3 Setting up the Clinical module for the first time After you have installed Bp Premier for the first time, or converted from an existing clinical software package, you must apply some basic configuration to start using Bp Premier. You will need to log in to Bp Premier as a user with administrator-level permissions, such as practice manager or principal doctor, to access many of the configuration screens in the instructions. For some Windows configuration, you will need to log in to Windows as an administrator user. If you have already set up the Management module of Bp Premier, you may have already performed some of the steps below. The instructions in this guide describe the minimum required to configure a new installation of Bp Premier for use in a live environment. For more information on the configuration options available, or for more complex configuration requirements, search for the Bp Premier function in the online Knowledge Base. Complete each section in sequence. Best Practice Software appreciates that it can take some time to configure software to completely suit a practice's preferences and workflows. All of the settings described in this section can be reversed or changed at any later time. Before you start Before you start configuring Clinical functionality, you should have arranged the following items: 1. Obtain your Site ID and Minor ID from Best Practice Software. If your practice has multiple locations, each location will need a unique Minor ID. 2. Obtain your Medicare Site certificate and passcode from Medicare. If you have converted from a previous software package, you will need to let Medicare know your change in software and new Minor ID. 3. Contact the Department of Human Services to register your practice with My Health Record and obtain a NASH certificate, which allows Bp Premier to access the My Health Record gateway. Your practice will be assigned an HPI-O number. Individual providers must register and obtain an HPI-I number. 4. If your providers will be prescribing, register your practice with erx and obtain Entity IDs for prescribing providers. 5. Installed an evaluation copy of Bp Premier, and discussed your proposed Go Live date with Best Practice Software Sales. If you have converted from a previous software package, you will need to confirm that Bp Premier has successfully converted the data from the previous package and return the Completion of Trial Conversion form. 1. Run the latest data update 1. Log in to Bp Premier and select Help > About. Inspect the Last drug update field. Bp Premier Clinical Setup Guide Page 3
4 2. If Last drug update is earlier than the previous month, download and run the most recent Data Update from > Resources > Bp Premier Downloads. If Last drug update is two or more months old, you will need to download a comprehensive update. 3. Select Help > About again to check that the updated version is now shown. 2. Enter practice details and locations If you have converted your data into Bp Premier from another package, the practice details will contain the practice details you recorded in the old system, but you should check that the details are still correct. If you have installed Bp Premier with an empty database, you are prompted to enter the practice details when you first log in to Bp Premier. There is only ever one practice name in the software. However, you can create multiple practice locations, which represent different practice sites. Locations affect almost all workflows in Bp Premier and are printed on scripts, invoices, and reports, for example. Users select the location at which they are working when the user logs in. There must be at least one location record. Add practice details: 1. Select Setup > Practice from the Bp Premier screen. The Practice details screen will appear. 2. Click Change. 5 3. Record the details for your practice and click Save: Field Description Practice Name Your practice s trading or usual name. The value here populates the General / Practice name template field, which can be used in practice letterheads and templates. ABN Number VSP Number Health Identifier Evaluation only Site ID Number Licence Code Your practice s Australian Business Number. Your practice s Vaccine Service Provider Number. Your practice s HPI-O number, obtained from the Department of Human Services, and used to access My Health Record online. Tick if the licence you have received from Best Practice Software is for a limited-days evaluation of the software. Bp Premier Support or Sales will advise you whether to tick this checkbox during your initial installation. Bp Premier site code issued from Best Practice Software. If you do not have a site ID, contact Best Practice Software on and press 2 to talk to Sales. Licence key issued to you by Best Practice Software. 6 1. Click Add next to the Practice Locations list. The Location details screen appears. 2. Complete the details for this location and click Save: Field Rural / Remote area LSP Number Exempt from Diagnostic Imaging rules Minor ID Description Determines which Medicare incentive is added to bulk-billed invoices for pensioners, health care card recipients, and children. If unticked, incentive '10990' is used; if ticked, '10991' is used. address for this practice location. Location Specific Practice Number, for practices that provide diagnostic imaging services or radiation oncology services. Tick if your practice is exempt, or has been granted an exemption, from the Department of Health rules regarding Diagnostic Imaging Service equipment. 