3 Accessing myconnections From HealtheConnections website ( click on myconnections link to bring you to the log in screen 3
4 myconnections Login Web address (bookmark) for HIE log-in page: There are two ways to create a shortcut to this log-in page. You can add it to your favorites/bookmarks OR you can drag the address from the address bar and drop it on your desktop and an icon will appear When logging in you will have five attempts to enter your correct username and password. Your account will lock after five failed log-in attempts, and you will need to contact HealtheConnections Support at (315) x5 to request a password reset Green box is valid username and password; Red box is invalid or nothing entered You can click on the Forgot Password? link to prompt a password recovery process 4
5 Navigating myconnections Upon logging in, you will see the myconnections screen. Your name and the Sign Out button both appear in the top right-hand corner Weather is based on location. You will have to click Allow on your browser when prompted Important messages and news from HealtheConnections regarding outages or new features will be posted on the right side of the screen, below the weather Clicking the red lock will unlock the buttons on the screen. You can click and drag the buttons to rearrange them. Click the lock again (it will be blue) to re-lock your screen HealtheConnections News displays the most recent news from the HealtheConnections website ( 5
6 Navigating myconnections The icons in the top left-hand corner of the screen will take you to: o Home: myconnections homepage o World: HealtheConnections website ( o Pinpoint: Interactive Participant Map Patient Lookup button will take you to Patient Lookup and myresults Direct Mail will take you to Direct Mail service myalerts will take you to the myalerts portal 6
7 Patient Lookup: Search There must be a Reason selected in order to execute a search. The reason automatically defaults to 2: Treatment ; however, you can choose other options by clicking the down arrow or selecting a number (1 through 4) that corresponds to the options below: 1. Emergency Treatment 2. Treatment 3. Manage Consent 4. Public Health Agencies Only You may search for the patient by using either: Demographic Search where Last Name and Date of Birth are required Identifier Search by using the Patient Alias which is the patient s Hospital Medical Record Number The required fields are marked with asterisks. You can further refine your search by filling out the other data elements, however they are not required 7
8 Patient Lookup: Search If multiple patients with the same information are returned (e.g., they have the same last name and date of birth), the system will then display additional criteria (e.g. first name, address, gender, etc.) in order for you to choose your patient Click on the desired patient and the Patient Summary will be displayed, dependent on the patient s consent for your facility 8
9 Patient Lookup: Consent and Summary If patient consent is No, you will not be able to access their records If the patient authorizes a change in their consent, you may Add Consent If the patient does not authorize a change in their consent, you are unable to view further and may return to patient search for a new patient lookup 9
10 Patient Lookup: Consent and Summary If patient consent is Emergency Only, you will have the option to Break the Glass in an emergency situation only, Add Consent, or return to patient search 10
11 Patient Lookup: Consent and Summary If Patient consent is Yes, you will be directed to the patient s summary record The patient s information is categorized into results, medications, allergies, encounters, etc. The number appearing in parentheses ( ) indicates the number of records in that category Laboratory results, radiology reports, and transcribed reports (documentation) are broken out If there are more than 100 records associated with this patient, a pop up dialogue box will appear in the top right corner of the screen that asks if you would like to show all patient data 11
12 Patient Lookup: Consent and Summary Users have two options to choose from when viewing images in Patient Lookup: Option 1A: Click on View All Images Tab Option 1B: Open any image you would like to view by clicking on the image description Option 1 A. Click on the View All Images tab. Once clicked, the Image Worklist will open. B. Click on any image you would like to view 12
13 Patient Lookup: Consent and Summary Option 2B: Click View All Images button and worklist will load Option 2A: Click on View This Image and you will only view the image connected to this report Option 2 A. When the Image Report is open click on View This Image which will open the image viewer for the currently opened image B. Click on the View All Images button and the Image Worklist will load, showing all images in a patient s record You will still have the ability to Download Report The report will be posted into an Adobe PDF document where you can then save, print, or per Adobe Acrobat functionality Close Report by clicking X in top right corner to return to Patient Summary 13
14 Patient Lookup: Consent and Summary Once the Worklist is open, select the image(s) you would like to view by clicking on the radio button to the left of the image information After selecting the image(s) you would like to view, click the View Studies button Up to four images can be viewed at the same time for a side-by-side comparison: 14
15 Patient Lookup: Summary Users who have been configured with Transfer-to-PACS privileges will have access to historical image transfer information. The Transfer History tab, as shown below, provides authorized users with a list of all imaging study transfer requests initiated by all users associated with their facility. The list is configurable to include requests made over the past 24 hours (default), 7 days, or 30 days.
