1 2016 Cystic Fibrosis Awareness Month Art Contest: Win a Trip to Hawaii with Mauli Ola Official Rules Eligibility 1. You MUST be a patient with cystic fibrosis in order to be eligible. 2. The winner MUST prove his or her eligibility by providing proper documentation. A Mauli Ola Foundation ( MOF ) representative will contact you with specific details if you win the trip to Hawaii. 3. You may not enter if you are under the age of No purchase is necessary to participate in the contest. The contest is open to residents of the 50 United States, the District of Columbia, and United States Territories (collectively, the Territory ). 5. This contest is void outside of the Territory and where prohibited. 6. Employees of the Mauli Ola Foundation or any of its affiliates and their immediate family members are not eligible to win. Judging and Winner A panel of judges from the Mauli Ola Foundation Corporate Team will review entries. Voting will take place July 18, 2016 July 22, Finalist will be selected and notified by phone and on Wednesday July 27 th, Selected participants artwork will be featured at the North Shore Benefit in December Second place runner up will receive a certificate of recognition, Mauli Ola gear, and have the opportunity to substitute for the first place winner if he/she and guest cannot make scheduled dates. Third place runner up will receive a certificate of recognition, Mauli Ola gear, and have the opportunity to substitute for the first place winner if the first and second place winner are not able to make the scheduled dates. Winner Prize Round- trip airfare for two from home airport to Oahu (1 for the patient, 1 for a parent). Lodging at North Shore on Oahu. 1 rental car for duration of stay. Arrangements for air travel, lodging, and rental car will be made by MOF on behalf of the winner and winner s guest. Winner must agree to give a speech at our annual North Shore Benefit Concert in December Winner will also attend our Hale iwa Surf Experience Day on Oahu on December The rest of the trip is up to participant and guest planning. We have various contacts for horseback rides, snorkeling, etc. if you are looking for recommendations on activities. ***Should the family choose to bring more guests with them, it will be at their own expense. Additional Terms Artwork Theme: The theme for the 2016 Cystic Fibrosis Awareness Month Art Contest: 1. Why do you want to surf with Mauli Ola in Hawaii this December 2016? We want to see what this topic inspires in you, whether it s through your own personal experience with the foundation, the ultimate wish of surfing in Hawaii, or your own amazing story. In addition to artwork, please describe, in 400 words or less, your artwork and why you want to go surfing with Mauli Ola in Hawaii this December Media and Technique Requirements. 2 b. Drawing materials (pencil, charcoal, colored chalk, pastels, oil pastels, colored pencils, markers, etc.) c. Collage materials (cut and glued papers, such as construction paper, tissue paper, wrapping paper, aluminum foil, etc.) d. Prints (handmade stencils, gadget printing, calligraphy, block prints, etc.) e. Sculpting materials (clay, papier- mâché, etc.) f. Original digital illustration g. Photography h. Video and cinematography i. Music/Lyrics/Poetry 3. Judging Criteria: A selected committee will review artwork submissions based on the following criteria a. Interpretation of the theme: Execution of style and clarity of intention b. Written Submission: clear, well thought out, and organized c. Originality: Does the submission contain original and unique material? 4. Copyright Law: Any artwork that depicts copyrighted images will be rejected. For example, no artworks depicting characters from television shows, movies, video games, or books will be accepted. All artworks must be original and created entirely by the participant. 5. License: By submitting an entry, each participant hereby grants to MOF and its affiliates, licensees, and assigns, an irrevocable, perpetual, and royalty- free right to use, reproduce, edit, display, transmit, prepare derivative works of, modify, publish, otherwise make use of the submitted entry in any and all media, whether now know or hereinafter created throughout the world and for any purpose. In addition to other things, the rights granted to MOF include but are not limited to the right to censor, compress, edit, feature, caption, affix logos to, and to otherwise alter or make use of the submitted entry. 6. Single Entries: One entry per participant. 7. Minors: in order to be eligible, this form must be signed by participant and parent if participant is under the age of Lost or damaged submissions: The MOF is not responsible for late, lost, misdirected, or damaged submissions that are not received in a timely manner, or are lost due to mail errors, computer or electronic malfunction or other error, or due to inaccessibility of the Internet. 9. Content: Any submission that includes inappropriate content, such as offensive information or images, adult material, inaccurate information, or information that might lead or attempt any person into unlawful or dangerous behavior, will not be eligible for participation in the contest. 