7 Field Medicare Certificate Description The address associated with the Medicare PKI certificate for this location. Click Attach HeSA Certificates to link a Medicare PKI certificate to this location. The address associated with the certificate is assigned when you register with Medicare, but you can view the address in the Medicare PKI Certificate Manager. You only need to populate this field for practices with multiple locations. Delete a location: 1. Select the location on the Practice details screen and click Delete. A prompt will appear if any users have provider numbers linked to that location. 2. If the location is deleted, users with linked provider numbers will still be visible, but the provider number linked to that location will no longer be visible. Restore a deleted location: 1. Select the Show Deleted checkbox at the bottom of the screen to view deleted locations. 2. Select the deleted location in the Practice locations list field and click Restore. 3. Secure messaging You will need to install and configure a secure messaging provider, such as Argus or Fetch, to send ereferrals and pathology and radiology requests. Install messaging software If you did not install Argus the first time you installed Bp Premier, rerun the installer, click Argus Connect from the 3rd Party Products screen, and select the Secure Messaging checkbox. 8 If you are using Fetch or another messaging software, obtain the installation files from the software vendor and run the installer on the Bp Premier server. Your messaging vendor will supply the incoming and outgoing Windows folders used by the software to retrieve and send files. In Windows, share the outgoing folder with Everyone and grant Full control permissions to enable the software to retrieve files from this folder. Your IT resource can assist with sharing folders. 9 Add the messaging provider 1. Select Setup > Configuration > Messaging. Click Add to open the Messaging provider screen. 2. Your messaging vendor can supply most of the values to populate this screen: If the vendor is HealthLink, the Provider name must be HealthLink, with the same capitals. Tick Use the same path for all computers. The same incoming and outgoing paths will be used on all Bp Premier workstations that use secure messaging. Tick Use as default messaging provider to automatically select this vendor in screens that involve messaging. The Path for outgoing messages must be in a UNC format, for example, \\server- \messagingprovider\out'. 3. If you are setting up HealthLink, click Load Config and browse for the HealthLink configuration file hms_config.xml. 4. Click Save to save the messaging provider configuration. 10 Set HL7 file defaults 1. Select Setup > Configuration > General. 2. Change the following HL7 settings if required by your referrals or laboratory: Default HL7 export format Use Argus format Include a formatted text OBX in REF messages. For more information on setting up messaging providers, search for set up HealthLink or set up Argus on the Bp Premier Knowledge Base. 4. Pathology and radiology The investigation and request workflow requires you to set up multiple components: 1. Set up a secure messaging provider. 2. Add pathology and radiology laboratory details. 3. Set up results import and acknowledgement. 4. Enable Bp Service to monitor incoming results. 5. Set up investigation request templates. 6. Set pathology and radiology request workflow preferences for each provider. To set up a secure messaging provider, review 3. Secure messaging on page 7 Add laboratories as contacts 1. Select View > Contacts. The Contacts screen will appear. 2. Click Add new. The Contact details screen will appear. Page 10 Bp Premier Clinical Setup Guide
11 3. Select the Company/Institution radio button. 4. Select 'Pathology provider' or 'Imaging provider' from the Category drop down list. (If this option does not appear in the list, you will have to recreate the contact category.) 5. Complete the rest of the fields for the laboratory. 6. If the Messaging provider you select has Accept CDA referrals enabled, you will be able to send CDA-formatted ereferrals to this contact. 7. Click Save. Bp Premier Clinical Setup Guide Page 11
12 Set up results import 1. Select Setup > Configuration from the main Bp Premier screen and select the Results Import tab. 2. Set the following fields: Field Import investigation reports on this computer Don't allow unallocated reports into Inbox Description Tick this checkbox. This would usually be the Bp Premier server only. Note: Don't set up two computers to import results. Results import may conflict between the two machines. Tick if you do not want to forward results that have not been allocated to a patient to a provider's inbox. If ticked, someone in the practice should be allocated the task of checking for unallocated results on a daily basis from the View > Incoming reports screen. Page 12 Bp Premier Clinical Setup Guide
