5 About this Document Purpose This document outlines new and updated features for the version 9 release of Harris CareTracker. Before You Begin Clear the Cache You must clear your computer cache after every release. 6 1 Patient Portal Forms Security Override A new override has been added to control whether an operator can post forms that are visible to all Patient Portal users. This will help prevent users from erroneously posting patient specific forms publicly. When an operator has the Display Forms in Patient Portal override, a field will appear in the Document Management upload window to Display to ALL in Portal. When this field is set to Yes, the uploaded forms will be available to all Patient Portal users in that group. Note: this security override is currently OFF for all operators. If you have an operator who would like access to this security, please log a ToDo to Customer Service. 2 PM Header Displays Patient Balance and Next Appointment The CareTracker Name Bar has been updated to provide more patient information at a glance. When a patient is pulled into context, the Name Bar now displays the following: The date of the patient s next appointment and the appointment type. The patient balance (not the responsible party balance) is specific to the company and not the group in context - regardless of how statements are processed. The patient balance is the total private pay balance, minus any posted unapplied credits. 3 Transactions > Charge: Carry date value in service date fields On the Transactions > Charge page, the operator previously had to enter a service date in the Date & Date To fields. Now the date will automatically carry over from the service date. Note: This functionality only applies when the Date & Date To fields are blank. If a service date was selected when the batch was created, that service date will populate the Date/Date To fields. 4 New taxonomy added to Location maintenance The option "282N00000X - General Acute Care Hospital" has been added to the Facility/Visit Type list in the Location maintenance application in the Administration module. 6 of 33 CareTracker v9 Release Notes v4

7 5 Claim changes for Purchased Diagnostic Tests When purchased diagnostic tests are transmitted with a procedure both the purchased provider and service amount must be included. New fields have been added to the Misc window (Charge or Claim Edit) to select the provider who performed the service and the amount charged. The Purchased Service Provider field is populated with the values from the Referring Provider quick pick list. Both fields must be populated. If either the Purchased Svc Amount or Purchased Svc Provider field is null, then the corresponding segments are not created in the claim file. 7 of 33 CareTracker v9 Release Notes v4

8 6 Edit case on booked appointments The Scheduling workflow has been updated with the ability to change the patient case after an appointment is booked. The Case field in the View Appointment window has been changed to a drop-down list. Note: When you save a visit, it updates the encounter with the case from the appointment. However if a progress note has already been created with data driven by the case, it will not be updated. Modifications to the patient case are logged in the Appointment History at the bottom of the View Appointment window. 7 CareTracker Domain Indicator A new domain label has been added to the Practice Management Name Bar to indicate, at a glance, the CareTracker environment in use. This will be useful for operators to identify which environment they are working in when calling customer service or when trying new functionality in the Beta, Rapid Release or Live environments. Note: The LIVE indicator will include a month. This is not meant to illustrate the current month but can be used as a visual cue to indicate which month s LIVE software deployment your operator is logged into. 9 8 ERA Enhancements 8.1 Allow 3 rd transfer through ERA posting The electronic remittances application has been updated to allow balance transfers to a third payer. Previously, the system did not transfer the balance to a patient s tertiary insurance or to private pay. Note: The transfer to the third payer is still subject to the conditional transfer logic and no transfer will be made if the transaction fails this check. 8.2 No Transfer status added to ERA detail grid A "No Transfer" status has been added to the ERA check details grid to identify service lines that were not transferred. When a remittance is created the system uses specific conditional logic to determine if any transfers should be performed. If this logic fails then the transfer is not done and the remittance is created. A new highlight color has been added to easily identify transfers were not posted as expected. Additionally, a check box has been added to the service line filter to allow user to only display lines that are flagged with the status of No Transfer. This will facilitate the review of those service lines that may require intervention by end user. 9 of 33 CareTracker v9 Release Notes v4