16 Patient Lookup: Consent and Summary You can use the options in the Patient Actions area on the left to do the following: Back to List Brings you back to the list of Patients that were returned with your search Download CCD/Download CCDA CCD Creates a Continuity of Care Document of the Patient Record as a summary of the most commonly needed information about the patient Download Summary PDF Creates a summary of the entire Patient record Configure Layout Allows you to configure the layout of the Patient Summary screen You can return to the Patient List or Patient Search screen by clicking on the Patient tab in the upper left 15
17 Patient Lookup: More Patient Information The More Patient Information tab shows additional patient demographic information Clicking on the links Other Information, Advanced Directives etc. will display additional information in each respective area Clicking on Consent will also allow you to manage consent to make changes to the patient consent value for your practice You can log out at any point by clicking Logout in the top right corner 16
18 Disabling Pop-Up Blockers You must disable any pop-up blockers to view downloaded reports or eht images You will see the screen above on Internet Explorer if images are blocked by a pop-up blocker Type or copy/paste the following address into the box labeled Address of website to allow in order to view images: ix-heccny.ehgt.com Click Add to save settings 17
19 Disabling Pop-Up Blockers You must disable any pop-up blockers to view downloaded reports or eht images You will see the screen above on Google Chrome if images are blocked by a pop-up blocker Type or copy/paste the following address into the box labeled Hostname pattern in order to view images: ix-heccny.ehgt.com Set Behavior to Allow and click Done to save settings 18
20 Query-Based Exchange: Veterans Administration/Department of Defense Documents VA/DoD Patient Lookup includes documents not included in the Summary or More Patient Information tabs. This includes documents returned from an automated query of the VA/DoD. Once the patient s record is accessed, the VA/DoD Documents query will begin based on the patient information on the record To view any data pulled from the query, click on the VA/DoD Documents tab NOTE: Not all patients have VA/DoD data available. A No documents returned notification will appear if no documents exist from the VA/DoD systems 19
21 Query-Based Exchange: Veterans Administration/Department of Defense Documents Click on a Document ID (highlighted in blue) to view the document The document from the VA/DoD system will open in a new tab for READ ONLY access All returned documents from the VA/DoD documents query will remain on the patient record for 15 minutes 20
22 Query-Based Exchange: Immunization Query Click on the Immunization History Query tab, then click Create Immunization Query to search for a patient s immunization history 21
23 Query-Based Exchange: Immunization Query v To search for a patient, enter in the patient s last name, first name, and date of birth. These fields are required, but you can advance your search by entering more search criteria (e.g. middle name, gender, etc.) Click Submit Query to search for the patient. Once the search has been entered, click Back to List to view the query you just submitted 22
24 Query-Based Exchange: Immunization Query The submitted query will appear in the Sent On column. Once a response is received, this will appear in the Response Received On column, and the patient s information will appear in the last name, first name, date of birth, and any other fields you entered in the search criteria. You may need to refresh the page, as the query response will not be received immediately and may take a few minutes Click on the query you would like to view. The patient s immunization query will appear with the name/date of birth fields filled in 23
25 Query-Based Exchange: Immunization Query Scroll down the page to view the immunization results from two places: o NYSIIS database Immunizations from response o HealtheConnections database (Mirth Results) Immunizations from MR If the patient has no immunizations listed with NYSIIS and/or HealtheConnections, the notification No Immunizations from will appear underneath the respective section If the patient has immunizations records listed with NYSIIS and/or HealtheConnections, the immunizations will appear 24
26 Query-Based Exchange: Immunization Query Choose one option v If the patient s information matches your records, choose Use this patient to reconcile an existing patient record with the immunizations listed If the patient s information does not match your records, choose Make a new patient to create a new patient record To add immunizations, click the check box next to Code and this will select all immunizations. If you do not want to reconcile all of the immunizations listed, you can manually select the immunizations you would like to use Click Reconcile Immunizations to upload the selected immunizations to the patient record, then click Back to List to return to the list of immunization queries 25
27 Query-Based Exchange: Immunization Query v The query will now have a timestamp in the Reconciled On column Re-enter the query to view the patient record 26
28 Query-Based Exchange: Immunization Query v Next to Reconciled to Patient, click on the patient s name to enter the patient s record. You must have consent to open the record Reconciled immunizations will be available under the Immunizations section of the patient record 27
29 myresults Please note: the first login of the day may require additional time to allow the most recently available results to load To view myresults click on the myresults tab in the upper left-hand corner of the screen 28
30 myresults By default, only the last 24 hours of provider records will show. To change the time frame of records shown, click Advanced Search in the upper right-hand corner of the screen. You will see the screen below: Once you have set the search criteria, click Submit to begin your search, then choose the record you wish to view by clicking on it 29
31 myresults You will see the screen below for each record viewed. To download a report, click the Download Report button in the upper left-hand corner of the screen To view the patient s community record, click the patient name, located below the Download Report button 30
32 myresults Consent is not needed to view results through the myresults screen. If you choose to view the patient s community record, consent will be applied at this time. If your patient has provided a yes consent value, you will be directed to the patient record immediately. If your patient has an unknown consent value or has provided a no consent value, you will see the screen above and will need to follow the standard process to access the patient s records (see PATIENT CONSENT and PATIENT SUMMARY section for consent procedures) 31
33 Statewide Patient Record Lookup Once the patient s record is accessed, the State MPI query will begin based on the patient information on the record To view any statewide data pulled from the query, click on the Statewide Data tab NOTE: not all patients have statewide data available. A No documents returned notification will appear if no documents exist for that patient in other regions of New York state 32
34 Statewide Patient Record Lookup Click on a Document ID (highlighted in blue) to view the CCD from the other regions of NYS The CCD from the other region in NYS will open in a new tab for READ ONLY access All returned documents from the Statewide Data query will remain on the patient record for 15 minutes 33
35 IMPORTANT REMINDER: You MUST LOG OUT after every myconnections session in order to securely close your session. DO NOT just close the window. 34