13 Field Add header to incoming letters Description Referrals and other letters received electronically usually have a patient's name and address and the top and don't need to show the header visible on results. Tick if you want to see the results header as well as the address shown on the letter. Set default storage locations 3. Click the Set storage locations button. The Report Location screen will be displayed. 4. By default, each result type stores in Correspondence In. You can change the store location to Investigations. 5. If you need to add a new report location for your practice (for example, a unique type of referral), click Add from the Report Location screen. Type the Report name and select a default store location from the Store in field. Click Save to return to the Report location screen. The Report name will be matched against the content of the report and identified accordingly. When processing reports from the provider Inbox, the Inbox indicates where the report will be saved in the patient s file: Correspondence In or Investigations. 6. Click Close to return to the Configuration screen. Add download paths 7. Click Add next to the Report file search paths. A file explorer will open. 8. Browse to and select the folder to download the report. Click Ok. 9. Repeat steps 7 8 for each path where results are being downloaded to. Bp Premier Clinical Setup Guide Page 13

Complete the rest of the fields in the Results Import tab: Field Keep HL7 and PIT files after import Report file extensions Acknowledgements Number of months to display in Investigation reports list Number of months to display in Inbox follow up list Enable polling for insurance reports (UHG) Description Only tick this box if advised to do so by General Products Support to diagnose an issue with importing reports. Most labs will use the standard extension for their results file. However, additional file extensions can be added here if required for a lab. Some labs require that an acknowledgement is sent back to the lab to confirm that the result has been received. Defaults to 1 month, but you can extend the default month range. Defaults to 12 months, but you can extend the default month range. medebridge is a UHG proprietary system that communicates between Bp Premier and UHG to electronically complete insurance reports. Tick this checkbox to enable UHG functionality and enter the login password. Include BDI documents in the Incoming reports list Tick this checkbox to view documents that are scanned or imported via the Bulk Document Import utility (BDI ) in the Incoming reports screen. 11. Click Save to save your results import configuration. Enable Bp Service to monitor incoming results 12. Select the Bp Service tab of the Configuration screen. 13. Tick the checkbox Use BP Service for importing results on this machine. 14. Click Save. Set up results acknowledgement Incoming results in HL7 format can be acknowledged. 1. Select Setup > Configuration > Results import from the main Bp Premier screen. 2. Next to the Acknowledgements list, click Add. The Send acknowledgements screen will appear. 15 3. Enter the Facility name. This needs to be exactly the name stored in the file by the sending facility. 4. Enter the Acknowledgement path. This is the folder in which the acknowledgement files will be saved and from which the lab or sending facility can collect the acknowledgements. 5. Click Save to update this setting in the system. 6. Click Cancel to cancel any changes and return to the Results import screen. After a HL7 result has been imported intobp Premier, the system will check if the facility name in the file corresponds with the name listed on the Sent acknowledgements screen. If Bp Premier finds a match, an acknowledgement file will be created in the path indicated. Set up request form layouts Request forms for pathology are dependent on the laboratory requirements, and can be set per individual provider. 1. Select Setup > Preferences > Pathology from the main Bp Premier screen. The User preferences screen will be displayed for pathology. 2. In the Laboratory Details section at the bottom right, select the laboratory that you want to set up the layout for and click Edit. This will display the Laboratory code screen. 3. Complete the fields in this screen: Field Laboratory code Preferred laboratory Description Enter a code if the laboratory requires a code printed on the request. Tick this box if this laboratory is the user s preferred laboratory. When the user creates a pathology request from the patient record, the request will default to this pathology lab. 16 Field Use plain paper Paper has specimen labels Description Tick this box if the laboratory does not provide pre-printed stationery. Tick this box if the pre-printed stationery has a section for specimen labels. When this box is ticked, the Move duplicate section down and Move labels down fields become available. Enter a measurement in mm to adjust the position from the top left that the duplicate and label sections are printed. 4. Click Save. Pathology requests can now be printed for this laboratory. Set provider preferences Workflow preferences for radiology and pathology can be set per provider. 