5 Clinical Document Architecture (CDA) Messages Introduction Health and Social Care Information Centre (HSCIC) are introducing Clinical Document Architecture (CDA) messages between health systems in England. The great advantage of CDA messages is that as well as being received directly into Mail Manager ready for processing, they can be linked to both new and existing Read coded and free text entries within Consultation Manager. This release, allows messages to be sent to your Vision system via the Data Transfer System (DTS) for the following categories: Community and Out of Hours Hospital Reports Out of Hours Reports Discharge Summaries Ambulance Reports Emergency Reports Outpatient Reports Transfer of Care Other Clinical Documents Unclassified Clinical Documents Note - The DTS client software regularly checks for incoming messages, meaning CDA messages arrive in a timely manner throughout the day. This guide explains how to receive and process the new messages. Important - As Windows XP is nearing the end of its supported life, the CDA functionality is designed for Windows 7 and later. 7 What do I have to do? The following options should be considered to assist in the processing of the new messages: Mail Manager - Within Mail Manager the following should be set up: Autofiling - Autofiling should be switched on so that messages are filed to the patient automatically providing they can be matched, see Switch on Autofiling (page 4). Auto-create a Clinical Review Daybook Task - see Setting up the Auto-create a Clinical Review Task (page 4). Auto-create Deducted Patient Task - see Setting up the Auto-create Message Assigned to Deducted Patient Task (page 5) Auto-create Failed Message Validation Task - see Setting up the Auto-create Failed Message Validation Task (page 5). Daybook - Within Daybook the following must be set up: Clinical Review Daybook Task Template - This must be tailored to match your practice protocols, see Setting up the Clinical Review Daybook Task Template (page 7). Archive - The archive time frame for tasks issued by an external system must be set, see Archive (page 8). Consultation Manager - Within Consultation Manager you can add an Attachment tab: Add an Attachment tab in Consultation Manager - see Adding an Attachment Tab in Consultation Manager (page 11) RBAC Permissions - Ensure your staff have the correct RBAC permissions to view and process the incoming documents, see RBAC Business Function Codes for CDA Document Processing (page 11). 8 Mail Manager Settings Switching on Autofiling Autofiling can be switched on to enable messages to file automatically into patient records. To switch on Autofiling for CDA Documents: 1. From Mail Manager, select Tools - Options. 2. Select the Autofiling tab: 3. Tick CDA Document. 4. Click OK. Options - Autofiling tab Setting up the Auto-create a Clinical Review Task When CDA messages are received, a Clinical Review Daybook task can be automatically created to notify the message recipient. The Auto-create 'Review clinical document' task option is selected by default, and we strongly suggest you leave it selected as clinical documents are expected to be clinically reviewed. 9 The Clinical Review Daybook Task automatically created should be tailored to match your practice protocols, see Setting up the Clinical Review Daybook Task Template (page 7). Setting up the Auto-create Message Assigned to Deducted Patient Task If a message is automatically assigned to a patient who has either left the practice or for whom a GP2GP notes request has been recently received, a Daybook task can be automatically sent to a nominated member of staff or a group alerting them to the fact that documents have been received. The appropriate action should be taken eg inform the patient s new practice and/or inform the organisation that sent the document message. Important - The Auto-create 'Message assigned to deducted patient' task is initially switched off by default, we strongly advise you switch this option on. To switch the Auto-create 'Message assigned to deducted patient' task option on: 1. From Mail Manager, select Tools - Options. 2. Click the Daybook tab. Mail Manager - Tools - Options - Daybook - Auto-create 'Deducted patient task' Task 3. Tick Auto-create 'Message assigned to deducted patient' task and select the member of staff or the staff group, you want the deducted patient tasks to be sent to from the list available. 4. Click OK. 10 Setting up the Auto-create Failed Message Validation Task When a message is received, It runs through message validation checks, if it does not pass validation it is rejected. You can select for a task to be automatically raised and sent to a nominated staff member or group notifying you of the message rejection. Important - The Auto-create 'Failed message validation' task is initially switched off by default, we strongly advise you switch this option on. To switch the Auto-create 'Failed message validation' task option on: 1. From Mail Manager, select Tools - Options. 2. Click the Daybook tab. Mail Manager - Tools - Options - Daybook - Auto-create 'Deducted patient task' Task 3. Tick Auto-create 'Failed message validation' task and select the member of staff or the staff group, you want the validation issue tasks to be sent to from the list available. 4. Click OK. The Daybook task created consists of the following: Description - Message failed validation Due - Today Notify - No Priority - Routine Priority escalation - No Comment - Contains details of the message that has failed validation 6 Vision 3

