3 Logging into the Covenant Physician s Portal 1. Double click the CovMD icon. If no icon is available, click Covnet, then CovMD at top of screen. 2. Type your username and password (make sure the caps lock is not on). Note: The first time you login to the portal, you will be required to change your password and sign the user agreement. You will be prompted annually thereafter to change your password and sign the agreement. 3. Passwords must consist of only alphanumeric characters (no spaces or symbols) and a minimum length of 8 characters. 4. You will be prompted at first login, then once yearly, to sign the Acceptable Use agreement. 3

4 Anatomy of the Physician Portal The basic definitions of terms you will need to know when operating within Horizon Physician Portal are: 1. Patient Navigation Bar drop down box providing a list of patients selected from the working patient list for easy access across all pages. This is available on multiple tabs and provides an easy way to switch between patients. See page 16 to customize. 2. Page or Tab used interchangeably, the page or tab can contain one or many modules and also one or many subtabs. 3. Module component of information displayed within a chrome bounded area 4. Chrome bar across top of module that contains the name of the module and the icons and edit buttons available for that module. 5. Help Button small icon with? that accesses the help screens for the portal. 6. Edit Button allows user to open an edit screen where defaults can be changed and saved for that user 7. Minimize/Restore Button - allows the user to minimize the module so that ONLY the chrome still shows 8. Printer Button small printer icon on top right portion of the module chrome which allows the user to print the information contained within that module in a printer-friendly formatted window. To exit, use the logout icon, not the internet explorer X. Do not use the back button... 4

5 Patients Tab The Physician Portal will default to the Patient s Tab. If you are designated as a physician of record (Admitting, Attending, Consulting, Referring, Primary Care), your patients will appear under Working Patient List. The default Census Type is My Patients. You have a choice of two views for your Patient list: Data Monitor provides a quick glance at your patients and the indicator for Lab results including Alerts. New results for Rad, Transcription, and Lab will also be indicated. Note: Once a result has been reviewed, it will not be displayed as a new result. Patients can be selected by clicking the checkbox to the left of the patient name. These buttons will then become active for use: Note: Clicking on the will select all patients. Clicking on the will deselect all patients. Census List displays patient demographic data such as age, gender, diagnosis, relationship, etc. Note: While in census list view, the print selected and print all buttons display for printing census list. The number of blank lines between patients can be changed in Edit (see below). CM Working Patient List Note: When choosing new selections within this section of Portal, the Refresh button must be used to change the data. Edit (Set Preferences) for Patients Page An edit option is available on many modules. Be sure to click on this button upon entering Portal for the first time in order to guarantee that all available options have been personalized to the fullest extent. Editing the patients page allows users to set up defaults for searches, hyperlinks to other pages, add/remove columns, controlling the number of blank lines between patients on the printed census list, etc. Note: Don t forget to scroll to the bottom and changes that are made on the edit page by clicking the button. Once defaults have been changed, they will remain that way until changed again, even after a user has logged out and logged back in. Clicking reset will change your settings back to system defaults. 5

6 The flowsheet tab displays nurse or ancillary staff charting on the selected patient. Flowsheet Tab Selecting different criteria will change your views. Click to update view. Flowsheet Hotlinks allow you to quickly access other flowsheets. Six Hotlinks are available for use. Click Edit to select your Hotlinks. Red values indicate significant values. Red magnifying glass indicates significant values within time period. To switch between different flowsheets, click the Report drop down list arrow and select a report from the list, then click Refresh. The Time Interval drop-down allows your view to be in Shifts, Days or Hours. To search by number of days back, click the Start On check box, a default date is entered. Enter the number of days to go back in the Go Back text box. Click Refresh. Hot Links & Defaults To change or create Hotlinks, click the Edit Button on the Flowsheet toolbar. Note: Don t forget to scroll to the bottom and by clicking the button. changes that are made on the edit page 6

7 DocView Tab The Documentation View tab provides a narrative view of the flowsheet data and charting. Selecting search criteria, views, defaults and setting preferences are the same as with the flowsheet tab. Please refer to previous page for details. Results Tab Lab, Radiology, Pathology and Cardiodiagnostic results can be viewed on this page by clicking the name of the test/exam in the list. Selection criteria for searching results. Clicking the Radiology icon will open the WebPacs application to see images. Note: A V will appear beside the results icon after a Result has been opened, indicating that the results have been viewed. 7

