1 Create a New or Clone an Existing Survey Administration using the Retention Data Center 1. Log into and mynoel Levitz with your e mail address and password. If you have forgotten your password click on Forgot your Login and your password will be sent to you immediately. If you have any trouble with your log in contact us at RMS Tech@noellevitz.com or call for Annette Herring or Courtney Greene. 2. Once logged into your mynoel Levitz account, click on the button in myapplications titled Retention Data Center. 3. Click OK at the FERPA notification and you will land on the Dashboard or your Retention Data Center account. 4. Go to to begin your administration set up. 5. Click at the top of your administration list to create a new administration. OR To the right of the administration you wish to clone (replicate), under Actions, select Clone Survey Administration. This will create a new administration with the identical customization and assignments as the selected administration. Edits can be made as your review the settings. A window will pop up with a pre defined survey name populated (previous administration name followed by cloned ). Change the administration name. Click Clone Survey. Note: You will have another opportunity to change the survey administration name when reviewing the setup. IMPORTANT: Under Actions for the newly created survey administration select Edit Survey. Verify each of the survey wizard steps and make any necessary edits.

2 6. Select or Update survey form, name your administration typically for DeVry this is Form B (100 items), but many campuses use Form C (74 items) or create an administration for both. If using both be sure to include B or C in your name to distinguish between the two. 7. Provide survey description (optional) to provide additional information related to the individual survey. 8. English as the survey language remains in place. 9. Enter the last date of student access for the administration and access to their Student Report. Note: The URL specific for the survey builds as you complete the fields. 10. NOTE: DeVry University COLL 148 classes ALLOW access to the individual Student Report upon completion. 11. Add a local help desk or contact person that will appear at the bottom of each survey screen or leave it for the Noel Levitz team to follow up with students or alert you if there is a concern. 3 11. Determine the use of grouping fields. Select optional grouping field(s) if you want to capture additional demographic data. These field(s) can be used later for filtering purposes. If you do not want to use grouping fields leave all deselected and proceed. Students either select from the response fields you provide or you may pre assign response fields by including the assignment in the designated area of the student access file. If loading a file these are already included and no action is needed. If you wish students to self select, you are required to add response selections in the steps following your choice of yes. 12. Click Not grouping? Go to Step Use Display Name to correspond with titles at your institution. Example: Change Advisor to Instructor, Custom Grouping to Campus Location, etc. 14. Use Responses to enter a list of choices students can select from. (If loading a student access file, include these as part of the automation.) 15. Click (Not limiting or adding access to others? Go to Step 18.) Limiting/Allowing additional access: If you wish to assign access through a grouping field (many DeVry campuses do this to limit access to a colleague to only student records within a specific campus) you must have that person added to the Configuration list. Then the following steps activate their access: 16. Click Assign Users to the response options. Select the name(s) for access and click Save. 17. Click

4 18. Include custom questions to gather additional information about your students. Click Yes to select your questions from the existing list. Note: Questions must be added through Configuration prior to them appearing on this list. 19. Place a check mark next to the questions you wish to include in your survey. 20. Click 21. Add campus specific resource and referral information. Click Yes and use the rights side fields to paste or type your text that will appear on the report as the leading paragraph under the general Motivational Assessment categories. For example in Receptivity to Support Services: For help with financial aid questions contact or visit the Finance Aid Office at Jessup Hall. 22. Click to complete and review your survey administration set up. To review your work or your students experience taking the assessment, use this preview button to review all customization and how it appears in the survey. Note: The preview does not provide a Student Report upon completion if you have selected that as an option in the setup. If you are posting to the DeVry COLL 148 shell, copy and paste the Survey URL to the shell. This is a clickable link by your students from their own machine. The Print Survey Invitation provides an Adobe PDF that can be saved and/or distributed to any student needing the link to the administration. The URL for this specific administration is embedded in the 8 ½ x 11 document. Print as

5 many copies as needed. A Microsoft Word version of this document is also available with other quick start documents in the question mark. If additional edits are necessary for your administration, you may go back to Survey until the first student enters the administration. and use Edit For additional assistance with setup of your administration, don t hesitate to e mail the help desk at RMS Tech@noellevitz.com. Upload Student File provides a quick link to the IMPORT DATA function so you can load a file of student information with pre assigned groupings (see step 5). When a student file is loaded, tools are available for e mail invitations or scheduled reminders to personalize your communications. If you are not loading a student file, copy and share the Survey URL (link) provided or create invitations for students to use.