1. Select Setup > Preferences. 2. Select the Pathology tab to create custom tests, test lists, favourite test lists, and default billing for pathology. Click Save. 3. Select the Imaging tab to create custom tests and the preferred imaging provider. Click Save. For more information on the reminder workflow in Bp Premier, search for pathology on the Bp Premier Knowledge Base. 5. erx erx must be set up if your providers will be prescribing from Bp Premier. erx software is installed when you install Bp Premier. You do not need to install any additional software. Run erx Setup You will need your Medicare site certificate and passphrase, and the Entity IDs for each prescribing provider. 1. Browse to the folder C:\Program Files\Best Practice Software\BPS and double-click on the file erx Setup.exe, or browse to Start > Best Practice Software > erx setup. 2. Log in using your Bp Premier username and password. The erx Setup screen will appear. Page 16 Bp Premier Clinical Setup Guide
17 3. Enter the Server name of the Bp Premier server machine. You should not need to change the Port. 4. Insert the Medicare Site Certificate CD into the Bp Premier server. 5. Click Setup Users to display the erx users screen. This screen lists all providers set up in Bp Premier. Bp Premier Clinical Setup Guide Page 17
18 6. For each provider registered for erx: 1. Select the provider's name in the list and click Change. A second erx setup screen will appear. 2. Enter the provider's Entity ID. 3. Click on the... button next to Enter the certificate file to use field and browse to the Medicare Site certificate CD. 4. Select the file called FAC_Sign.p12 and click OK. 5. In the Enter the certificate file s password, enter the PIC password for the Medicare certificates. 6. Click Save. Do the same for each prescribing provider with an Entity ID. 7. Click the Close button to return to the erx setup screen. 8. Click Import Certificates on the erx setup screen. The system will automatically import the erx certificates. You may receive a Windows Security Warning about the validity of the erx Script Exchange certificate. Click Yes to continue. 9. Click Close to close the erx setup screen. Start erx Windows services Perform this step on the Bp Premier server only. If you are not sure how to configure Windows services, your IT support can assist. 1. In Windows, navigate to the Services screen. The path to this screen will depend on your version of Windows: On Windows 7 and earlier, click the Start button. In the search field, type Services and press enter. On Windows 8 and later, click the Windows logo in the bottom left. Click the down arrow to open applications and navigate to Control Panel > Administrative Tools > Services. The Services screen will appear. Page 18 Bp Premier Clinical Setup Guide
19 2. Scroll down the list and look for ERx Standard Adapter Service and ERx Standard Adapter Store & Forward Service. 3. Set the Startup type of both services to Automatic and start the services. Set prescribing preferences Each provider can set their own preferences for the prescription workflow, such as automatically generating repeats for regular Rx, and adding patient notes when scripts are generated or ceased. Select Setup > Preferences and select the Prescribing tab. Tick the workflow options you prefer and click Save & Close. 6. My Health Record Bp Premier integrates with My Health Record online. You can upload and download patient details and documents from the My Health Record gateway. You will need: Your practice s HPI-O Each provider s HPI-I Your Medicare site certificate and passcode Your Medicare NASH certificate. Bp Premier Clinical Setup Guide Page 19
20 Record My Health Record numbers 1. Select Setup > Practice Details and click Change. The Practice Details screen will appear. 2. Check that the correct practice name is displayed at the top of the screen. 3. Enter the HPI-O into Health Identifier and click Save. Click Close to close the screen. 4. Select Setup > Users. 5. Select a user and click Edit. 6. Enter the HPI-I into the Health Identifier field and click Save. 7. Repeat steps 5-6 for each provider with an HPI-I number. Import site certificates 1. Insert your Medicare Site Certificate CD and run C:\Program Files\Best Practice Software\BPS\BPSupport\BPCertificateManager.exe. Page 20 Bp Premier Clinical Setup Guide
21 2. Log in to certificate manager using your Bp Premier username and password. The BP ehealth Certificate Import Tool screen will appear. 3. Click on the File Explorer icon to the right of Path to certificate and browse to the Medicare Site certificate CD you inserted. 4. Select the Practice Certificate (FAC_Sign.p12) and click Open. 5. Type your Certificate Passphrase in. This is the PIC code provided to the practice by Medicare with your Medicare Site certificate. 6. Select the Health Identifier radio button and click Import. If the import was successful, a message The ehealth certificate has been successfully imported will be displayed. Import NASH certificate 1. Insert the e-health NASH certificate CD into the server and run C:\Program Files\Best Practice Software\BPS\BPSupport\BPCertificateManager.exe again. 