8 Results Tab continued... Clicking the test name will direct you to the results. Clicking Check 5 will trend results. Click Check 10 to see further trending. Red values are significant values. Tip: Use the Back to List to return to the entire results list after viewing a result. Covenant Reports Module Many customized reports are available in the Covenant Reports Module and will print for the patient in context in the Patient Navigation Bar, including the MD Progress Note. Note: For physicians, there is also a link to add the current patient that is displayed in the Patient Navigation Bar as a consult in STAR (registration system). There are additional pages available under the Results Tab: Mass Print Module HCS Medication Reconciliation Covenant Reports Module Add Patient as Consult in STAR Clinical Phone Module is available on the Results Page: Patient Demographic Module is available on Most Pages and contains Patient Allergies as well as radiology icon to view a list of all images: 8

9 Results Tab continued... Edit (Set Preferences) for Results Page Select chosen defaults based on your preferences in the edit of Results Module. Graphed results will display at the bottom of the check 5 or check 10 results page, if display check graphs is chosen on the Edit screen. Transcription Tab The transcription tab allows viewing of transcribed reports for a selected patient such as Case Management (Allscripts), History & Physicals, consultations, operative reports, etc. Click on the name of the report to display the transcribed report. Edit (Set Preferences) for Transcription Page Select chosen defaults based on your preferences in the edit of Transcription Module. 9

10 EKG Tab Access to MUSE to view EKG tracings and stress test reports. Click on the report name to open Adobe for viewing. Clicking the hyperlink name will open Adobe pdf in a new window and display tracings and findings. Summary Tab The summary tab displays a summarized view of information from the last 24, 48 or 72 hours. Views can be manipulated on-the-fly by selecting criteria then clicking refresh. Note: Viewing the summary tab will mark all results as viewed on the Results tab. The summary report includes sections for: Vital Signs Review I/O s Review Laboratory Review Radiology Review Transcription Review Orders Review Medications/IV Orders Review 10

11 Vital statistics link provides a quick snapshot of patient s vitals for dosing aids. Meds Tab Displays all active, inactive, discontinued medication and IV orders for the patient name displayed in the patient demographic box. Hyperlinks: Drug = Drug Information Detail Order Dose = Order Information Detail Frequency = Future Scheduled Times Last Given = Medication Administration History Next Sched Admin = Future Scheduled Times Launch Micromedex to search Drug, toxicology, disease and lab databases. Edit (Set Preferences) for Meds Page Changing Sort Order of Meds 11

12 Pt Locator Tab Searching for a Patient: By Name: o Enter full or partial last name and a % (wildcard) for a complete list of all possible matches. (i.e. if you are searching for John Smith Jr, enter Smith% in last name field and John in first name field. By MRN: o Enter the 10 digit MRN of the patient in the last name field (leading 0 s must be entered if MRN is less than 10 digits). By SSN: o Enter a hyphen followed by the number with or without separator characters in the last name field. By Account Number: o Click Active or Recent Encounter radio button, enter account number in the account number field. Patient Location by Floor: Choose Facility and Station from drop down lists. Click Get Census. Columns can be sorted by clicking on the column header To add a patient to your Horizon Clinicals patient list, select the Add next to the patient name, then click Add to Patient List. NOTE: This does not add the patient to your STAR patient census. Edit (Set Preferences) for Pt Locator Page Clicking a patient name, will direct you to the summary page for that patient. You can select your hyperlink preferences to any column by personalizing the Edit section of the CM Station Census. 12

13 Facesheet Tab Displays the patient facesheet from STAR or Horizon Clinicals. Use the print button in the upper right corner of the module to print the selected facesheet. Orders Tab Displays all orders for the patient name displayed in the patient navigation bar. The displayed columns, order groups and order statuse s can be customized using the Edit button. Selection criteria for searching orders. Click Edit to set default preferences & click Save. If results or images are available, click the Available hyperlink to view results or icon to view images 13

