1 Creating Club Registration Forms This user guide explains how to create a club registration form. Water Polo Canada will provide annual templates for club registration. This guide explains how to customize the club registration form for your province. 1. Go to wpc.goalline.ca and select your PSO site from the dropdown menu. Then login using the Admin Login. 2. Go to Master Menu, then the People tab, and then click Individual Registration Forms. 3. Find the club registration form for the current year. Then click Edit. Translations Please note that translations to the written fields can be added throughout the form. Translations can be added in two ways: Creating Club Registration Forms / 1
2 1. Clicking on Provide Translation. 2. Clicking on the fleur-de-lys button. Note: If the translation buttons are not displayed right away, scroll to the bottom of the page and save the step. Once the step is saved, the translation buttons will appear. Step 1: Personal Information 1. Click on Step 1: Personal Information. Extra Information 1. Click "Click here to insert additional instructions at the top of Step 1" if you have any information to add to the top of the form. General Form Information 1. Keep Club Registration selected as the type of registration form. 2. Ensure that it is indicated that this is a membership form. 3. Keep No Memberships Required selected. 4. Ensure that the Registration Closing Date is set for the end of the program year. Creating Club Registration Forms / 2
3 5. Select whether you will Display this form in the main menu. If yes is chosen on this question, this form will automatically show up in your menu, grouped with your other registration forms. If you choose no, we suggest you either create a webpage containing registration information and a link to your form, or send an with the link to your form to those expecting to register. 6. Keep Yes selected to enable, require or restrict returning registrants from the Goalline database. This will allow people who are already registered in Goalline and people who are new to Goalline to register a club. 7. Select whether you would like collapsing sections in Step 1. Collapsing sections are designed to keep the person focused on the information that they are filling out. However, when sections collapse, people are at risk of forgetting to fill something out. Event Information 1. Check the Event Information checkbox and select Yes in for Is this a featured event? if you would like for the registration form to be displayed on the main page of your Goalline site (see Publishing the Form for an example). Leave the Event Information unchecked if you would not like for the registration form to be displayed on the main page of your Goalline site (see Publishing the Form for an example). 4 Priority Registration 1. Do not select Priority Registration. This is not applicable to club registration forms. Club Information Collected 1. Select the Club Membership Requirement membership categories for the current year. Note: This is an important step to ensure that Water Polo Canada s club registration fees can be added to the PSO s fees. 2. Select Required or Yes for any other information you would like to collect. Any information set as Required must be filled in before proceeding with the registration. Extra Questions to ask About the Club 1. Enter the question to ask in the Question Title field. 2. Select from the dropdown list for the Question Style. Text Box: Small text box, free field for a text or number input Text Area: Large text area for several lines of free text or number input Radio Buttons: Provide multiple options. Only one option may be selected on the form by clicking on the radio button Dropdown Selection: Provide multiple options. Only one option may be selected on the form by clicking on the dropdown menu and selecting Checkboxes: Provide multiple options. More than one option can be selected by clicking on the check boxes Creating Club Registration Forms / 4
5 3. Questions will appear in the order they are entered. Enter numbers in the order field to set the order questions should display. 4. Extra questions can be set as required by selecting Yes to the Required field. 5. Office Only questions are not available on the public side of the form. The Office Only fields appear in the Review Registered Clubs tool (found under the Communication tab). If you would like to track something on your clubs that shouldn't be asked on the registration form - like a cheque number, for instance - set Office Only to Yes. Club Contact Information Collected 1. Click Click here to insert/edit instructions above Club Contact Information if you would like to enter additional instructions. Note: Water Polo Canada s instruction, Please fill in information for the club registrar will appear automatically. 2. Select Required or Yes for any information you would like to collect. Any information set as Required must be filled in before proceeding with the registration. Extra Questions to ask the Club Contact 1. Refer to Extra Questions to ask About the Club for instructions on filling out this section. Creating Club Registration Forms / 5
6 Note: The Office Only fields appear in the club registrar s profile to be updated by an administrator with permission to edit club registrars. Parent Information 1. Select No for all the information collected. Collecting parent information is not required for a club registration form. 2. Do not add any extra questions for the parent fields. Extra Information 1. Click "Click here to insert additional instructions at the bottom of Step 1" if you have any information to add to the bottom of the form. Attachments Creating Club Registration Forms / 6