11 Daybook Settings Setting up the Clinical Review Daybook Task Template When messages are received a Clinical Review Daybook task can be automatically created to notify the message recipient. This task is based on the INPS Clinical Review task template. This template must be reviewed and updated to match your practice protocols. To review and update the INPS Clinical Review task template: 1. From Daybook, select Maintenance - Task Templates. 2. Maintain Task Templates is displayed. Maintain Task Templates - INPS Templates - INPS Clinical review 3. From INPS Templates, highlight the INPS Clinical Review template and click Edit. Clinical Document Architecture (CDA) Messages 7

12 4. The Maintain Task Templates screen is displayed. 5. The following options are completed automatically on your behalf: Template name - INPS Clinical review Task Description - Review clinical document Tasks created from this template are normally attached to a patient to a clinical document INPS Clinical review - Edit Update the remaining options to match your practice protocols. Important - It is highly recommended that you add default recipients to this template. Clinical Review tasks are always assigned to the staff who is allocated to the message, in case of staff absence it is useful to have at least one other member of staff receive the Clinical Review tasks. This is especially useful for messages that are not automatically allocated to staff. Change the Initial comments if required. 6. Click OK. 8 Vision 3

13 Archive The option Archive Old Tasks screen has a new option to archive Tasks issued by external systems: Archive Old Tasks The option That were issued by an external system and are older than currently includes Tasks received via: Mail Manager eg CDA messages, SCR messages GPES System administrators can select the time frame for tasks issued by external systems to be archived using the up and down arrows in the usual way. 14 Out of Office When you use the Out of Office option in Daybook, you can now nominate a staff member or group to receive a copy (you still receive the originals) of your Tasks during your absence. To nominate a member of staff or a staff group to receive a copy of your Tasks in your absence: 1. From the Out of office entry screen complete the First Date, Last Date and Reason as before 2. If required, click Choose and select either a member of staff or a group of staff from the Select staff members list. 3. Click OK to the staff selections and then OK again to save the out of office setting. Out of office entry screen 10 Vision 3

15 Consultation Manager Settings Adding an Attachment tab in Consultation Manager Adding an Attachments tab to your Consultation Manager view enables you to view all attachments received for the selected patient. To add an Attachment tab: 1. From Consultation Manager, select any patient. 2. Right click on any of the tab headings, eg Therapy, and select Organise Tabs. 3. Click Add and select Attachments List. 4. Click OK. 5. Attachments is added to the bottom of the list in Data View Pane - Organise Tabs. 6. If you want to change the position of the tab, click on Attachments and using Move Up and Move Down position it where you want it. 7. Tick Multiline Tabs if it is not already ticked, this puts the patient record tabs in multiple rows. 8. Click OK and then click OK to the refresh the Patient Record View message. If you want to filter the attachments further, to display just CDA documents: 1. Right click on the Attachment tab and select View Options. 2. Tick against Display CDA documents only. 16 RBAC Business Function Codes To be able to process the documents received it is essential for all relevant staff members to have the following business function codes added to their Smartcards. Most of your staff, if not all, should already have these. Remember - It is the responsibility of your Registration Authority to add functions to your Smartcard, please contact them to add the following functions. 17 CDA Message Processing Important - Incoming messages should be checked daily to ensure they are seen by the correct member of staff in a timely manner. When a CDA message is received by Mail Manager the following processes automatically occur: It is validated, if it fails validation a Daybook task is created and sent to your designated member(s) of staff, see Setting up the Auto-create Failed Message Validation Task (page 5). It is allocated to the designated recipient, see Allocate to Staff Mailboxes (page 14). It is assigned to the specified patient and filed, see Assign to Patient and File (page 14). Once the message is filed, a Clinical Review Task is created, see Setting up the Auto-create a Clinical Review Task (page 4). Once messages are allocated to a member of staff, they are displayed in that person's mailbox within Mail Manager. They are also displayed by message type within the mail category folder in the following sub-folders: Community and Out of Hour, Hospital Reports and Other Clinical Documents. 18 Message Validation Each message is validated against schema defined by the HSCIC. If a message fails the validation process it is rejected and not passed to Mail Manager. We recommend you set the Auto-create Failed Message Validation task option, to ensure you are aware of any messages that are rejected, and can contact the sender where appropriate, see Setting up the Auto-create Failed Message Validation Task (page 5). Example of a Message failed validation task Allocate to Staff Mailboxes When a message is received, providing the recipient is both recognised by Vision and has permission to view clinical data (see RBAC Business Function Codes for CDA (page 11)), it is automatically allocated to their Mail Manager mailbox. If a message has more than one recognised recipient, it is sent to the first mailbox Vision matches. If the recipient is not matched, but the patient is recognised, the message is allocated to the patients Usual GP. If, however the recipient is not matched and there is no Usual GP or the recipient does not have permission to view clinical data, the message is allocated to the Unallocated Mail folder ready for manual allocating. See Allocate Mail to Staff on-screen help for further details. 14 Vision 3