10 8.3 Added ability to configure remit rules base on how the payer processed the claim Practices can now configure remittance rules based on how the payer processed the service (Primary, Secondary, Denied, etc) using the new Status parameter on Remit Rules. The 835 file contains a value indicating how the payer processed the claim. When processing a remittance, CareTracker checks the value that was received against the Status configured on the remit rule to determine if the rule should be used. This functionality will allow greater granularity in the configuration of remit rules and how specific transaction types are processed. Note: The status of All will execute the rule regardless of the value that was received in the 835. The status options in the Remit Rule maintenance are: All Processed as Primary Processed as Secondary Processed as Tertiary Denied Processed as Primary, Forwarded to Additional Payers Processed as Secondary, Forwarded to Additional Payers Processed as Tertiary, Forwarded to Additional Payers Reversal of Previous Payment 10 of 33 CareTracker v9 Release Notes v4

11 Not Our Claim, Forwarded to Additional Payers Predetermination, Pricing Only - No Payment 8.4 Create Check application The Create Check application allows you to quickly post electronic remittances that contain matched service lines. You also have the option to exclude lines that meet certain conditions. Excluded lines will be unmatched by the system so they can reviewed in the Electronic Remittances application. Only complete matches will be processed electronically, partial and un-matched lines must be manually matched via the Electronic Remittances application before they can be processed. Note: The Create Check application being released is a functioning foundational prototype that will be incorporated into future iterations of the product. Additional features and functionality will be available as the application matures. 11 of 33 CareTracker v9 Release Notes v4

12 9 Visit Page Improvements 9.1 Ability to edit diagnosis from visit/charge Clients now have the ability to deactivate a diagnosis from the Transactions > Charge tab and Visit > Diagnosis tab. This feature allows operators who are only using CareTracker Practice Management to remove diagnosis codes from a claim. If a user selects the check box for a diagnosis on the Visit > Diagnosis tab or Transactions > Charge tab and then deactivates the same diagnosis in the Change Status window, the check box will be deselected on the Visit > Diagnosis tab or Transactions > Charge tab and the diagnosis will be deactivated. Changes made to a diagnosis status from the Charge or Visit applications will be recorded in the EMR. However, it is recommended that practices using CareTracker EMR make all diagnosis status changes from within the medical record. Operators must have the Change Diagnosis Status override included in their operator profile. 9.2 Visual Grouping of Diagnosis Codes The corresponding ICD-9 and ICD-10 codes for a single diagnoses will be grouped together with a single check box on the visit page for diagnoses added from the EMR if the group is enabled for dual coding. The ICD-9 code will appear first and the ICD-10 code appears immediately below the ICD-9 code. When selected, the appropriate code version based on the insurance flag setting with be transmitted. 12 of 33 CareTracker v9 Release Notes v4

13 9.3 Today s Selected Diagnosis moved to top of Visit Page Diagnoses added via the Today's Selected Diagnosis section of the progress note that are not added to the problem list will automatically appear above the Patient Active Diagnosis section on the Visit > Diagnosis tab. 14 10 Patient Module Updates 10.1 Enhanced Demographics Save button moved to header The Save button has been moved to the top of the enhanced Demographics page to streamline the workflow. Operators may also use Ctrl + S to save changes to the demographic record Advance Directive in Demographics Advance directives can now be added to a patient s record via the enhanced Demographics. When the Add icon is clicked, the document upload window opens, with the following defaults pre-selected: -Document Type = Clinical -Sub-Type = Global > Advanced Directive -Patient = Patient in Context After adding the advance directive, the user is returned to the Demographics screen. The "Yes" radio button is automatically selected when an advance directive is linked to a patient from either the enhanced Demographics or from Document Management. The No radio button is selected by default, indicating no active advance directive has been linked to the patient. Operators can select one of the following reasons from the drop-down list: Not applicable (N/A) or Patient Refusal. Note: Users cannot select the Yes button manually. Also, when entering a new patient into CareTracker, the demographic record must be saved prior to adding an advance directive Verify no checkbox A new checkbox has been added to the Patient Details tab to indicate that a patient has no address or has refused to provide one. For practices seeking NCQA PCMH recognition, selecting this box prevents the patient field from appearing blank in PCMH reports. The checkbox is deselected by default. Selecting the checkbox will remove all values for all phone types and disable the fields. If any address was marked as the "Preferred Contact," the preferred designation is removed. You cannot mark an address as preferred if Verify no is selected. 14 of 33 CareTracker v9 Release Notes v4

