1 MyMedicalLocker.com is a state of the art personal health record. It allows users to track their most relevant medical information, share health information and communicate with providers across the nation securely through the web site. MyMedicalLocker accounts are free. Please use this guide to assist with setting up a MyMedicalLocker account and networking with providers. 2 Step 1: Create a MyMedicalLocker Account 1. Open an Internet Explorer browser and type in 2. Click on the Create New Account link 3. Fill out each of the Registration Fields 4. Press the Register button at the bottom of the registration page 5. MyMedicalLocker will send an to the address provided in the registration process. 3 Check your Spam or Junk folder if you can t find the in your Inbox. 6. Create a password for your MyMedicalLocker Account 7. Press the save button your MyMedicalLocker is now active. 5 Step 3: Connect to Physician Practices Connecting a MyMedicalLocker personal health record with a physician office allows MyMedicalLocker users to pull their chart, share pertinent health data, securely communicate with providers of care and view account balances with the connected physician offices. To connect follow these steps: 1. Press the Connect menu option 2. Select Connect to a Practice from the pop-up menu Page 5 of 19 MyMedicalLocker.com User Guide

6 3. Select a Physician Office to connect to and then press the Connect button 4. Enter your Demographics and Connection Key Contact your physician office for your Connection Key 5. 7 Share My Data MyMedicalLocker allows users to share their data with connected physician offices. Follow these simple steps to share health data with a physician office: 1. Click on the About Menu 2. Click on the Summary menu option Page 7 of 19 MyMedicalLocker.com User Guide

9 Send a Message to a Provider MyMedicalLocker accounts connected to a physician office can securely communicate with physicians. Follow these steps to view and send messages: 1. From the Home Page click on Messages 2. Unread messages from physician offices will display click on a message to read it 3. 11 Pull My Chart MyMedicalLocker accounts connected to a physician practice can securely pull electronic heath records to view the most up to date medications, problems, allergies, and lab results from physician practices. Follow these steps to view, download or send your electronic health record: 1. On the Home Page - Click on the Chart Icon 2. Click on the Pull Clinical Summary Button Page 11 of 19 MyMedicalLocker.com User Guide

12 3. A Clinical Summary will display MyMedicalLocker.com User Guide 4. Click on the Print button to Print the Clinical Summary; Click on the Export button to save the Clinical Summary on your computer or external device; Click on the Send button to send the Clinical Summary to a provider s DIRECT address. 13 View/Request an Appointment MyMedicalLocker accounts connected to a physician office can view future and past appointments as well as request appointments. Follow these steps to view or request an appointment: 1. Click on the Appointments button on the Home Page 2. Future appointments will display for all connected family members. 14 3. Fill out the appointment request. Press the send button. 4. A member from the physician s office will send a response with an appointment time or call you to schedule the appointment. Table of Contents Page 14 of 19 MyMedicalLocker.com User Guide

15 View Account Balances MyMedicalLocker accounts connected to a physician office can view account balances for connected family members. Follow these steps to view account balances: 1. Click on the Account Balance icon on the Home Page. 2. Account balances will display for each practice and family member. Click on the family member s name to view detailed claim information. 3. 17 Manage My Family MyMedicalLocker also allows users to track and connect family members health records. Follow these steps to setup and view family member s personal health records: 1. Click on Add a Family Member 2. Enter the family member s information. Click on the Add button to create the family member. If you wish for the family member to login and view his/her personal health data select the Create a login for this family member checkbox. 3. The family member s account is now active. 4. 18 5. Select the appropriate family member to move to his/her personal health record view. This will allow you to fill out the family member s personal health information, connect to physician practices, pull charts, request appointments and view account balances for the selected family member. 6. MyMedicalLocker is now personalized for the selected family member. 7. Connect the family member to his/her physician offices to send secure messages, pull charts, request appointments and view account balances for him/her. Click on the appropriate sub menu option to manage your physician directory, change your security question/answer, change your password, view your account balance, view connected practice announcements or view the pulled chart security logs.