19 Assign to Patient and File When a message is received the system matches the patient using the patient NHS number and date of birth. If the NHS number is not part of the message a match is attempted using the date of birth, gender, first forename, surname and post code, unless all of these are matched the message is not allocated to a patient and must be allocated manually, see Assigning Patients on-screen help for further details. In rare cases, a message may contain an unverified NHS number. An unverified NHS Number being one that has not been checked against the Spine, eg a patient has provided the NHS Number themselves. Mail Manager still performs patient matching, but the message tab displays a warning. Unverified NHS number indicator If a CDA message is allocated to a deducted patient, the message tab within Mail Manager contains a warning. We recommend you contact the originator of the message to notify them that they need to send the message to the patient's current registered practice. 20 If you manually assign a patient, you must then manually file the message. Manual Filing To file a message: 1. Highlight the message required and click File. 2. The Attachments - Add screen is displayed. 3. Click OK to save. 4. The filing progress bar is displayed and then the message is updated with Filed. Deducted Patients If a CDA message is assigned to a deducted patient, or a patient for whom a GP2GP request has recently been made, the message viewing pane displays a transferred out warning message and a task is sent to the nominated recipient as set up in Mail Manager see Auto-create Deducted Patient Task Option (page 5)): Mail Manager - CDA message for deducted, recent GP2GP record transfer or unregistered patient 16 Vision 3

21 Viewing a Received Document You can view a document from any of the following places: Mail Manager - Highlight the message and it is displayed in the reading pane, click View Document and the document details and available actions are displayed, see CDA Messages in Mail Manager (page 24). Consultation Manager - Locate the attachment entry (this is where the Attachments tab is useful), double click on the entry and click View or, right click and select View Attachment. See CDA Messages in Consultation Manager (page 27) for details. Daybook - A clinical review task is created for the recipient of a CDA message, if a task has a document attached to it, View Document is enabled, click to open and view the document. See CDA Messages in Daybook (page 25) for details. Clinical Document Viewer - From Apps controller - Clinical Document Viewer, select the patient required, right click on the document and select Preview, see CDA Documents in Apps Controller (page 29). Find Clinical Document - From Reporting - Search and Reports - Reports - Clinical Documents - Find Clinical Document, select the document you require, see Find Clinical Document (page 31). 23 Processing Documents from the Document Viewer All aspects of processing data contained within a document can be carried out from the document viewer toolbar: Status Filing Where a green tick red cross indicates the action has been completed and a indicates the action is yet to take place. Read - Indicates the document has been read by a clinician. Reviewed - Indicates the document has been reviewed by a clinician. Filed - Indicates the document is Filed. Outstanding Tasks - Displayed if an outstanding task is linked to this document. File Document - Files the document to the patient record in exactly the same way as Mail Manager does. File All Clinical Items - CDA documents can contain Read coded data, once the document has been filed this can be filed against a patient record (a separate function from filing the whole document). This is currently limited to medications and allergies. File Clinical Item - Multiple clinical items can exist in a message, once the document is filed you can select an individual item from the Fileable Data list (only visible once the document has been filed) and then select File Clinical Item. Add Clinical Item A document can contain clinical data which needs to be manually recorded to the patient record. Click Add Clinical Item to display the Read Dictionary select screen. Select a Read code in the usual way and complete the resultant History - Add screen and click OK to save. Note - Once data has been added via any of the Filing options, the document is linked to the data and can be viewed from Linked Data. Clinical Document Architecture (CDA) Messages 19

