1 Be the Difference. CDI Provider Inquiry Notification and Response UHCare Acute Secure Health Messaging and Documentation Clarification for Providers Clinical Documentation Improvement (CDI) Provider Inquiries are generated in UHCare Acute and sent to providers via a Secure Health Message from the UHCare Acute Inbox. This job aid provides instruction on receiving and responding to CDI inquiries using Secure Health Messaging and the Documentation Clarification Structured Note in UHCare Acute. Secure Health Message Notification Message notifications are sent to and can be retrieved from the provider s UH Outlook account and from the UHCare Acute Inbox. The CDI Specialist uses Secure Health Messaging to advise providers of missing or incomplete patient documentation that prevents the final assignment of accurate diagnosis codes to the patient chart. Notification When a secure health message (SHM) is sent to a providers from UHCare Acute, an notification is sent to the provider s UH Outlook address. Job Aid 1 January 9, 2014
2 UHCare Acute Inbox Notification If a provider has an unread SHM s in his/her UHCare Acute Inbox, an Alert Detail Screen displays with a notification message after log on to UHCare is completed. The SHM alert notification will continue to display when logging into UHCare Acute until the Secure Health Message(s) associated with the alert is opened and read in the UHCare Acute Inbox. 1. From the Alert detail window, click Proceed to acknowledge the SHM notification. 2. Toggle from the Patient List to the Inbox. 3. Open and read messages in the Inbox. 4. Take requested action as quickly as duties permit. Note The Signature Manager window displays first if there are items pending signature. Items can be addressed in Signature Manager first or the Signature Manager window can be closed as desired. Secure Health Message Management Secure Health Messages are viewed and managed from the UHCare Acute Inbox. 1. From the UHCare Acute Patient List tab, toggle menu, select Inbox. 2 Job Aid IT&S Training and Development, MSC
3 The Inbox view displays Note Unread messages display in bolded font. 2. From the message grid, double-click to view the desired message. The Secure Health Messaging window displays. Documentation Clarification Note Entry The Documentation Clarification Structured Note is used by providers to provide the requested missing or incomplete patient documentation in response to the CDI Providers Inquiry Secure Health Message. Note Clinical Evidence supporting the diagnosis clarification should be included in the entry. See the Informational section at the beginning of the Documentation Clarification note for examples. 1. From the UHCare Acute Main Application toolbar, select Enter Documentation. 2. From the Documentation Entry Worksheet, manual entry field, type documentation. 3. Select Documentation Clarification Note. 4. Click Open. IT&S Training and Development, MSC Job Aid 3
4 The Documentation Clarification Note displays 5. In the Provider Clarification field, enter the requested clarifying information. 6. Click Save. 7. Click out of the Provider Clarification field to activate the Save, Save/Forward and Cancel action buttons. Secure Health Message Reply to CDI Specialist Use the Reply feature in Secure Health Messaging to send a message back to the CDI specialist advising the action you have taken after your review of the inquiry. 1. From the UHCare Acute Patient List tab, toggle menu, select Inbox. The Inbox view displays Note Unread messages display in bolded font. 2. 5 The Secure Health Messaging window displays. 3. Click Reply. 4. Compose your message to the CDI Specialist and click Save.