15 10.4 Patient Portal registration letter update The following information has been added to the Patient Portal registration letter in enhanced demographics: The logo has been changed to Harris CareTracker. The group provider name, address and phone number is printed in the upper right corner. The patient s account number has been added below the registration ID. The patient needs this information when they register for the Patient Portal. The patient print out is accessed from: Patient module > Demographics tab> Practice Details tab> Patient Printout link Note: Changes are available in enhanced demographics CareTracker auto-populates billing address with home address when no billing address is entered in Enhanced Demographics The Home Address for Billing field determines whether the patient's home address is also used as their billing address. For new patients, the Home Address for Billing check box is selected by default, which means whatever is entered as the home address automatically becomes the billing address as well. For existing patients, the Home Address for Billing check box is deselected by default. However, if the patient has no billing address entered, CareTracker will automatically use the home address as the billing address. Note: If an operator deselects the Home Address for Billing check box and does not enter a billing address, CareTracker will use the home address as the billing address. Important If you enter a separate billing address for a patient that is different from the home address, you must deselect the Home Address for Billing check box. Otherwise CareTracker will continue to use the home address for billing Demographic Data Cleanup Utility A new patient demographics data cleanup utility has been added to the Administration module. The Patient Contact Info Update application allows you make the following changes to multiple patients at a time: 15 of 33 CareTracker v9 Release Notes v4

16 -Set a patient s phone number to a specific phone type: home, mobile, work, business -Set a patient s phone number as the preferred contact -Set the patient s notification preference: , letter, non-declared, patient portal, patient refusal, phone, text Important The Patient Contract Info Update is to be used when upgrading to CareTracker s automated patient notifications. Using this utility operators will have the ability to bulk update phone types and notification preferences. CareTracker offers a global report located in Report Manager > Global Folder > Patient Folder > Patient Notification (Patient Contact Information Update Report) that may be run and used during implementation of CareTracker s automated patient notifications to identify which numbers are actually mobile numbers versus landline numbers so the operator can set accordingly using the utility. The utility can be used to update patients across all groups in a single company. 16 of 33 CareTracker v9 Release Notes v4

17 11 Physician Workflow Improvements 11.1 Document Management Action Buttons Two new action buttons have been added to the Document Management viewer to streamline the document workflow: Sign and Sign and ToDo. These buttons work in conjunction with the two new option buttons: Advance to Next and Close. When the Sign or Sign and ToDo button is clicked, the action will take place and the user will either be automatically presented with the next document in the work list or land back on the document work list grid based on the user defined selection of either Advance to Next or Close Adjust height of the Doc Management grid The size of the Document Management work list has been increased to optimize the usable work area. 17 of 33 CareTracker v9 Release Notes v4

18 11.3 ToDo macro and template defaults (per operator) Operators can now set a default ToDo Macro and Template, eliminating the need to manually populate the fields each time a ToDo is created. For example, you can create a standard ToDo message used for test results. Each time a ToDo is created, the operators default macro and template will be in context. The operator default macro and template can be configured by opening the message center>set Defaults Also, when a new ToDo is launched, the cursor now automatically appears in the text field so the operator can begin typing a message immediately eliminating the need to click in the text field 11.4 Remove No Macro label With the addition of the default macro and template functionality, the (No Macro) text has been removed from the New ToDo link in Message Center drop-down list. If an operator has a default macro and template set up, the ToDo will launch with the default template in context. The (No Macro) text has also been removed from the New Fax and New Mail links for consistency, but the functionality for those links remains the same. 18 of 33 CareTracker v9 Release Notes v4

