1 Instructions for Completion of IDP Forms To complete an Individual Development Plan (IDP), a PDF document and an Excel document will need to be completed. The PDF document: The PDF document includes: An Individual Profile Personal Details Further details: Who attends the meeting? Record of Agreement/Authorization to Share Information (signatures) Adobe Reader Version The programme that normally opens PDF documents on your computer is Adobe Reader. Recent XI or DC versions of this programme are required to enable the IDP form to work. Version DC enables electronic signing of documents. To upgrade to version DC visit the web page by following the link below: Attaching a Photograph You can attach a photograph of the pupil on the front page. Adobe Reader DC can upload photos in JPG, GIF, PNG, TIF, and PDF formats to the IDP form. Versions Adobe Reader XI or earlier will only permit uploading photos in PDF format. Follow the instructions below to upload photos in PDF format to the IDP from. Attaching a Photograph in PDF format The most convenient method to create a photograph in PDF format is while scanning the original photograph. Often, a scanner provides options to directly create a PDF document. Follow the scanner instructions to do this. If a photograph already exists in JPG, PNG etc. it will need to be converted into PDF format prior to attaching to the IDP. To re-format a photograph to PDF format, it will have to be ensured that a programme that enables printing to a PDF document has been installed on the computer. If you do not have a PDF printing programme on your computer, you can install the PrimoPDF virtual printer by visiting the following web page: Instructions for Completion of IDP s 1 20/04/2017

2 and selecting the Download Free button below the Try PrimoPDF heading and follow the instructions. To re-format a photograph in JPG, PNG etc. select the photograph in Windows Explorer and click the right hand button on the mouse and select Print from the menu. A Print Pictures window will appear. Select PrimoPDF (or other PDF printer if installed) from the Printer option and then press the Print button at the bottom of the same window. This will open the appropriate window of the selected PDF printer. Click the button to create the PDF and follow the instructions to select the name and location for the PDF document. 3 Signing the Record of Agreement Page If version DC of Adobe Reader is not installed on the computer the Record of Agreement will need to be printed, then signed and re-scanned to PDF to with the rest of the IDP documents. With version DC installed it will be possible to electronically sign the IDP. Adobe Reader DC will look like the following: Select Tools Select Fill & Sign by clicking on the relevant icon. Select the Sign icon: Instructions for Completion of IDP s 3 20/04/2017

4 Select Add Signature Select "Type" to type a signature and choose a style by selecting "Change style" or Select "Draw" in order to hand draw a signature using the mouse. Select "Apply" to finish. Paste the signature to the document with the mouse and change the size if necessary. Lack of Space in the Text Boxes If more space is required than the text boxes allow, complete the information separately in a Word document and send with the other documents. There is a need to ensure that there is clear guidance on which section of the IDP the additional information belongs to (see chapter Naming IDP Documents below) The Excel Document: The Excel document contains several tabs: Angh-Supp Tab: Personal Details Summary of the requirement fields and level of support provided Linked Plans Instructions for Completion of IDP s 4 20/04/2017

5 Cyrh-Attain Tab (Achievement): Learning Fields: Foundation Phase (FP)/FP Outcomes Subject / Field of Achievement: KS 1-4 Teacher s Standardized Assessments Subject / Field of Achievement: National Tests. Subject / Field of Achievement: Curriculum Subject / Field of Achievement: Other Standardized Tests Deill-Out x 3 Tabs: Outcomes Action Plan Cynllun-Plan Tab: Provision Plan Only fill in the blue cells in each tab. The blue cells are a mixture of free text cells, defined option cells and others that will require entry in a specific format. Support boxes with a yellow background will appear. These boxes can be removed using the Esc key on the keyboard, or moving them by pressing on the box with the left hand button on the mouse and dragging it to another area while holding the left hand button down. Naming IDP Documents: Order is required on how to name IDP documents so as to understand the contents of each document. One IDP will include xlsx, pdf and possibly docx documents. The following system is suggested for naming of IDP documents A document s name should include: 1) Date of the document 2) Information about its contents 3) Pupil s surname 4) Pupil s name/s 5) Pupil s date of birth; Format: yymmdd document information surname name/s ddmmyy There will be no problem when using - or _ in place of an empty space between the items. 6 Example of Excel: IDP Davies Siôn Dewi Note that names of Excel and PDF documents are the same. As both documents are of a different kind, their suffixes (PDF, DOCX, XLSX etc.) will differ. Because of that, the system will identify them as different names.
