4 You will need to designate a Provider Administrator for your facility. This person will need to add and manage all other users of the Provider Portal. The Provider Administrator should visit and click on the link shown below labeled please click here to send us an . Complete the form and click Send. You should expect an from adminregistration@kepro.com by late December with your registration code

5 The Provider Portal is accessible at Click on the register here link under the LogIn section. Enter your facility NPI and the registration code that you received via from KEPRO.

6 Complete your Account Information by creating a username, password and security question and answer. This will be used if you need to reset your password in the future. Complete the Contact Information section. Click Next. Review the Terms of Use, click the acknowledgement check box, then click Continue.

7 Click on SETUP on the left navigation pane. You will see the Provider Groups and Users you have access to manage shown here in respective tabs. Under the appropriate Provider Group, click Add New User. Create a username and password and complete the contact information section. Click Create.

8 On the Manage Users tab, you may select a user and edit a user s information or delete the user. You can also assign the user to different provider groups that you manage and change his/her role. Each provider group that the user has access to will be listed under their name along with the access role. The standard role for users should be Staff User. Staff User Staff User Staff User

9 The Provider Portal is accessible at An Administrator at your facility will provide you with a username and password. You may reset your password at any time by clicking the Forgot Password link. In which city were you born?

10 When you log in, you will see the Home Page shown below. There are several ways to search for consumers and cases in the Provider Portal. You will most commonly want to use the CREATE CASE tab when creating a new request. You will also want to review the status of your submitted cases, which can be done from the home page or by searching the CONSUMERS or CASES tabs. Search all Cases Create a new request (Case) Search all Consumers Manage users (Administrators) Messages from KEPRO about submitted cases Training materials & other resources

11 To add a new case, click on CREATE CASE from the left navigation pane. Search for the consumer using last name and date of birth. If the correct consumer match is found, you can click on the button under SELECT MEMBER to proceed with the case. If a consumer match is not found, you will be able to add the consumer to the system (next slide).

12 When no consumer match is found, click ADD TEMPORARY CONSUMER to add the consumer to the system. Enter the consumer details. For Contract and Plan, select FL PASRR from the drop-down menus. Then, click CREATE.

13 After selecting or adding the consumer, the next section CASE PARAMETERS is defaulted to the FL PASRR values. Click CREATE CASE. The Facility will be populated to your Facility. You may add a legal representative, search for the attending physician and diagnoses code(s), and add documents and notes. Note: These are not required for a Level I Review. The most important sections of the case to complete for a Level I PASRR and a Residential Review are the REQUEST DETAIL section and the QUESTIONNAIRES. More info on these sections in the next slide. For Level II PASRR requests, it will also be important to upload the required documentation related to the request.

14 In the REQUEST DETAIL section, select the appropriate assessment type. This selection will determine which questionnaire you will complete. In the QUESTIONNAIRES section, you will complete the required questionnaire (Level I or Resident Review) by answering a series of questions and inputting information where needed. When finished, click Mark as Complete. PASRR Level I

15 After completing the electronic Level I or Resident Review questionnaire, you have completed all of the case details. Next, click Submit for your results. You will be redirected to the Case Summary page with all information submitted with the case as well as the status and outcome of the request. You will also be able to view and print letters and the entire case summary from this screen.

16 There are several ways to find an existing case within the Provider Portal. Search by specific case number View recent cases from Home page Search all Cases submitted by your facility Search for the consumer and view all relevant Cases Read a message about a specific case and access case from there

17 You can view the case summary for any case that has been submitted by your facility. Messages from KEPRO about the case will be accessible from the MESSAGE CENTER on the left navigation pane as well as directly within the case in the Messages tab. If the case is not yet locked, you can add additional documents within the Case Overview section and add notes within the Notes section.

18 Within the Provider Portal, a separate case is required for each review. The same basic case structure and process is used for each type of review. Here are some tips for the differences between how the different types of Reviews are processed. PASRR Level I Resident Review Questionnaire completed when submitting the case. KEPRO will complete PASRR Level II Questionnaire when completing the review. Required Documents None None Informed Consent, 3008, H&P, MAR, Nursing Notes, Psych, MDS Outcome Determination Completed by the system based on Questionnaire results Completed by the system based on Questionnaire results Completed by KEPRO reviewer during Level II review Next Steps Positive Determination: Submit a Level II Case Request Negative Determination: Consumer may be admitted to Nursing Facility A Level II Review will be required Recommendation determined by KEPRO based on Level II review. May result in admission to Nursing Facility or arrangement of other specialized services.