19 11.5 Drag & drop reordering of Today s Diagnosis Diagnoses in the Today s Selected Diagnosis section of the Progress Note now appear in the order in which they were selected instead of alphabetically. Operators can now also reorder the list using the new drag and drop feature. To reorder, click and hold the drag and drop icon, which appears to the left of the diagnosis description, and move the diagnosis to a new location in the list. 20 12 Rx Updates 12.1 Nevada custom RX form template The following custom Rx form was created to comply with the state of Nevada Board of Pharmacy prescription requirements: Default_Rx_State_NV. The printed Rx will display Brand Necessary within the Rx instructions when the prescriber indicates that substitutions are not allowed. Note: The state address of the encounter location must equal Nevada for the changes to be seen Prevent orders for drugs with no active NDC CareTracker has been updated to prevent operators from writing an order for a drug that does not have an active NDC by preventing drugs that do not have an active NDC from being selected in Provider Favorites, Order Sets and Reordering Meds Provider Favorites (Access Point: Rx Writer & Order Set Maintenance) Provider favorite medications that do not have an active NDC are visible (gray text) but cannot be selected. When the cursor is placed over the disabled medication, the message "No active NDC" appears. Order Sets & Medications Health History Pane > Reorder Meds The checkbox is disabled next to any medications that do not have an active NDC. Medications Health History Pane > Actions list The send, fax, send electronically, and print options are disabled in the Actions menu for medications that do not have an active NDC. When the cursor is placed over the disabled medication, the message "No active NDC" appears Custom prescription forms to allow substitutions A new prescription form has been added to comply with the WA Board of Pharmacy requirements. The DEFAULT_RX_STATE_WA form contains two signature lines: one to indicate dispense as written and the other to indicate that substitutions are allowed. 21 12.4 Drug search filters out drugs with no NDC code The drug search in the Rx writer, Order Set maintenance and Provider Favorites maintenance now filters out any drugs that do not have an active NDC to a avoid a failed transmission. Note: This change has not been applied to the Add Patient Medication access point in case a provider wants to add a drug that no longer has a valid NDC to a patient's record Default RX Supervisor to prescription template The Default_Rx_wSupervisor2 prescription form includes the following enhancements: The following text appears below the signature line when an electronic signature is used: Electronically Signed by [Insert Filling Provider's First Name Last Name, Credentials on MM/DD/YYYY, HH:MM AM/PM Time Zone The fill quantity is displayed in numerical and text format Electronic Prescribing of Controlled Substances CareTracker is introducing a safe and secure method to electronically prescribe controlled substances from CareTracker. Electronic Prescribing for Controlled Substances (EPCS) eliminates paper prescriptions entirely by allowing clinical prescribers to electronically write prescriptions for schedule II-V controlled substances. CareTracker has integrated third-party certified EPCS services to comply with the strict identity proofing and 2-factor authentication process required by the DEA. Note: Please reference the EPCS Onboarding and User Guide for detailed instruction on the EPCS functionality and enrollment. 21 of 33 CareTracker v9 Release Notes v4

