2 The following is important information regarding Group registration for the 2018 Annual Meeting of the American Academy of Dermatology. Group policies for registration differ from individual participant policies listed in the program materials, and web. In order to qualify as a group, all group policies, as well as all meeting policies, must be adhered to. REQUIREMENTS You must be registering 10 or more delegates Registration is only available via the online registration system with individual credit card payments. Wire transfers & checks will not be accepted. Registration will need to be completed by the Designated Group Representative. Individual, delegate s must be supplied when registering each of your delegate via the online system. If you wish to receive a copy of the registration confirmation, the designated group representative s can be entered in the CC area during registration. You must designate one (1) primary Group Representative (DGR) to receive all correspondences and handle payment of all of the delegates registration fees. Deadline Date: Monday, December 18, 2017 at 12pm (noon) CT. Only groups that have submitted all of the required forms by the deadline date will be eligible for the mailing of the materials. Only delegates with a completed, paid in full, registration can be included in the group process. Individuals cannot be added to the group after the deadline.

You will need to obtain this information directly from the member. Non-Member Registration Categories Non-Member Physician (treating patients in an office-like setting) Mandatory questions during the online registration process: Please indicate if funding of your delegate s base registration (not including housing) is provided by one of the options below: Government Institution Learning/Educational Institution Research Institution Medical Education Company Other Self (the delegate). Does your delegate have a valid physician license/certificate? Yes or No Is your delegate a Dermatologist? Yes or No If they are not a Dermatologist, you will need to know the delegate s physician type (i.e. pediatrician, plastic surgeon, etc.)

4 REGISTRATION CATEGORIES Non Member Registration Categories (continued) Non-Member Non-Physician (corporate individuals, pharmacists, distributors, industry representatives, marketers, business developers, and scientists/researchers, CEO, COO, or Director level titles working in the field of dermatology. This also includes aestheticians and cosmetologists and all employees affiliated with aesthetician or cosmetologist procedures) Mandatory questions during the online registration process: Please indicate if funding of your delegate s base registration (not including housing) is provided by one of the options below: Government Institution Learning/Educational Institution Research Institution Medical Education Company Other Self (the delegate) Are you currently working in the field of Dermatology? 5 REGISTRATION CATEGORIES Non Member Registration Categories (continued) Resident/Medical Fellow Medical Student The two categories listed above have a pre-requisite of submitting a request for an access ID number prior to registering for the meeting. Find out more about the Access ID# request process here. If your delegate requires an Access ID number prior to registration, please allow 5 business days to process your request. We suggest that requests are submitted by Monday, December 11, 2017 to ensure you receive the Access ID number prior to the group deadline date of Monday, December 18, 2017 at 12pm (noon) CT. 6 IMPORTANT INFORMATION Verify that registration categories are correct before completion of each registration. If a category adjustment is required, a $100 USD category adjustment fee will be applied. You will want to list the delegates preferred personal mailing address when registering them. This information is encoded on their badge and will be scanned and collected by exhibitors in the hall. Individual delegate s must be supplied when registering each of your delegates via the online system. If you wish to receive a copy of the registration confirmation, the group representative can be entered in the CC area during registration. Company name for delegates registered as Non-Member Non-Physicians will be a mandatory field. We do not allow Group registrations onsite at the meeting. IMPORTANT DATES AND POLICIES Monday, December 18, 2017 at 12pm (noon) CT Deadline date to complete delegates registration and be considered a group. All delegates must be paid in full to be included on the Delegates Listing. DGR form and Delegate Listings must be sent to aadgroup@experientinc.com. New individuals cannot be added to your group after this deadline. Deadline date for delegates to finalize session ticket selections. Individuals may still exchange/return session tickets at the meeting. AFTER Monday, December 18, 2017 at 12pm (noon) CT Registration completed after the group deadline date and time of 12pm (noon) CT on Monday, December 18, 2017 thru Wednesday, February 7, 2018 at 12pm (noon) CT will be treated as individuals registrants and cannot be included on your delegate listing or your invoice summary and will not be included in the group, and will have their meeting materials mailed to the address provided during the online registration process. Individuals registered after Wednesday, January 10, 2018 at 12pm (noon) CT will be registered at the standard registration rate and will need to pick up their own materials onsite at the meeting with their photo ID. Tuesday, December 19, 2017 AAD to send group rosters to the designated group representative (DGR) for the verification of the spelling of their delegates names. Corrections due back to the AAD by Wednesday, January 3, Replacements and name changes are not permitted.

7 Wednesday, January 3, 2018 IMPORTANT DATES AND POLICIES (continued) Deadline date to submit name corrections to the AAD. Replacements and name changes are not permitted. Due to the mailing of the materials, corrections cannot be processed after this date. The delegate will need to make the correction at the meeting with their photo ID to have their corrected materials printed. Wednesday, February 7, 2018 at 12pm (noon) CT Advance registration will close. Onsite registration at the standard registration rate will now be required. After this date and time delegates will now need to register themselves onsite at the meeting with a photo ID and individual payments. Due to the volume of attendees at the meeting, we do not allow groups registration onsite at the meeting, or organizers to complete the registration and payment process for delegates. Wednesday, February 14 at 12pm (noon) CT Deadline date to cancel a registration and receive a refund. A $100 USD processing fee per person will be applied to each cancellation. Refunds will not be issued for cancellations received after the deadline date and time. Note: Please be aware that name changes are not allowed in the event of a cancellation. You will have to cancel the entire registration for the delegate who is not attending and a new registration for the new delegate will need to be completed, along with payment. New delegates cannot be added to Groups after the deadline date of 12pm (noon) CT on Monday, December 18, Please refer to the cancellation fees and deadlines listed above. Any sessions that were held in the name of the cancelled delegate cannot be transferred to a new delegate. Once the delegate is cancelled, any sessions that were affiliated with that registration will also be cancelled and placed back into the general sale.

8 RECEIPTS AND INVOICES Upon completion and payment of each individual registration, the delegate will receive a registration confirmation letter via . If the DGR was supplied in the CC area during registration, the DGR will also receive a copy of the delegates confirmation letters. This will serve as your receipt/invoice for the delegate that you registered. The AAD will provide the DGR with an all-inclusive invoice for delegates registered and paid in full by the Monday, December 18, 2017 group deadline date. Only groups that have submitted all of the required forms by the deadline date will be eligible to receive an all-inclusive invoice. Invoices will be sent via to the DGR on Tuesday, February 27, MATERIAL DELIVERY INFORMATION A Group Materials Delivery form will be sent via to all organizers who have registered and submitted the required group forms for their delegate by the Monday, December 18, 2017 at 12pm (noon) CT deadline date. The form will provide you with 2 options for the delivery of your delegates materials. Option 1 Fed Ex all delegates materials to the DGR, at the address listed on the submitted DGR form. Option 2 Mail delegate materials directly to each delegate utilizing the address listed on their registration record. To complete the group registration process please follow the steps listed below: 1. Complete the DGR form included in this brochure. 2. Once you have registered and completed the payment process for all of your delegates, complete the Group Delegate listing found at the end of this brochure.
