1 Guidance for Teacher Access to CODE To access CODE, please visit Then you will enter your username and password that should have been sent to you in an from NIET. In most cases, your username will be your license number without the leading zeroes. If you forgot your password, please click Forgot Password? and NIET will send you an with your login credentials. If you forgot your username, please contact your school or district administrator to verify your username.

2 Guidance for Teacher Access to CODE If you are unable to retrieve your password via , there are two potential causes: 1. The from NIET was sent to your spam folder. Be sure to check your spam folder if you do not receive an from NIET in a timely manner. 2. Your address is incorrect in CODE. If you do not receive an from NIET and it is not in your spam folder, contact your school or district administrator to have them correct your address in CODE.

3 Main Page After successfully logging into CODE, your page will show your data for the year and the year.

4 Scores from Previous Years To access scores from previous years, select the appropriate year from the drop-down menu. When you select a year from the drop-down menu, you will see your scores from that year, as well as the previous year. Ex) If you select you will see your scores from and If you select you will see your scores from and Choose from the drop-down

5 Accessing Observation Scores To access your observation forms with indicator level scores (i.e. more than just your average observation score) you need to select the appropriate year from the drop-down menu. If you want to see your observation scores for the year, select that year from the drop-down menu, look under Submitted Observations and click Print. Choose Print to view complete observation scores

6 Accessing Observation Scores After you click print next to the observation, it will open a separate window with your observation form from that observation. Please make sure your pop-up blocker is enabled. If your browser is set to block pop-ups, you will not be able to open the form.

7 Special Notes: Growth Measure If you have a growth score in the Individual Growth Score and Growth Score fields, the Growth Measure you see listed is only a back-up measure. If you do not have an Individual Growth Score, your Growth Score corresponds to the selected Growth Measure. If you do have an Individual Growth Score, your TVAAS Evaluation Composite score is used for evaluative purposes. Since this teacher has an individual growth score, the growth measure School-Wide: Composite is NOT the score that was used. That is the back-up measure that would have been used if the educator had not generated an individual growth score.

8 Special Notes: PYE Since the achievement score has not been entered, the Level of Effectiveness is calculating as a PYE. PYE stands for partial year exemption. Educators who did not meet the 120 day requirement or who are missing a score for one of the measures would be calculated as a PYE. In this example, the achievement score is missing, so the educator s Level of Effectiveness is showing a PYE. When the evaluator enters the achievement score, CODE will re-calculate the Level of Effectiveness and it will no longer show PYE. NOTE: If you have scores for all components and still show a PYE, that means your school or district administrator designated you as a PYE for the year. If that is incorrect, please contact your school or district administrator.

9 Special Notes: Average Observation Score This average observation score was not calculated based on observation submitted in CODE. A district administrator manually calculated and entered it. If your average observation score has ^ next to it, that means that the average observation score was not calculated based on observations submitted in CODE. This should only be the case in the event that your evaluator did conduct an observation but did not enter the observation into CODE before the deadline.
