2018 CAWG ENCAMPMENT REGISTRATION GUIDE Dear Applicant, This registration guide has been created for your reference as you register to attend the 2018 CAWG Encampment.

We recommend that if this is your first time using this module and/or if you're having a parent or guardian pay for the activity fee, that they assist you in the completion of this registration

If you have graduated as a student from Encampment and would like to return as a non-staff member, please proceed to register for the Advanced Training Squadron (ATS).

1 2018 CAWG ENCAMPMENT REGISTRATION GUIDE Dear Applicant, This registration guide has been created for your reference as you register to attend the 2018 CAWG Encampment. This guide contains all instructions/advice on how to complete the registration for the activity. We recommend that if this is your first time using this module and/or if you're having a parent or guardian pay for the activity fee, that they assist you in the completion of this registration process. If you have never been to Encampment, you will proceed to register as a Cadet Student (First Time). If you have graduated as a student from Encampment and would like to return as a non-staff member, please proceed to register for the Advanced Training Squadron (ATS). The deadline for first time students is Friday, 18 May. The deadline for ATS students is Friday, 1 June. It is recommended that you do not wait until the deadline nears to register, as spots will be filled on a first-come, first-served basis. If you are having difficulties completing this registration module or uploading your signed documents, please contact the administration staff at ctg@cawgcadets.org. Those having issues with payment, please contact Capt Paula Urbom-Shope at paula.urbomshope@cawgcap.org or call (562) This must be done BEFORE the deadline for special consideration or additional instructions. PAGE ONE: INTRODUCTION Please read the pre-requisites that must be completed prior. (Your registration will not be accepted if prerequisites are not met by the established deadlines.) This module will require you to have signed forms: 31, 160, 161 and 163 (If under 18). If you do not have any of the forms, you can download them in section on the left within the form. We will also need you to download and complete the Camp SLO liability form and the DD1381 form, both are available for download on the first page of the registration module.

FILLING OUT FORMS: CAPF 31 (NEED ALL SIGNATURES TO ATTEND) When filling out CAPF 31, all fields are required entry. Every space must be filled out. Rename the form (Last Name, First Name - F31).

On Staff Position(s) Sought, write N/A as you are attending Encampment as a student.

2 FILLING OUT FORMS: CAPF 31 (NEED ALL SIGNATURES TO ATTEND) When filling out CAPF 31, all fields are required entry. Every space must be filled out. Rename the form (Last Name, First Name - F31). Title of the Activity is dependent on your registration, it will either be Encampment or Encampment ATS, Location of Activity is Camp SLO., Activity Dates are 30 June - 07 July. On Staff Position(s) Sought, write N/A as you are attending Encampment as a student. (All signatures are required unless otherwise noted, see highlighted, Only one parent/guardian s signature is required.) FILLING OUT FORMS: CAPF 160 (NEED ALL SIGNATURES TO ATTEND) When filling out CAPF 160, all fields are required entry. Every space must be filled out. Rename the form (Last Name, First Name - F160). This form is about your health history, please have a parent/guardian present when filling out this form. If a field does not apply, write N/A, rather than leave it blank. If you have any dietary restrictions please include them in the allergies box as well as in the appropriate field when requested during online registration. (All signatures are required unless otherwise noted, see highlighted)

FILLING OUT FORMS: CAPF 161 When filling out CAPF 161, all fields are required entry. Every space must be filled out. Rename the form (Last Name, First Name - F161).

FILLING OUT FORMS: CAPF 163 (For Cadets Under the Age of 18) When filling out CAPF 163, all fields are required entry. Every space must be filled out. Rename the form (Last Name, First Name - F163).

3 FILLING OUT FORMS: CAPF 161 When filling out CAPF 161, all fields are required entry. Every space must be filled out. Rename the form (Last Name, First Name - F161). This form is details your emergency information, insurance, family physician and emergency contact. Please have your parent/guardian assist you in filling out this form. FILLING OUT FORMS: CAPF 163 (For Cadets Under the Age of 18) When filling out CAPF 163, all fields are required entry. Every space must be filled out. Rename the form (Last Name, First Name - F163). This form is asking your family s consent for the provision of OTC (Over- The-Counter) medication, should the need arise. Please have your parent/ guardian assist you with the completion of this form. If a field does not apply, write N/A, rather than leave it blank. (All signatures are required unless otherwise noted, see highlighted)

4 FILLING OUT FORMS: CAMP SLO LIABILITY WAIVER This form will require your signature as well as your parent/guardian s in order to stay on the base of Camp San Luis Obispo. Rename the form (Last Name, First Name - CSLO). This form is about the liability of your stay within the base of Camp San Luis Obispo. Should any incident occur, the base will not be responsible for the incident. State the program activity as Encampment. (All signatures are required unless otherwise noted, see highlighted) FILLING OUT FORMS: DD1381 AIR TRANSPORTATION AGREEMENT This form will require your signature and signature of one witness. Please have your parent/guardian sign for the witness. Rename the form (Last Name, First Name - DD1381). This form is about your liability upon O-Rides in military aircraft. Fill in Camp SLO as the place. Permanent address is your personal address/home. (All signatures are required unless otherwise noted, see highlighted)

SCHOLARSHIPS: Scholarships are available for you to attend Encampment. The scholarship is based off financial eligibility and has priority for first time students.

CANCELLATION POLICY: All refund information will be found on the cancellation policy section. Please mark all important dates in case you cannot attend Encampment.