7 1. Select whether you would like the club registrar to upload document(s) (e.g. an insurance form). 2. Enter the document title. The document title should reflect the type of document you want the registrant to upload (e.g Insurance Form). This will be displayed when "Have registrant upload document(s)" set to Yes" or Required." 3. Select the number of documents to upload. 4. Upload attachments that you would like the club registrar to access when filling out the form (e.g. the blank insurance form). Save Personal Information 1. Click Save Personal Information. Step 2: Programs and Products 1. Click on Step 2: Programs and Products. Programs/Payment Types 1. Instructions can be added above the program s information by clicking the link to Click here to insert/edit instructions above programs information. 2. Enter the name of the program or payment type (i.e. club type) that will be available as a registration option (e.g. Club, Affiliate Club, Developing Regions Club, etc.). 3. Click "Add Payment Type." 8 4. Click Edit to add payment details. 5. Do not select any gender or age restrictions. This is not applicable to a club registration form. 6. Do not select a Maximum number of. This is not applicable to a club registration form. 7. Choose the Membership Required by selecting the corresponding Water Polo Canada club registration fee. 8. Select Forced Purchase as the Membership Style. 9. Set the Membership Expiry for the end of the program year. 10. Enter the Payment Details for the program. The details will appear on the public form. 11. Enter the amount (price) of the registration option and taxes (if applicable). 12. Enter the payment due date (optional). 13. Select that the payment is active. 14. Choose whether the form is tax eligible. 15. If you would like to set up recurring payments or installments, please refer to When you are finished, click Save Changes. Creating Club Registration Forms / 8
9 17. Repeat for all your province s club types. Products and Discounts Do not add products and discounts. They are not applicable to club registration forms. Final Steps 1. Click Save Programs and Products. Step 3: Policies The following documents are required for your registration form: Waiver, Privacy Policy, Refund Policy, and Contact Information. Water Polo Canada s waiver, privacy policy, and consent message will appear automatically on the front end of your site, once the form is published. If necessary, you may add additional information to the different fields. 1. Click on Step 3: Policies. 2. Instructions can be added above the policies by clicking the link to Click here to insert additional instructions at the top of Step 3. Creating Club Registration Forms / 9
10 3. WPC s waiver will appear automatically on the front end of your site. You may also enter an additional waiver (or a link to an existing waiver). If you would not like to add additional information, enter n/a in this field. 4. WPC s privacy policy will appear automatically on the front end of your site. You may also enter an additional privacy policy (or link to an existing policy). If you would not like to add additional information, enter n/a in this field. 5. Enter a refund policy. 11 6. Enter the organization's contact information. 7. Enter a message about Canada s Anti-Spam Legislation (CASL) (optional). Refer to for more information. 8. Select a consent option. Water Polo Canada s consent message will appear automatically on the front end of your site. a. If you would like the method of consent required in the form of a check box, select Checkbox from the dropdown list. b. If you would like the method of consent required in the form of text input, select Name Input from the dropdown list. c. You may enter an additional message to display next to the consent form. 9. 12 Step 4: Payment Information 1. Click on Step 4: Payment Information. 2. Instructions can be added above the payment information by clicking the link to Click here to insert additional Payment Information instructions at the top of Step Select Yes or No to allow offline (cash/cheque) and online (PayPal) payments. If you select online payments, you can also choose to allow payments to be made through a Goalline registration account. If collecting online payments, select the payment gateway from the dropdown list. 4. Instructions can be added for the offline payment instructions by clicking the link to Click here to insert offline payment instructions. 5. Click Save Payment Information. Step 5: Confirmation Message Creating Club Registration Forms / 12
13 1. Click on Step 5: Confirmation Message. 2. Instructions can be added on the final page by clicking the link to Click here to insert additional information on the final page of the registration process. 3. The registrars name and are required. This will serve as the reply to address when the club registrar receives their confirmation. 4. Any addresses that should be sent a copy of each registration should be entered in the Copy Confirmation To (bcc) line. Multiple addresses need to be separated by a comma. 5. Enter the subject as desired. 6. Enter the body as desired. You may choose whether to select the check box to "Show on confirmation page?" You may use the various text placeholders available to include personalized information in the confirmation. Creating Club Registration Forms / 13
14 6. Click Save Confirmation Information. Previewing the Form Before publishing your form, it is recommended that you preview your form. 1. Click on Preview Form on the top right hand side of the page. 2. Review all the form steps in test mode by entering in the required fields. Please note that the information that you enter will not be saved. Publishing the Form Once your form is complete and has been reviewed, it needs to be published in order to be open for registrations. 1. Click on Publish Form on the top right hand side of the page. 15 2. If you chose for the registration form to be a featured event in Step 1, the form will be displayed on the homepage of your Goalline site. If you did not choose for the registration form to be a featured event, the form will be available on the far left-hand side of the page. Creating Club Registration Forms / 15