24 Annotations Add Annotation You can add your own notes to a document if required. Highlight the text you want to annotate and click Add Annotation. Enter free text as required and click minimise once finished. Annotate text is highlighted in green with a green text box next to it. Manually annotated text Delete Annotation You can remove any notes you have added to a document if required. Click on the annotation you want to remove and click Delete Annotation(s). Note - You can only delete your own annotation. Links (only enabled once a document is filed) Link Document Select to link this entire document to an existing clinical item on the patient record, see Linking Documents to Clinical Entries (page 21). Link Section - Select to link a highlighted section to an existing clinical item on the patient record, see Linking Documents to Clinical Entries (page 21). View Link Select to display the patient clinical records that are linked to this document, highlight the section to interrogate and click to view. Note - Currently View Link only displays the first link highlighted. Delete Link(s) Select to remove all links from this document to any patient clinical records, click Yes to the 'Are you sure' message, or highlight a specific link and select Delete Link(s) to delete just one link. 20 Vision 3

25 Display Tasks Hide Annotations/Links - Select to hide all annotations and link highlight from your view. Show Annotations/Link - Select to display all annotations and link highlights that have been hidden. Expand Annotations - Select to expand and display view all annotations regardless of the author. Collapse Annotations - Select to collapse the annotations displayed on the screen. Audit Outstanding Tasks - Select to display the Outstanding Tasks screen, a simple list of tasks, their due date and assigned staff. This screen is for information only, click Close to close the screen. Audit - Select to view a full audit history of this document. Linking Documents to Clinical Entries The great advantage of documents received via CDA messages is the ability to be able to link them to both existing and new clinical entries on the patient record. 26 Linking a Complete Document To link an entire document to an existing clinical entry: 1. From the document viewer, click Link Document. 2. The Link Items screen is displayed: Link Items 3. Click the clinical entry you want to link this document to, you can select more than one entry. Note - Use standard Windows keys to add this document to more than one clinical entry.eg hold <Ctrl> for ad-hoc entries and <Shift> for a consecutive list. 22 Vision 3

27 4. If you are not the clinician that has authorised this link, select from the Are authorised by list. 5. Click Create Link(s). Linking a Section of a Document To link a section of a document to an existing clinical entry: 1. Highlight the text you want to link (click, hold your click and swipe until it is all highlighted in grey and release your click). 2. Click Link Section. 3. 28 4. Click the clinical entry you want to link this document to, you can select more than one entry. Note - Use standard Windows keys to add this document to more than one clinical entry.eg hold <Ctrl> for ad-hoc entries and <Shift> for a consecutive list. 5. If you are not the clinician that has authorised this link, select from the Are authorised by list. 6. Click Create Link(s). 7. The section you have linked is highlighted in yellow. Linked section Attachments Clinical documents can contain embedded attachments, double click on the attachment to view it. If the attachment is not legible, please contact the sender. The most common attachments are expected to be: Word Documents PDF Documents Internet Links Note - There is a list of attachments that, for the security of your system, are black-listed. If a document is received with a blacklisted attachment it is sent to Mail Manager with a Request rejected status. You are not able to open the document or file the message. Contact the sender to notify them that they have sent the practice a document with a black-listed attachment. 24 Vision 3

29 CDA Documents in Mail Manager To view a document in Mail Manager: 1. Highlight the message you want to view. 2. The basic message information is displayed in the message display pane. 30 CDA Documents in Daybook When a CDA document is filed to a patient, a Clinical Review task is automatically created. Providing you have set your Clinical Review Task template to be allocated to either a member of staff or a staff group, it is automatically allocated to them within Daybook (see Setting up the Clinical Review Daybook Task Template (page 7)). Viewing Documents in Daybook To view a document linked to a task from Daybook: 1. Open Daybook from either the Daybook task pane or by selecting Daybook. 2. Click on the clinical document task required 3. Click View Document to open the document. 4. Process the document as required, see Processing Documents from the Document Viewer (page 19). 5. Close the document to return to the Daybook task. 6. Once the review has been carried out by a clinician, the task can be completed in the usual way. Note - If the task has not been reviewed by a member of staff authorised to review clinical data, the Complete button remains greyed out. Reminder - Although a task may be assigned to multiple staff, only one of them needs to complete a task. 26 Vision 3

