2 The purpose of this document is to determine based on your current legacy products and systems, what the Legacy to LLEAP Implementation will include in terms of components and if a Site Survey should be scheduled prior the LLEAP Implementation. Some of the questions in this document might require an IT representative at your site. Legacy to LLEAP Implementation means: - Laerdal will map Legacy hardware and systems and provide a quote for the Legacy products to be implemented to LLEAP which includes a Laerdal Field Service Engineer to come and do the LLEAP Implementation at the Customer site - The quote for the LLEAP Implementation will depend on the hardware, its compatibility with LLEAP and the size of the lab (numbers of manikins and debriefing systems) - If hardware is identify to not be LLEAP compatible, new hardware is required and will be quoted - After the LLEAP Implementation is completed the FSE will give a basic introduction to the LLEAP user interface Note: The Legacy to LLEAP Implementation does not support the following systems or hardware: LLEAP does not support Windows XP or older Onyx AVS Some 3 rd party debriefing system If your lab has one of the above debriefing systems, please fill in as much as possible in this document and contact your local Laerdal office for further assistance for a Laerdal Does It (LDI) LLEAP Implementation. 7 Summary Based on the input to this form, your local Laerdal representative/fse will decide if a Site Survey is needed prior the LLEAP Implementation. If the need of a Site Survey is determined, a quote with the total cost of the LLEAP Implementation will be sent to you after the Site Survey is completed.
