When completed, please fax, post or this form to Centaur Software using the details below. What is Database Configuration? Database Configuration is the customisation of Centaur applications specifically for your practice. The information you provide in this form will help us to modify your Centaur application to your specific needs. Information Regarding Computing Environment Where Centaur Application is referenced, it refers to either of the following products: Dental4Windows sql Version 12 Practice Studio sql Version 12

These will help us to manage issues that are not software related and require either an on-site visit by your technician or require a specialist to resolve. Full Name: Contact Number(s): Important Centaur Software is not responsible for your Windows or hardware issues such as printers, backups, networking or any other similar issues. This is the responsibility of the person or company that supplied that equipment in the first instance. Centaur Software does not setup any form of backup tools for your data. All backup processes must be setup by your computer technician. However, for our customers who are on full Support & Maintenance agreements, if time allows we will spend minutes looking into the issue (or longer if we think we can solve it in a reasonable amount of time) at no charge. If we cannot resolve the issue we will pass it back to your hardware technician. We will then move into a management role, where we will contact your technician on your behalf and explain the issue, follow up and assist with information/experience if possible. Centaur Software is not responsible for any costs billed to you (or us, on your behalf) by your hardware technician or any other third party. X-ray Software Technician Details If you intend for your Centaur Application to link to an external X-ray/Imaging system, please provide the contact details for the technician or sales representative that is responsible for this system.
