1 JourneyWell Health Assessment and Well being Activities User Guide For State of Minnesota employees Welcome to JourneyWell where your health is our priority. Use this step by step guide to walk you through logging on, taking your personal health assessment and participating in well being activities.

2 1 Go to 2 If you have taken the health assessment before, enter your username and password and click Go. Proceed to step 4, page 4. If this is your first time taking the health assessment, click Register for an account to set up your JourneyWell web account. 2

3 3a Enter your Employee ID as provided to you by the State of Minnesota, followed by the last 4 digits of your Social Security number (no spaces). Complete the other required fields and click Continue. 3b Create a username and password, then enter your address. If you don t have an address, you may receive one from any internet service provider free of charge such as Google, Yahoo, or Hotmail. If you choose not to sign up for a free address, enter a non valid address such as: yourname@no .com. Select and answer 3 security questions Click on the link to read the terms of the site and check the box to agree, then click Continue. 3

4 4 Click Continue. 5 Drag and drop to rank your interests, then click Get Started Now. 6 This is the first page of the health assessment. Move through the assessment, answering every question. Your responses automatically save every time you click Next. 4

5 7 In the Healthy Changes section of your health assessment, you are asked if you would like to receive a follow up call from a JourneyWell health coach. You must select Yes to receive the lower copay. A JourneyWell health coach will only contact you if your results show some risk, but you will receive the lower copay just for agreeing to a follow up call. 8 When you complete your health assessment, you will see your current health score, as well as potential and difference. Click 2012 Results to see your HealthPotential personalized report. If you made it this far and have agreed to a follow up call by November 14, 2012, you qualify for the lower copay in

6 9 Start a well being activity! You have access to phone coaching, online programs and virtual coaching. These activities are available to you any time after you complete your personal health assessment. Hover on the to the right of the activity name to learn more about that activity. Click on the blue arrow next to the activity name to expand that activity and enroll. Some activities will show in your list of current activities based on your interests. See step 10 to learn how to add activities Some activities will show in your list of current activities based on your interests. There are 2 ways to change these activities. Click on Manage Activities to open a pop up and select which activities you would like to see on your page by checking or unchecking the box next to them and clicking Done. Use the Recommended activity carousel. Just click Add to activities. You can click on the blue arrow to see additional activities.

7 Once enrolled, log on to JourneyWell regularly to access your well being activities. Go to enter your username and password, and click Go. 7