No refunds will be issued on the day of registration or during the event. ALL REFUND REQUESTS should be emailed to the Encampment Finance Officer (Capt Paula Urbom-Shope at paula.

5 SCHOLARSHIPS: Scholarships are available for you to attend Encampment. The scholarship is based off financial eligibility and has priority for first time students. Scholarships are limited, please apply now. Scholarship Application: Cadet Encampment Assistance Program (CEAP) at programs/cadets/newcadet/ceap/. CANCELLATION POLICY: All refund information will be found on the cancellation policy section. Please mark all important dates in case you cannot attend Encampment. Full activity refund will need to be requested by 09 June Half activity refund will need to be requested between 10 June - 29 June No refunds will be issued on the day of registration or during the event. ALL REFUND REQUESTS should be ed to the Encampment Finance Officer (Capt Paula Urbom-Shope at paula.urbom-shope@cawgcap.org) PAGE ONE: INTRODUCTION Confirm that all of your forms (31, 160, 161, 163) are completed. Note that 163 will not show, as it is not required for Cadets 18 and older. Select Cadet Student (first time) or Advanced Training Squadron (returning) and press Continue.

PAGE TWO: ATS STUDENTS This section is intended for ATS students, Encampment first-time students will not need to answer these questions during their online registration, skip to Page Three.

6 PAGE TWO: ATS STUDENTS This section is intended for ATS students, Encampment first-time students will not need to answer these questions during their online registration, skip to Page Three. This section will be a fairly simple page, it will require you to answer based off your ES (Emergency Services) qualifications. A great way to check if you are qualified for such certifications (GES, IS 100, IS 700 and ICUT etc.) is to check your 101 card on CAP eservices. TO ACCESS THE 101 CARD: Go to Civil Air Patrol eservices Go to Operations on the left side. Go to Operations Qualifications on the Applications menu. Click 101 Card under Emergency Services. Enter your name or CAPID to access your specific card, it will list all qualifications.

7 PAGE THREE: BASIC INFORMATION (ALL APPLICANTS) This page will be about your basic information in personal and CAP sections. Please enter your Full name, gender, Civil Air Patrol ID (CAPID) and Date of Birth. Please confirm your age, current Cadet Grade (Ex: C/Amn) and T-Shirt size. Please enter your Unit Charter Number (EX: PCR-CA-###). Please enter your Squadron Number (Not your Squadron s Name) Please enter your Group Number (1-9). If not part of CAWG, then use your Wing s group numbers if available or enter 0. Please enter your residential address information, phone number and address, should be (cawgcap.org) or other if you don t have a cawgcap.org on Microsoft Outlook. State the name of the person who will be paying for the activity. They will be required to fill out the PayPal section after submitting your registration. Then press Continue.

PAGE FOUR: EMERGENCY INFORMATION Please enter your Parent/Guardian #1 s name, relationship, phone number and email address.

Please enter Parent/Guardian #2 (Exactly the same as #1 but different representative) This contact information will be used to alert your parent/guardian if any incident occurs. Please choose Yes or No if you require a special diet (Allergies/ Vegetarian etc.) If yes, then describe the diet. Then press Continue. PAGE FIVE: This is the page where you will upload all signed forms. (Note: All forms must be COMPLETE, all signatures) Please scan these documents and upload them in either a.doc/.docx or PDF format. If these are scanned in any other format or if you complete them online and do not save, they may come through the registration portal as blank. Name all form uploads as: Last Name, First Name - F# (EX: Doe, John - F31) Again, Cadets under 18 will need to upload a CAPF 163. Press Continue.

9 PAGE SIX: SCHOLARSHIPS (PLEASE APPLY BEFORE REGISTERING) If you applied for a scholarship, press Yes. If not, press No and press Continue. Specify your scholarship and onfirm if you have received notification of approval for your scholarship. Provide us with the scholarship code that was given when ing ctg@cawgcadets.org about your scholarship. Press Continue. PAGE SEVEN: The of the module will be your review of your entries, please make sure to read all instructions on this page. Please type in your first and last name as your electronic signature. The PayPal site must be filled out within five minutes of registration submission. Otherwise it will time out and will need to be resubmitted. The form will not save itself or submit for you if you close the browser. If the browser is closed, then you will have to fill the entire form again. Press Review.

10 PAGE EIGHT: REVIEW Please review your entries on the form, it will display all information that you have entered throughout the module. Before you submit, please read the PayPal section to be prepared to pay for the activity. When finished, press Submit. Do not exit the window. PAGE NINE: PAYPAL The site will now redirect you to the PayPal welcome screen. You will need the person stated as the payer for the activity to fill this information out, as stated on Page 1. If you have a PayPal account, sign in with your . If you do not have a PayPal account, then press Pay with Debit or Credit Card. The payment recipient should state Civil Air Patrol, California Wing on the payment site. Please enter all payment information and submit the payment. You may close the window once the payment is completed. You should receive two s from PayPal confirming the payment and from the Registration Module. The should be titled 2018 Encampment Registration Confirmation This will contain all documents and entries that were entered in the Registration Module.

11 You should now be successfully registered for Encampment. Parents, please ensure that you have received a confirmation both on your PayPal payment and the confirmation from the Registration Module. Please keep the confirmation number and a copy of all forms uploaded in case of an issue. An acceptance packet with additional information pertaining to the activity (such as directions and an equipment list) will be sent to you once the Administration staff confirms your registration and payment is complete.