14 Orders Tab Continued... MOTS (Medication Order Transmission System) only consists of documents scanned to the hospital pharmacy by nursing as part of processing medication orders. It does not represent a complete chart. Clin Prof Tab The clinical profile tab displays the patient s clinical history gathered at Admission. 14

15 Sovera Tab Deficiencies IDocView The medical record can be viewed via the Sovera tab and can be accessed by clicking one of three icons. Sovera application will open in a new window. The total number of deficiencies will be displayed and clicking the link will allow a physician to review/sign his medical records. Click the account number to review the document, then click the folder icon with green check mark to sign the documents. Clicking the hyperlink to view all document types will provide an indicator of document types within visits: Launching IDocview will allow physicians to review transcriptions and electronically sign their deficiencies. Click the Launch IDocView icon to open application. When idocview application launches, click the inbox, then doubleclick the patient s name to view the report and electronically sign. 15

16 Misc Tab Manually add/update application account login/passwords to access the available applications from within the portal Providing feedback will generate an to the physician support group and a ticket to the helpdesk. Click Edit to enter your preferred contact information. Allows I.T. to shadow your pc when providing support Add links to commonly used websites. Site Controls Add Station Census to First Page or Remove Mailboxes in Arrange Portlet Layout Customize Patient Navigation Bar in My Preferences : Patient Navigator Settings Patient Data In addition to the Patient Name display: Change Password MRN Change Password PI Customize Patient Navigation Bar Remove MMPI ailboxes Add Station Room/Bed Census Module to Patient Page 16

17 Misc Items Mom & Baby Link Edit the Working Patient List to include the Mom/Baby column. This adds a link to the baby s chart and displays on the list across from the Mother s name. Click on baby to be directed to the baby s chart. (Multiple babies for one mother will display on a list. Select the appropriate baby from the list). In the system, babies are named as follows: Last name=last name of mother First name=sex of baby+number if multiple Middle name= First name of mother OBLink If mapped, OBLink will be available to launch on the Results Tab and is a link to the fetal monitoring system. Mass Printing The Launch Mass Print is located on the Results Tab under Additional Covenant Reports. Mass printing MD Progress Notes or MD Rounding Reports from your census list can be printed by selecting your patients and clicking either Print Selected for MD Progress Notes or Rounding for MD Rounding Reports on the selected patients. Find Staff Tab Search for physician addresses / phone numbers in STAR Staff Directory. Search for a physician s or group s patients in CM Staff Directory to add yourself as a consulting physician in Horizon Clinicals (does not update STAR). 17

18 Directions to set up Covenant Secure Gateway (Remote Access using a FOB) 1. Login to the PC as a user with Administrative rights 2. Open Internet Explorer 3. In the address line type in and save this in your Favorites. 4. Click on Physicians link at the bottom of the Covenant Health website. 5. Click on the Covenant Secure Gateway and Physician Portal box. 6. Click on the Citrix client download site link on the lower right. You will be taken to the vendor s download site. 7. Click the Citrix Clients box. 8. For Wind ows, choose the Online Plug-in Windows 32, 64 bit. (There is also a Mac client but we will not cover the installation in this document.) 18

19 9. Choose the Download button for the Citrix Online Plug-in Web. 10. When the window for the Download Manager pops up, choose the click here link at the bottom of the window. This will download the client without the assistance of their Download Manager. 11. Click Run to any windows that open. 12. After installation, proceed back to the CSG login page Enter your Username, Password, and Passcode as indicated on the small sheet that was initially included with your FOB. Note: the Passcode consists of the number (PIN) written in the Passcode box on the sheet followed by the # displayed on the FOB at that time. 13. Click Log In. 19

20 1. Open Internet Explorer and in the address line type in wi.covhlth.net. Accessing Portal via Web Interface (WI) (Remote Access on Covenant Network) 2. Enter the username and password assigned to you and click Log In. 3. Two icons will be present in the Applications box. One each for CovMD and Sovera. Note: If only Sovera icon is present, click the Top or Up icons to move up one folder 4. Click the CovMD icon. CovMD will load up into Full Screen mode. Enter your username and password in the fields provided if not already saved. Otherwise click the Sign In button. 5. After exiting CovMD, click the blue Covenant Log Off icon at the top of the screen to exit WI. 20
