Critical/unstable patients who are deteriorating (Sepsis, acute hypotension, hypovolemia, hemodynamically unstable) Labs ordered less than every 4 hours (i.e.,q2 Na) Apnea testing Vent weaning Rapid Response Team/Code Blue

6 Epoc Blood Gas Analysis System The epoc blood gas analysis system is a portable blood analyzer composed of THREE items: Host: Communicates via bluetooth with epoc reader. It calculates analytical values sent from reader. Similar to a hand-held PDA. Reader: Battery powered device with internal barcode scanner The readers accepts test cards, measures electrical signals from test card sensors, and transmits test results via Bluetooth to epoc host. Test Card: Single use, credit card sized card with port for blood sample entry. It contains an array of sensors and calibration fluid in a sealed reservoir. Note: Wireless Printer is an optional item.

10 System Overview Turn on both host and reader Log in Host and reader sync (happens automatically) Reader runs quick internal QC (approx. 15 seconds, happens automatically) Insert test card Three minute test card calibration Enter patient ID, sample type, and select ordered tests Inject sample when prompted Sample Analysis (happens automatically after sample injected) Results displayed in 40 seconds under measured and calculated tabs Tap X to send/sync results and to exit out of test screen Log out Return system to epoc charging station

13 System Operation Initial Screen Initial Screen EPOC automatically runs quick internal QC (approx. 15 seconds) Once configuration is complete, it will prompt you to insert test card.

14 System Operation Test Card **Do not touch the sensor strip!! Open package only when ready to use test card. This side up to insert Test card should go directly from package into reader. Test card is single use only. Reader reads barcode on test card upon insertion.

15 System Operation Test Card Insertion Insert the test card in one steady motion until resistance is felt and the blood drop icon on the reader starts blinking green. If the test card is not in the proper position or the barcode is not read, you must reinsert the test card. Test card will be rejected if card is expired.

16 System Operation Test Card Calibration A 3 minute calibration will begin once test card is properly inserted. Calibration status counts down in seconds.

21 Pre Analysis Data Entry 1 st icon Patient ID is REQUIRED on the 1 st icon: Patient ID = Encounter (HAR) number (you MUST scan Patient ID)

25 Sample Analysis Once calibration is complete the device will prompt you to inject the sample. Note: You have 7.5 minutes to inject sample. If the sample is not injected within 7.5 minutes, you must dispose of test card and start process over with a new test card.

26 Sample Analysis Note: If sample is not analyzed immediately after collection, mix by gentle inversion for at least 15 seconds To inject the sample: Expel all air from syringe Place sample syringe in card port Give a quarter turn Inject the sample slow and steady The device will beep when sample volume is adequate Remove the sample syringe Note: The reader should remain flat and steady during analysis

34 Post Analysis Data Entry 2 nd icon Optional items on the 2nd icon: Reject Test Box: Check box if you do not want the sample reported due to a sample integrity issue. Comments: only required if reject test is checked to explain why test was rejected. OR: You may manually fill out the Point of Care Testing Exceptions Form and send to lab, attention: POCT LeAnn Cowan.

35 Test Conclusion Exiting Patient Sample If patient id is scanned in after test results are complete, you must tap the save icon to save the information. The results will then be displayed. After your entries are saved, click on the Red X. Results will automatically sync to the LIS/Epic system. Note: You cannot deselect a test after the sample has resulted.

36 Test Conclusion Subsequent Tests and Logging Out Tap here to log out Double Tap here to run another sample or Tap and Hold and then select Run Blood Test To recall results, tap the reader icon and select view test

37 Storage Wipe down the EPOC with a Sani-wipe upon leaving the patient room. Return your EPOC Device to the charging area. Plug in all devices when not in use. ONLY use power cords labeled EPOC. Using the incorrect power cord could destroy the EPOC.

38 Troubleshooting Insert a new test card. Turn Reader and Host each off then back on. Perform a soft reboot by holding down the power button for 5 seconds. Screen will go blank and then reappear. If the issue is not resolved have superuser perform a hard reboot. Refer to the following for further troubleshooting: Epoc Procedure or Epoc System Manual Super Users Alere Technical Support
