(the subscriber) hereby requests African Information Systems CC to provide ADSL access to the Internet, on the basis of a monthly subscription (paid in advance), subject to the following Terms and Conditions: All prices Include VAT. 1. The initial once off set-up and connection fee is R (incl.). This fee also applies to package downgrades. The monthly subscription (all inclusive) and cost per MB which is payable in advance, will be as per attached schedule. 2. Payment for the monthly services of African Information Systems CC shall be made in advance by direct transfer or cash/cheque payment into the Afrisys Bank Account. Top-Ups will be made with request and Plus (Pay-peruse) on receipt of invoice. Top-Up fees are available on the website 3. Please note that all ADSL-line installations will be carried out by TELKOM and not African Information Systems CC. Progress reports and installation dates must be obtained directly from Telkom ( / / ). 4. This contract is subject to African Information Systems CC legal conditions as set out at 5. The Uncapped Services DO NOT PROVIDE full line speed 24 hours per day 365 days per year. Please see the AUP on 6. The contract may be terminated by giving one calendar month notice in writing by fax or post. The subscriber, by his or her signature below, authorises African Information Systems CC to withdraw from the account specified above the once-off and monthly fees stipulated in the contract, or any outstanding fees on this account. This authority will remain in full force for the duration of the contract and any extensions of it. 7. Please note that all subscriptions are charged Monthly in advance and for a calendar month even if there is only 1 day left for the month. It is therefore recommended to start a subscription as early in a month as possible.