22 13 Reports Module 13.1 Report Manager Addition Insurance plan phone number was added to the patient finance package in Report Manager Clinical Query Builder Updates The following updates have been made to the Clinical Query Builder in support of ICD-10: The diagnosis search in the Clinical Query Builder has been updated to include ICD-10 codes. Users now have the ability to indicate whether to search for ICD-9 or ICD-10 codes and searcy by code or keyword. The ICD-9 column has been renamed ICD Code and and ICD Version column now appears in the grid. The ICD code will appear in the ICD Code column and the corresponding code version can be seen in the Code Version column 13.3 Add stratified data elements for vulnerable populations to CQM reports To satisfy PCMH 6 Element A Factor 4, a new table has been added to the report to show the practice receives data stratified by race and ethnicity or by other indicators of vulnerable groups that reflect the practice s population demographics. This allows practices to analyze patient data in a meaningful way Addition of query builder filters for race, ethnicity, preferred language and group provider The Clinical Query Builder has been updated to generate reports based on key demographic fields. 27 16 Digital Wallet Payment simplicity for patients and front office staff is essential in achieving increased productivity. The digital wallet feature will save a patient s credit card at the point of transaction processing and avoid having to re-enter or re-swipe every time the patient makes a payment. Note: Digital wallets offered through CareTracker s Payment Connect improve patient and office experiences without increasing the burdens of PCI compliance. Patient credit cards are stored by a PCI certified third party partner of CareTracker using tokenization. Tokenization is a highly secure industry standard. The token has no value outside of the payment transaction it was being used for because it was exclusively associated with a merchant ID. Note: When digital wallets are enabled, a Payment Plan button appears in the credit card processing window. 28 16.1 EMV Capable Device Supportability The term EMV stands for Europay, MasterCard and Visa. EMV is a global standard for authenticating credit and debit card transactions with integrated circuit cards, or chip cards at capable point of sale (POS) terminals. As of October 1, 2015, merchants that accept credit card payments at the point-of-service will be liable for any counterfeit card transactions that occur when a chip card is swiped at a terminal. This is a transition from the way things are today, where the issuing bank is liable for point-of-service cardpresent fraud. This transition from the issuer to the merchant on October 1, 2015 is known as the liability shift. CareTracker s Payment Connect solution, powered by InstaMed, now supports POS solutions that can read chip cards. Below are FAQs on how you can equip your practice with EMV capable devices. How does EMV protect merchants? There are two ways that EMV protects merchants: Chip cards are more difficult to counterfeit than their magnetic-strip predecessors, and counterfeiting makes up the majority of point-of-service fraud in the industry. If point-of-service fraud does occur at a terminal that can accept EMV transactions, then the liability for that fraud stays with the issuer, and does not shift to the merchant. How does EMV verify the cardholder? There are two different verification methods: Chip & PIN: User dips the card into the payment terminal and enters a PIN number Chip & Signature: User dips card into the payment terminal and presents a signature for verification 28 of 33 CareTracker v9 Release Notes v4

29 How does an EMV transaction work? New payment transaction devices are required; instead of simply swiping a card, a consumer must dip or insert their card into a PIN device and enter a PIN or provide a signature before a transaction is processed. Is EMV the only payment data protection I need? EMV by itself does NOT protect the merchant from a data breach. EMV will protect healthcare organizations from accepting fake credit/debit cards at the point of service. Encryption will protect healthcare organizations networks in the event of an enterprise-level data security breach. When implemented properly, payments processed with EMV encryption can almost completely eliminate the risk of a data breach. Can any credit card reader support EMV payments using CareTracker Payment Connect? No. In order to accept EMV payments with CareTracker Payment Connect, merchants must use the MagTek DynaPro device. The following models support encrypted magnetic card swipe, EMV, and NFC (Apple Pay): (USB connection) 29 of 33 CareTracker v9 Release Notes v4

30 (USB connection, and signature capture) (Ethernet connection, and signature capture) Do I need to configure my receipts differently for EMV transactions? No. Additional information will be automatically generated on the receipt for EMV transactions. What happens if a cardholder has a chip card but swipes it on the DynaPro? When a credit card is swiped, the DynaPro can detect whether a chip is present. If the card does have a chip, then the cardholder will be prompted to insert the card into the EMV reader location. If the cardholder continues to attempt a swipe, the DynaPro will repeat this workflow three times before ultimately allowing the swipe. 30 of 33 CareTracker v9 Release Notes v4