2. Log in to certificate manager using your Bp Premier username and password. The BP ehealth Certificate Import Tool screen will appear. 3. Click on the File Explorer icon to the right of Path to certificate and browse to the NASH certificate CD you inserted. 4. Select the Practice Certificate (FAC_Sign.p12) and click Open. 5. Type your Certificate Passphrase in. This is the PIC code provided to the practice by Medicare with your Medicare Site certificate. 6. Select the PCEHR radio button and click Import. If the import was successful, a success message will be displayed. Set permissions to access and register For users who need to be able to upload and document from My Health Record: 1. Select Setup > Users. Select the user you want to update and click Edit. 2. Click Permissions. Set the following permissions to Allowed depending on the provider s requirements: If the user must be able to upload or download a patient's My Health Record information, set My Health Record Access. If the user is only permitted to assist patients with their My Health Record registration process, set My Health Record Registration. Bp Premier Clinical Setup Guide Page 21
22 3. Click Save. Check connection Log out and back in to Bp Premier as a provider and check that My Health Record is now a menu option in the main screen. For more information on the reminder workflow in Bp Premier, search for my health record on the Bp Premier Knowledge Base. 7. Clinical reminders The Lava version of Bp Premier only sends appointment reminders by SMS, not clinical reminders for regular scheduled events like cervical screening or asthma management. However, you can create as many clinical reminder reasons as you need, and set up workflow defaults for how your practice manages clinical reminders and reminder mailouts. 23 Add reminder reasons 1. Select Setup > Configuration > Reminders. Click Add next to the Reminder reason list. 2. Enter the reminder Reason. 3. Select the Default interval for this reminder in Weeks, Months, or Years and click Save to add the new reminder to the list. You can change from the default interval when you assign a reminder. You can now assign this reminder to patients from the patient record. Bp Premier Clinical Setup Guide Page 23
24 Set reminder workflow defaults 1. Select Setup > Configuration > Reminders. 2. Set the fields in the Reminders tab: Fields to be printed on Reminder list Sets the fields that are displayed on the Reminders screen (used to send reminders) and the fields printed on reminder lists from this screen. Send as Sets the default method selecting when sending reminders. You can change the default value at send time. Check 'Mark as sent' box as default Automatically marks reminders as sent when reminders move from the Reminders screen to the Sent Reminders screen. Default reminder list interval Sets the default interval to search for when selecting patients with reminders from the Reminders screen. Display Actions/Reminders less than n months old Period (in months) to display Actions and Reminders for in the Reminders screen and the patient record. Reminders older than this period will not be shown. 3. Click Save to save the defaults. Set up reminder letter templates Bp Premier installs with a system Reminder Letter template, which you can use or copy and modify to suit your practice. To open the reminder letter template: 1. Press F4 to open the word processor. 2. Select Templates > Edit template. 3. Scroll down to Reminder letter and click Open. Page 24 Bp Premier Clinical Setup Guide
25 4. Edit the template using the text formatting toolbar along the top. Double-click a field on the left to insert that field into the template. The field will be replaced with the database value (such as the practice name) when the letter is printed or saved to file. 5. Click the disk icon or select File > Save as and save the new reminder template with a descriptive name. Tick Available to all users to make this reminder template available to all Bp Premier users. Otherwise, the template will only be available for use by the logged-in user. Bp Premier Clinical Setup Guide Page 25
26 6. Click Save. For more information on the reminder workflow in Bp Premier, search for reminders on the Bp Premier Knowledge Base. For more information on using the word processor, search for word processor. 8. Bulk document import tool The Bulk Document Import (BDI) tool imports multiple clinical documents and images from file, scanner, or camera, and associates the files with a provider and patient record. Users with access to this tool can set source and import preferences that are specific to their user account. To set BDI utility: 1. On Windows XP or Windows 7, go to Windows Start > All Programs > Best Practice Software > Best Practice > Bulk Document Import On Windows 8 and higher, click on the Windows flag icon in the bottom right, select the down arrow to view apps, and click on Bulk Document Import under Best Practice Software. 27 2. Enter your Bp Premier username and password into the login prompt. The Bulk Document Import screen will appear. 3. Set the following preferences: Bp Premier Clinical Setup Guide Page 27
