1 Step-by-Step for Uploading the Teacher Bundled Report from esped to SKYWARD Teacher Access to Section 504 Accommodations Process created by Cynthia Rinehart, Assistant Principal at Johnson High School; NEISD

2 1. Go to the Hamburger icon 2. Scroll to the bottom and select Student from the menu 3. Select Student list to populate a list for the campus.

3 Click on the Cog! Select and click on the cog icon to the right side of where a column will be inserted.

4 Click Once the cog is clicked, this window pops up. click on the green button to add the column. A drop down menu, will appear.

5 Scroll to the bottom of this list and double click on the folder that is named Student. A list of ways to identify students pops up.

6 Scroll down this list to Has Active Section 504, double click or save. (If another column for a different group of students is desired, this is where it is selected)

7 The new column will populate with boxes that are either blank or checked. The entire list of students enrolled and the students who have been identified with an Active Section 504 are checked.

8 Filter by True / SAVE This column can now be sorted to show only the students with Active Section 504. Click on the cog to see the menu Click on the Filter By True radio button, and then save.

9 A complete list of students with Active Section 504 has now been created.

10 Look to the far right side of the screen and click where it says More MORE Add Live Tile The menu pops up select Add Live Tile to the dashboard

11 This creates a live tile on your dashboard. Once Add Live Tile has been clicked, an indicator to name the tile pops up. Name it 504 Students and click save.

12 Click on the house (home page) or log in to Skyward to see this tile. It is an active continuously updated- list of students who meet the 504 criteria. As a student withdraws or is dismissed or added, the total number updates and is always accurate. Reconcile the student list in esped and this list to make sure that all students are identified properly in Skyward

14 Attaching a Section 504 Teacher Bundled Report Teachers must have access to the accommodations in order to implement the program as designed by the committee. Begin by selecting a student and clicking on the curly arrow to open up the student information.

15 Attaching a Section 504 Teacher Bundled Report the menu of information for the student appears on the left. Health + Click on the plus sign next to the Health tab.

16 Attaching a Section 504 Teacher Bundled Report IHP + An expanded list under the health tab opens Click on the IHP tab (Individual Healthcare Plan). Click on the green radio button to add a student IHP attachment.

17 Attaching a Section 504 Teacher Bundled Report Section 504 Teacher Bundled Report enter the start and end date of the plan (Annual / Annual Due) Name of the report. If you do not name it, the teacher will see that there is an attachment, but will not know what the attachment is unless it is downloaded and read. After entering the information, upload the file. Select Choose Files. For District Consistency please name the file: Section 504 Teacher Bundled Report

19 Attaching a Section 504 Teacher Bundled Report The document is indicated when loading is completed. The last step is to click on the Save icon in the upper left of the screen.
