3 1. Instructions on how to register to use the service 1. Visit the website. Before using this service for the first time, the member must register by clicking Click to register 2. On the Register screen, the member is asked to enter the relevant membership data set or statement of provident fund account by TMBAM which includes: - Company Code - Unitholder Number - Ref. Code Complete the registration process and indicate an address which will be used to confirm the member s registration. In case of membership data set is locked, please go to page 11 Remark: 1) Securely keep the relevant data required for registration; it is needed again to re-register if the member loses his/her password 2) Please provide information to all boxes with an asterisk (*) 2

4 3. Once the system has successfully accepted the registration, the system will issue a confirmation of the registration to the address provided by the member. 4. Open the message received, and click on the link of the URL shown to proceed to the activation process. xxxxx xxxxx xxxxx xxxxx Remark: 1) If the member is unable to successfully click this link, please proceed to click it with the right-mouse button instead and select Copy Link Location. Then paste this link into the browser to open the web page. 2) This link for activating the service is only valid for up to 30 days; if unused within this time, the member must repeat the registration process again. 3

The member will be asked to provide the Unitholder Number and to select a Username and Password that meets the following conditions: - The Username must begin with an English alphabet followed by a number (0-9) or other alphabets (A-Z, a-z) or symbols (-,_,.,#) with a combined length between 4-30 characters. - The Password must contain at least 6 characters in length and must not contain spaces Remark: 1) Please provide information to all boxes with an asterisk (*) within 15 minutes 2) Do not select a Username and a Password that are identical and avoid using personal information such as a birthday as a password. 6. Once the system has acknowledged the member s registration and activated the service, the member is ready to use Fundlink M Choice. 4

6 The system will also confirm that the member has successfully set up a Username and a Password via that the member indicated at registration xxxxx xxxxx xxxxx xxxxx 5

7 2. Using 1. Visit the website and navigate to Click 2. Log in with the appropriate Username and Password that has been selected Remark: If the Password is entered incorrectly more than 5 times, the system will be locked for security purposes. Please refer to Forgot Password for additional details. 3. If the member enters the Username and Password correctly, the Disclaimer and the Terms and Conditions of the Service will be displayed. The member will be asked to acknowledge this message by checking the box and click Agree to proceed. 6

8 4. Once the aforementioned remarks have been acknowledged, the member will be taken to the welcome screen of the FundLink M Choice service. This is the main page where the member can navigate to retrieve information about his/her investment by clicking on menu items on the left hand side which features Inquiry and Historical Data. 9 3. Steps to take if you forgot Username and/or Password 3.1 In case of having Membership data set or Statement of Provident Fund Account 1. Click Forgot Password 2. Register again, using the same set of information provided by TMBAM to register before. Remark: please provide information to all boxes with an asterisk (*) 3. Once the system has accepted the new registration, the system will send a confirmation to the provided at registration, that it has successful reset the password. 8

Open the message received, and click on the link of the URL shown to set a new Password. xxxxx xxxxx Remark: The link that is provided for the purpose of resetting the password will only be valid for up to 30 days. If unused within this time, the member must repeat the registration process again. xxxxx xxxxx 5. The member shall specify a new Password which meets the following conditions: - The Password must contain at least 6 characters in length and must not contain spaces 6. Once the system has accepted the new Password, the system will send a confirmation via that was provided at registration.the member is now able to access FundLink M Choice. 9

11 3.2 In case of Loss Membership data set Follow as given below; 1. Visit the website 2. Click on Services (Left Corner of Website) 3. Click on Form 4. Click on Provident Fund 5. Click on Release/Reset register internet form of FundLink M Choice Online (for Fund member) 6. Fill the Form choose Reset registers information 7. Send the Form to fund committees for authorized signature. 8. Fund Committees send the form to TAPVD, TMBAM. Reissue Membership data set fee is 50 Baht (Included VAT) 10

12 4. Steps to take if membership data set is locked Follow as given below; 1. Visit the website 2. Click on Services (Left Corner of Website) 3. Click on Form 4. Click on Provident Fund 5. Click on Release/Reset register internet form of FundLink M Choice Online (for Fund member) 6. Fill the Form choose Release lock of register information 7. Send the Form to fund committees for authorized signature. 8. Fund Committees send the form to TAPVD, TMBAM. TMBAM will Release lock of register information 5.