28 Field Mode Source Interface Description Depends on the type of scanner: 'Twain' or 'WIA'. Consult your scanner documentation if you are unsure of which type is your scanner. Scanner to use as the source for scanned images. The scanner can be connected to the computer you are running BDI from, or any scanner connected to the same network. The 'Simple interface' option uses Bp Premier' interface for scanning documents. The 'Manufacturer's interface' option uses the manufacturer's scanning software, which may feature more options. Format File size The file format in which to save scanned documents. You can set the maximum size scanned image file that Bp Premier will accept, and a warning file size. If the file size is within the maximum file size limit but exceeds the warning file size, a Reduce size button appears. The user can reduce the file size using the Image resizing settings. Scanned files over the maximum file size can not be saved. Default settings are 6MB for a maximum file size and 2MB for a warning file size. Image resizing These settings are used in conjunction with the File size settings. The higher the Image quality value, the larger the file size. If the document can be reduced in width and height without sacrificing image quality, use the Image size dimensions in pixels to rescale the image. Default Save Location Hide assigned Documents Indicates whether documents are saved to the provider's Inbox or the patient file by default. Specifies whether already allocated documents are omitted from the Imported document list (for inbox) in the BDI utility. Ticking this option can make managing documents easier during an import, because the Imported document list will only show documents you haven't allocated yet. Default import type on this machine Users to display Specifies the default file type to look for when browsing for files to import into the BDI utility. Specifies which users are available in the User drop down list when allocating an imported or scanned document. 4. Click OK. 29 9. Clinical lists Bp Premier provides a number of standard lists that can be modified to suit your practice. 1. Go to Setup > Configuration > Lists. 2. Add and modify list items for the following lists: Reason for ceasing medication Supplies standard text to select when a patient ceases medication. Document type Standard set of clinical document types that can be sent from the word processor and imported into the patient record. 3. Scroll down and view the Contact categories available. If your practice deals with specialists or companies that do not have a contact category, click Add to add a new category. 4. Click Save. 10. Immunisations You can record batch numbers and expiry dates for vaccines held at your practice, and the default Influenza vaccination brand. 1. Select Setup > Immunisation batches. The Immunisation batch numbers screen will appear. 2. Select a Vaccine from the left that your practice stocks. Click Add to add a batch number. The batch number and logged-in user will be added to the Batches in stock list. 3. If your practice administers an Influenza vaccination, select a Default brand and Default batch number from the batch numbers recorded. 4. 30 isations section of the patient record. 11. Care plans If your practice engages with enhanced primary care for chronic disease management, you can set up care plan goals, tasks, and templates for providers. You can also import care plan templates from another Bp Premier site. 1. Select Setup > Configuration > Care plans. 2. Click Add next to the Care plan goals or Care plan tasks list to add new goals and tasks. 3. To create a new template, click Setup Care Plan templates. 4. From the Care Plan templates screen, click Add to create a new template. Click Import to open a file explorer and import an existing template. 5. From the Care Plan template screen, add Problems, Goals, and Tasks. 6. Click Save to save the template. Drug sheets Drug sheets, or medication schedules, are summaries of a patient's medications printed from the Page 30 Bp Premier Clinical Setup Guide
31 patient record. A drug sheet layout controls the look of a printed drug sheet. 1. From the main screen, select Setup > Drug Sheets. The Setup drug sheets screen will appear. 2. Click Add to create a new drug sheet layout, or select a layout from the list and click Clone to copy an existing layout. The Drug sheet screen will appear. 3. Enter the Name of sheet and Title of sheet. The name is for referencing in Bp Premier. The title is printed as the heading when the drug sheet is printed. 4. Click Load defaults to load default settings into the margin and position fields. 5. Select the Drugs per page and the Days per page to be printed. 6. Set the Times per day to be printed under each day. 7. Adjust the default margin and position settings for the drug fields shown. All measurements are in mm from the top left corner of the page. 8. 32 Include generic name prints the generic name of the drug. Landscape orientation prints the drug sheet in landscape. This is useful for drug sheets that span a longer time. Print PRN items separately prints a separate page for items marked as 'PRN'. Draw lines prints grid lines on the drug sheet for readability. 9. Click Save.
