1 Dodatok č. 17 ku Zmluve o poskytovaní telekomunikačnej služby Orange Biznis Mobilná Virtuálna Sieť uzatvorenej dňa Orange Slovensko, a.s. sídlo: Metodova 8, Bratislava IČO: IČ DPH: SK zastúpená: bankové spojenie: IBAN: zapísaná v Obchodnom registri Okresného súdu Bratislava I, oddiel Sa vložka číslo 1142/B (ďalej len spoločnosť Orange ) a Národná banka Slovenska sídlo: Imricha Karvaša 1, Bratislava IČO: IČ DPH: SK zastúpená: bankové spojenie: číslo účtu: IBAN: (ďalej len Užívateľ ) Spoločnosť Orange a Užívateľ (ďalej aj zmluvné strany ) sa za účelom úpravy zmluvných podmienok Zmluvy o poskytovaní telekomunikačnej služby Orange Biznis Mobilná Virtuálna Sieť, uzatvorenej dňa , v znení Dodatkov č.1 až č.16 (ďalej len Zmluva ) dohodli na nasledovnom znení Dodatku č. 17 k Zmluve (ďalej len Dodatok ): Čl. I Predmet Dodatku 1. Zmluvné strany sa dohodli, že nahrádzajú doterajšiu Prílohu č. 1 zmluvy Spôsob pripojenia novou Prílohou č. 1a. 2. Príloha č. 1a je prílohou tohto Dodatku, tvorí jeho neoddeliteľnú súčasť a stáva sa neoddeliteľnou súčasťou Zmluvy.

2 Čl. II Záverečné ustanovenia 1. Ostatné ustanovenia Zmluvy týmto Dodatkom nedotknuté zostávajú nezmenené a v platnosti. 2. Tento Dodatok sa vyhotovuje v šiestich rovnocenných rovnopisoch, z ktorých po ich podpise oprávnenými zástupcami obidvoch zmluvných strán štyri dostane Užívateľ a dva spoločnosť Orange. 3. Tento Dodatok patrí medzi povinne zverejňované zmluvy (vrátane dodatkov zmlúv) podľa ustanovení 5a zákona o slobodnom prístupe k informáciám (zákona č. 211/2000 Z. z. v znení neskorších predpisov) v spojení s ustanoveniami 1 ods. 2 Obchodného zákonníka (zákona č. 513/1991 Zb. v znení neskorších predpisov) a ustanoveniami 47a Občianskeho zákonníka (zákona č. 40/1964 Zb. v znení neskorších predpisov). Spoločnosť Orange súhlasí so zverejnením tohto Dodatku a faktúr spoločnosti Orange doručených Užívateľovi, pričom spoločnosť Orange tiež disponuje písomným súhlasom inej dotknutej osoby (osoby konajúcej za spoločnosť Orange) na zverejnenie jej údajov v tomto Dodatku a vo faktúrach spoločnosť Orange doručených Užívateľovi, a to zverejnenie Užívateľom počas trvania jeho povinnosti podľa 5a ods. 1, 6 a 9 a 5b zákona o slobodnom prístupe k informáciám; tento súhlas možno odvolať len po predchádzajúcom písomnom súhlase Užívateľa. 4. Tento Dodatok nadobúda platnosť a je pre zmluvné strany záväzný odo dňa jeho podpisu oprávnenými zástupcami oboch zmluvných strán; ak oprávnení zástupcovia oboch zmluvných strán nepodpíšu tento Dodatok v ten istý deň, tak rozhodujúci je deň neskoršieho podpisu. Tento Dodatok nadobúda účinnosť dňom nasledujúcim po dni jeho zverejnenia na webovom sídle (internetovej stránke) Užívateľa [ 47a ods. 1 Občianskeho zákonníka v spojení s 1 ods. 2 Obchodného zákonníka a s 5a ods. 1, 6 a 9 zákona o slobodnom prístupe k informáciám]. 5. Zmluvné strany vyhlasujú, že si text tohto Dodatku pozorne prečítali, jeho obsahu porozumeli a že tento vyjadruje ich slobodnú a vážnu vôľu, bez akýchkoľvek omylov, čo potvrdzujú vlastnoručnými podpismi osôb oprávnených konať v ich mene, alebo v zastúpení. SIP-Identity) = (na toto číslo budú účtované všetky hovory z PBX ) IP PBX typ: Mitel/Aastra MX One (predtým Ericcson MX One) ver. 5.3 Kapacita pripojenia IP PBX: 2 x 90 hlasových kanálov cez SIP Trunk v režime dual-homed / 2 lokality NBS Otvorené smery na strane Orange pre hovory z PBX: Všetky smery volané z PBX sú na strane OSK otvorené. Smerovanie jednotlivých hovorov do pripojenia OSK závisí od nastavenia na PBX (Orange, T-Mobile, medzinárodné hovory...) OSK bude exkluzívny operátor. Emergency calls budú smerované cez OSK. *Dialed # PBX - spôsob vytáčania závisí od nastavení, možností a užívateľského komfortu na PBX. Calling a called party posielané z PBX budú v takom tvare ako je uvedené v tabuľke. 3

5 IP PBX: Parametre požadované pre konfiguráciu IP PBX budú poskytnuté Orange SK - SIP Proxy address : - SIP port (5060) : - SIP media-rtp: - Username (P-Asserted-Identity in SIP) : - Password : - P-Asserted-Identity (PAI): - Transport protocol (UDP, TCP) : - From header domain IP : - Formát čísiel CLI a CLD pri odchodzích aj príchodzích volaniach je E.164 Implementácia IP PBX do siete Orange Implementácia PBX do siete Orange spočíva v zriadení konektivity pre PBX do siete Orange a následne v nastavení parametrov na PBX podľa akceptovaného číslovacieho plánu účastníka (CP) a overení týchto nastavení ako aj funkcionality PBX v rámci siete Orange prostredníctvom akceptačných testov podľa prílohy k CP. Zriadením konektivity pre PBX v zmysle vyššie popísanej implementácie je myslené nastavenie sieťovej komunikácie na oboch stranách a jej otestovanie. Fyzická konektivita pre pripojenie PBX musí byt pripravená vopred a čas potrebný na jej zriadenie nie je zahrnutý do času implementácie v tomto zmysle a je závislý od typu fyzického pripojenia. Čas potrebný na implementáciu PBX do siete Orange je obvykle 5 pracovných dní (PD), vrátane akceptačných testov, ktoré trvajú asi 0,5-1 PD. Po dohode môže byť rozsah akceptačných testov optimalizovaný pre potreby účastníka, pričom účastník môže zadefinovať funkcionality, ktoré neplánuje používať. Odporúčame, aby implementácia začala najneskôr 10 PD pred požadovaným spustením zmeny spôsobu pripojenia a aby si účastník vyhradil na implementáciu minimálne 5 PD. 5

6 V prípade portácie fixného číselného rozsahu PBX je okrem samotnej implementácie potrebná súčinnosť aj pri samotnej portácii fixného rozsahu v deň portácie. Vzhľadom na proces portácie odporúčame, aby implementácia začala najneskôr 15 PD pred požadovaným spustením zmeny spôsobu pripojenia- teda plánovaným dňom portácie a zároveň, aby si účastník vyhradil na implementáciu minimálne 5 PD. 6