10. Privacy. Information provided to enter the contest will not be sold to third parties but may be shared as may be necessary for the purposes of conducting and judging the contest and awarding prizes. MOF, its affiliates, and companies supplying prizes (or parts thereof) in connection with this contest may, from time to time, contact entrants concerning the contest, and each entrant consents to receiving such communications by Winner documents: a required signed affidavit of eligibility, HIPAA authorization, and liability/publicity releases will be required. If a winner fails or refuses to sign and return these documents within five (5) days of prize notification, the winner may be disqualified. 12. Same Start and End Dates for All Entry Methods: This contest begins on May 11, :01 a.m. Pacific Daylight Time (PDT) and ends on June 29, 2016 at 11:59 p.m. PDT. The start and end dates for each entry method are the same (e.g., online and regular mail). Entrants who mail in their entries must have a postmark date no later than June 29, 2016 for eligibility. 13. Not Transferable; Taxes and Fees; No Cash Option. Prizes are not transferable and not assignable. Winners shall be solely responsible for all taxes and fees or costs associated with any prize, including but not limited to any federal or state or other income tax, ground transportation (except as expressly stated above), meals, gratuities, personal expenses, and any other expenses not expressly specified herein. Should the winner choose not to accept the prize, there is no cash option and the prize will be forfeited. 14. Limitation of Liability. MOF assumes no liability, and shall not be liable, for typographical or other errors in the offer or administration of the contest, including, without limitation, errors in the printing or display of the offer and official rules, selection, notification, and announcement of the winner, or distribution of the prizes. 3 internet users or by any of the equipment or programming associated with or utilized in the contest, and MOF assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft, or destruction or unauthorized access to any systems associated with the contest. 15. Release. Acceptance of a prize constitutes the prize winner s permission for MOF to use prize winner s name and likeness for advertising and/or promotional purposes worldwide and in all forms of media in perpetuity without further compensation or authorization. Participants agree that MOF and its affiliates, and their officers, employees, agents, and representatives (collective, the MOF Entities ) shall not be responsible for any losses, damages, or injuries of any kind resulting from participation in the contest or from participants acceptance, receipt, possession, and/or use or misuse of any prize. Participants also agree that the MOF Entities have not made and shall not in any manner be liable for any warranty, guarantee, or representation, whether express or implied, with respect to any prize, including without limitation, the prize s quality or fitness for a particular purpose. MOF reserves the right in its sole discretion to disqualify any individual who is found to be tampering with the entry process or the operation of the costs, to be acting in violation of these Official Rules, or who otherwise takes actions that do or are intended to disrupt or undermine the legitimate operation of the contest. 16. All participants are treated equally: MOF will comply with all applicable laws prohibiting discrimination based on race, ethnicity, religion, color, sex, gender or gender identity, gender expression, sexual orientation, national origin, ancestry, citizenship status, veteran or military status, marital status, age, physical or mental disability, legally protected medical condition, genetic information or characteristics, or any other category protected by applicable federal, state, or local laws. 17. Governing Law. All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules shall be governed and construed in accordance with the laws of the State of California without regard to the choice- of- law provisions thereof. 18. MOF Final Determination. The determinations of MOF are final and binding with respect to any matter arising out of the interpretation or application of these Official Rules. 19. Winner Confirmation. 5 SUBMISSION OF ARTWORK ONLINE 1. Print out the above Official Rules: read and sign if agreed to all of the rules and regulations, and check the box at the bottom of the form. This is to be submitted in same with a scan or pictured photograph of participant artwork 2. Create a digital image of your artwork by either of these methods: a. Scan your artwork. b. Take a digital photograph of your artwork. Save your digital photo at the largest size possible that does not exceed 1 MB. For example, a 5" x 7" digital photograph of your artwork should be large enough for judges to see detail while still being smaller than 1 MB. If your digital image is larger than 1 MB, you can use a photo editing program (such as Photoshop or Microsoft Office Picture Manager) to resize it by either lowering the resolution (DPI) or by shrinking the image dimensions. You can also change the settings on your scanner or digital camera to create an image with a lower resolution.
