1 Welcome to Samples Plus If you are a Doctor, Midwife, Child Birth Educator, Nurse Specialist, Plunket Nurse, Specialist Doctor, Maternity Hospital - you can register on the site to obtain access to the brands represented above. Register your details. This document will show you how to place an order with Samples Plus First you must register your details. Please make sure you include the full name of the business that your order will be couriered (Signature required) to. Hospital Name or Practice Name If your deliver y is to a hospital or Doctors surgery with various different departments/buildings please specify that on the delivery details (example Ward 6 or name of building within the grounds) and please make sure you attention it to the receiver so that if you are not there it is known where to go. Please do not enter in a PO BOX number for delivery as we use NZ Couriers and they do not deliver to PO Boxes. Step 1 If you have already registered please log in If you are registering for the first time please be aware when choosing a username and password that these will be case sensitive so if you have a capital in your chosen wording or you choose to use a space, then you will need to make sure you do that all the time. To keep it simple make sure the word you use does not have any capitals or spaces in between

2 Image of what the registration page looks like. Once your registration is completed our team will process your application to make sure you meet the requirements of Johnson & Johnson. Please note this site is for the use of Health Professionals only. Not for public use. Where it asks for your medical number Please use your registration number (Doctor/ Nurse/Midwife registration numbers) so that this can be verified that your registration is current and active. (This is so we can keep the website strictly to Health Professionals only) If you have married/changed your name since your first registration in your profession please us on to let us know your maiden/birth name as this may result in delays with your application if the name you have signed up to and the registration name of your profession is not the same.

3 If you do not have a registration number (You are not a Midwife/Nurse/Doctor) but you are a midwife student/ office administration or other please write your occupation in the medical number section. We realise that sometimes the Practice manager may be responsible for stock control and they may not be a Nurse/Doctor/Midwife so it is important to list your occupation then. It is very important to have an address for registration this is a requirement to signing up to Samples Plus. Please note that we will never sell or share your details with any other company so you will not receive Spam mail. The purpose for the address is that this is how you will receive your confirmation of order and dispatch saying your order is on its way. Also we will you if we need to communicate with you about your order. It does not have to be a work address it may be a personal one if there is not one at work. Please note though that if more than one person from an organisation is placing orders with Samples Plus then they will not see those s only the person whose address is on the registration. Johnson and Johnson will from time to time let you know about New J&J products arriving for you to purchase via and also from time to time there may be Competitions available solely for Midwives/Doctors/Nurses e.t.c. You are not obliged to receive all these promotional s solely from Johnson and Johnson. When you register or at anytime if you go to your account, you can opt out from receiving this material Once your application has been sent it will be reviewed and if your application is successful, you will receive an welcoming you to samples plus and it will have your user details on there for you to store safely.

4 Step 2 Ordering. Go to the side menu on your left to choose which brand you need Roll your mouse over the brand you wish to purchase from. You will see 2 more sub categories pop up Products and Literature If you are wanting booklets or posters click on the Literature of any of the brand names (Aveeno/Clean &Clear/Carefree/Stayfree /Johnsons Baby/KY/Neutrogena/Purell/Retin A/ Miscellaneous) The same applies for products. If you are not sure who makes the item you are after simply scroll down to the bottom of the menu and you will see a drop down box. This drop down box has been made into 3 categories: Products/Samples/Literature. Click on the one you want and it will show all products/samples/literature of all the brands in one category. *If you roll over a category and there is nothing in it it may be that this is not allowed for your speciality. Every profession has different requirements, so at Samples Plus we try to make it as simple as possible for you. If your profession does not require an item it will not show up on your menu page. (Example a Skin cream may be for Doctors/Dermatologists but not used with Midwives/Plunket/Child birth Educators)*

5 Once you have selected the item you wish to purchase make sure you add in your quantity or use the drop down box to pick the allocated quantity. Click add to order *Please note that there are quantity restrictions on some of the items* You can then choose to continue shopping or you can choose to complete your sale as shown in the picture below. You can CONTINE ADDING PRDOUCTS if you want to add more or if this is the only item you want then click the COMPLETE ORDER button When you have finished requesting what you want click COMPLETE ORDER

6 Step 3 There are 3 types of orders. - Samples and or Literature items only (No Charge) - Full size products (chargeable items at great discounts) - Mix of both Samples and or Literature and Full Size Products If you have selected non chargeable items please follow instructions until you receive your order number (and an confirming that your order has been placed/received) For Chargeable items (Full Size Products) Please follow onscreen instructions and you can choose from the following for method of payment - Credit Card Secure Online. Your Credit Card Statement will show Pharmacy One as this is the parent company Pharmacy Direct who owns Samples Plus website. - Direct Credit You will see the bank account number at the end of the process. Please make sure you use your order number as the reference for your payment

7 - Cheque Please forward your cheque with your order number on the back of the cheque to: Samples Plus PO Box Penrose Auckland 1062 Please note on your billing statement it will say PHARMACY ONE for payment. This is the parent company of Pharmacy Direct who own the website. Johnson & Johnson own the content. Once you have received your order number and confirmation sit back and relax and wait for your order to arrive.