31 Processing Deducted Patient CDA Documents If a message is automatically assigned to a patient who has either left the practice or for whom a GP2GP notes request has been recently received, a Daybook task can be automatically sent to a nominated member of staff or a group alerting them to the fact that documents have been received. The appropriate action should be taken eg inform the patient s new practice and/or inform the organisation that sent the document message. Important - The Auto-create 'Message assigned to deducted patient' task is initially switched off by default, we strongly advise you switch this option on, see Setting up the Auto-create Message Assigned to Deducted Patient (page 5)). Complete the task in the usual way optionally recording your actions in Comment for audit purposes: Completing task with details of actions taken The Daybook task created consists of the following: Description - Message assigned to deducted patient Due - Today Notify on task completion - No Priority - Routine Priority escalation - No Comment - A message has been imported into Mail Manager for a deducted/soon to be deducted patient. 32 CDA Documents in Consultation Manager Once filed from Mail Manager, documents are displayed as attachments in Consultation Manager: An Outpatient Report in Consultation Manager Viewing Documents in Consultation Manager To view a document from within Consultation Manager: 1. From Consultation Manager, select the patient required and locate the document. Remember - You can filter your view by clicking the Attachment tab if set up, see Adding an Attachment tab in Consultation Manager (page 11) for details on how to do this. 2. Double click on the message. 3. The Attachments - Display screen is displayed. 4. Click View. Attachments - Display 28 Vision 3

33 5. The document viewer screen is displayed, see Document Viewer (page 18) for details. 6. Click to close the document. 7. Either, select Edit to enter free text Summary information if required and then click OK to save and close. Or, click Close to close the attachment entry. Viewing a Linked Document within Consultation Manager To view a document linked to a Read coded or free text entry within Consultation Manager: 1. From Consultation Manager, select the patient required. 2. Right click the entry you want to interrogate. 3. Select Links. 4. If there is only one document linked to a record, document viewer is displayed. If more than one document is linked to a record, Select clinical document is displayed, either double click on the document you wish to view, or highlight the entry required and click OK. 34 CDA Documents in Apps Controller Once filed from Mail Manager, documents are displayed in the individual patient's Clinical Document Viewer App: 1. From Apps Controller, click Clinical Document Viewer and select the patient required. 2. The Clinical Document Viewer is displayed. Clinical Document Viewer 3. Right click on the document you want to view and/or process, select Preview to open the document. 4. The document viewer screen is displayed, see Document Viewer (page 18) for details. The document is printed along with all other documents in the usual way from Clinical Document Viewer. 30 Vision 3

35 CDA Reporting There are two reports available for CDA documents: Find Clinical Document - Enables you to find a specific document, see Find Clinical Document (page 31). Clinical Document Report - Enables you to view and print a summary of clinical documents that match a specified criteria, see Clinical Document Report (page 31). Find Clinical Document To find a specific clinical document: 1. From Reporting - Search and Reports, select Reports - Clinical Documents and click Find Clinical Document. 2. The Find Clinical Document screen is displayed. 3. Enter part or all of the Document file name into Document Filename and click on Find. 4. The document(s) that match your file name are displayed. 5. Highlight the document you wish to view and click View Document. Find Clinical Document 6. Document Viewer is launched and the document is displayed. 7. 36 Clinical Document Report To find and create a report on a range of clinical documents: 1. From Reporting - Search and Reports, select Reports - Clinical Documents and click Clinical Document Report. 2. The Clinical Document Report screen is displayed: 32 Vision 3

38 3. Specify the report you require: Include Documents Received: Last week - Select for documents received in the last week. Last Month - Select for documents received in the last month. Date Range - Select for documents received in a given date range, and enter the range required. Document Type: Tick to include the document type. Untick to exclude the document types. Review Status: Reviewed - Select to include reviewed documents only. Not Reviewed - Select to include documents that have yet to be reviewed only. All - Select to include all documents. Include Unfiled Documents - Tick to include unfiled documents. Display Daybook task details - Tick to display the daybook task details.
