1 Annual Standardized Procedure Report The Annual report should include any practices/procedures beyond basic education being performed by all nurses providing care in your facility/agency, in accordance with the Alabama Board of Nursing Administrative Code Rule 610-X-6-.12(3). The deadline for submission of the Annual Report of Standardized Procedures reflecting practice in is April 30, Location: To access the Report, go to select the Nursing Practice tab, then select Standardized Procedure Information and Application, followed by Standardized Procedures Online Report. You may also access the page directly at STEPS DIRECTIONS 1 CLICK on the Standardized Procedures link from the menu on the lefthand side of the page. 2 Log onto the site using your ADPH number and P@ssw0rd. (capital P, sign, lower case ssw, 0 zero, and lower case rd) Note: Your password can be changed once you have logged into the site. 3 Verify your facility information 1. The site will open displaying your facility information. If your facility is part of a corporation, then the corporation information will be displayed. Note: If you need to add or change your corporation information select the check box, add or edit the information and save. The current sub- facilities information will default to the subfacilities tab. 2. The corporation/facility information is divided into 3 sections, not 1
2 including the section to change your password. 3. To change any of the details on this page, click the Edit button that corresponds to the section you would like to change. The field will open, allowing you to make any necessary changes. 4. After making your changes, click the Save button to save the new information. 4 Verify your procedures 1. The second tab on the page Reported Procedures lists all procedures that have been reported to the Alabama Board of Nursing. From this tab, you are able to add, delete, modify and export your procedures. Add procedures 1. To add a new procedure, click the Add New Procedure link. 2. Fill in all the open fields and click the Update link. 2
3 Modify procedures 1. To modify a procedure, click the icon that looks like a piece of paper. 2. This will open the fields that can be modified. 3. Make your changes and click the Update link to save. 3
4 Delete procedures 1. To delete a procedure, click the icon that looks like this in front of the procedure you wish to delete. Export your procedures 1. You can export your procedures to Excel or in PDF format by selecting the format you wish and clicking the Export button. 5 Verify your Sub-Facilities 1. If you are a corporation with many facilities, the third tab on the page Sub-Facilities lists all your facilities. Non-corporations will show a blank grid with no facilities listed. From this tab, you are able to add, delete, modify and export your facilities. Add a new facility to your corporation 1. To add a new facility, click the drop down menu arrow to show a listing of all ADPH numbers that ABN has received from the Department of Public Health. 2. Select the facility and click the Add button. Add a new facility not listed in the drop down menu 1. To add a new facility that is not listed in the drop down menu, click Add New ADPH # from the grid title. This will open a field to add a new ADPH number to your facilities. You must have the new ADPH number from the Department of Public Health. 2. Fill all the empty fields and click the Update button to add the new 4
5 ADPH number to your listing. Modify a facility 1. To modify a facility, click the icon that looks like a piece of paper. 2. This will open the fields that can be modified. 3. Make your changes and click the Update link to save. 5
6 Delete a facility 1. To delete a facility from your listing, click the icon that looks like this in front of the facility you wish to delete. Export your facility listing 1. You can export your facility listing to Excel or in PDF format by selecting the format you wish and clicking the Export button. 6 Final Step Attest to the accuracy Beginning on January 2 nd of each year, the affirmation checkbox will be shown in your facility profile. This checkbox must be selected before April of the current year. Once checked, you may still log onto your facility profile throughout the year to keep it up-to-date. NOTE Other notes about your profile page: Comments/Questions The Comments/Questions tab enables you to send an message directly to Joyce Jeter, Nurse Consultant for Continuing Education and Practice. This method is a fast and efficient means of getting a response quickly. Your Journal Any changes you make to your profile are recorded in your own journal. The journal is located under the green tab in the upper righthand corner of the page. 6
7 Your Password You can change your password at any time from the Facility Information tab. If you have forgotten your password, you may reset it using the link on the login screen. 7
