief summative statement indicating how you met your objectives and any plans to carry forward or revise unmet learning objectives. Individual Leadership Training Plan (ILTP) 1. Download the ILTP here: 2. Fill out the form. 3. Save the changes and attach it in this section of your eportfolio. 4. Throughout your UW LEND training you will be updating your ILTP to reflect your training activities and accomplishments. Use the electronic ILTP file you saved to your computer to make these updates and revisions. 4 Clinical Training Activities In this section record the clinical sites/activities you participated in. For list of clinical training opportunities, see: 1. Download the Word document: 2. Indicate clinic site, start and end dates, and record reflections of your experiences. 3. Throughout your training, record the clinical training activities you have participated in here. Save the Word document to your computer and attach a current copy of the document to your eportfolio (or keep a working document on Google Docs and attach the shared link.) Leadership Training Activities In this section, record your leadership activities. For list of leadership training opportunities see: 1. Download the Word document: 2. Enter the leadership activity, date, and record reflections of your experience. Throughout your training, record the leadership activities you have participated in here. Save the Word document to your computer and attach a current copy of the document to your eportfolio (or keep a working document on Google Docs and attach the shared link.) Leadership Project In this section, record information about your Leadership Project. Attach a copy of your PowerPoint presentation as well as any additional supporting materials. 5 STEP 3: SHARE YOUR eportfolio In order for the UW LEND Training Director and your faculty mentors to be able to view your eportolio, you must submit your site for review. 1. Go to UW CommonView 3.
