HMG WVP Dashboard Learn about features on the HMG WVP dashboard and how these can be used to enhance implementation 3. Questions Elicit responses from sites regarding planning, implementation and vision, and give them an opportunity to ask questions

3 Landing Page Access the landing page via the HMG portal or by typing in wellvisitplanner.org Click Register Now! to create your account

4 Creating Your Account Enter your full name (whichever staff member is creating the account) Make sure the address is valid and will be checked regularly. Please only create one account per site, as it is linked with the unique URL

7 Registration Overview Registration Landing Page The first time you log in, you will be directed to register your site A pop up box will appear, which will then walk you through the three steps of the registration process

8 Registration Overview Step 1: Register Your Affiliate Site First, create a site name (up to 25 characters) which will attach to the end of the main URL Example of a good URL: wellvisitplanner.org/schelpmegrow Example of bad URL: wellvisitplanner.org/southcarolina1hel pmegrowsurvey Next, click on choose file to add your affiliate logo

9 Registration Overview Step 2: Link Questionnaires to the Well Visit Planner This step allows you to add in information about any additional screenings or referrals that you would like to link with the WVP Once you ve filled out the tool name and URL (or if you don t have anything to add), you can move on to step 3

10 Registration Overview Step 3: Finalize Your Registration Your registration is now complete! In the future, when you log in you will be immediately directed to the dashboard

11 Dashboard Current Features See your logo and an alert letting you know that you ve completed the toolkit registration process Coming Soon Engagement Tracker see how many families have completed the tool Data on what priorities your families have selected on the WVP Possible additional features

12 Site Profile Changing the address and name associated with the account are OK. You may not change the site name after families have completed the survey Uploading a new logo replaces the existing one Make sure you save any changes
